I am S-OFF and Unlocked with Stock recovery. I need to send my phone back so I want to go back to Locked and S-ON.
Here is my current setup.

I tried Weaksauce and it says root failure. I am trying BMT626 method and it is saying permission denied so I am assuming I need root. I am trying to avoid the RUU method since I am on stock recover and stock rom.
Can anyone help?

Towelroot did the trick
I figured it out.
I used Towelroot and was able to get root and reset secure flag and relock.

So, I followed this guide and ended up with S-on, Locked Bootloader, and a fully functional phone. I then attempted to unroot using supersu's "full unroot" option, which worked for a temp un-root, but immediately re-rooted when rebooted. I then downloaded the Universal unroot app in hopes that it would work. I opened it, granted it SU, and let it do its thing. and I am STILL rooted. Does anyone have any ideas? I've tried manually deleting the su files in system/bin/su and system/xbin/su and system/app/superuser.apk. No luck. Please someone help me get this phone 100% unrooted. Is there any way that I can manually unroot and configure the stock kernel to NOT re-root on boot? Also, I have tried to get firewater s-off to work, but weaksauce thinks I am alreeady rooted, and the already rooted instructions for firewater fail repeatedly. I am seriously out of element at this point.
I have an AT&T HTC One M8 thats fully rooted. S-Off, unlocked boot loader and rooted stock sense ROM. Lately its been causing more trouble than its worth (just being slow) and I want to fully unroot it and turn it back to a factory state. Its still android 4.2.2 and I want to unroot it so I can get the lollipop update. I've tried a few times before and when I get to the point where I'm supposed to flash something in recovery I get the "md5 mismatch" error. All I want is for my phone to be unrooted and back to normal. If anyone can help, thanks!
And I also paid $25 for s-off on this phone so if I could keep that without any trouble, I'd like to know how to do that too.

You don't need to unroot. You can just restore to stock and keep root and S-Off.
Go to this thread, and search for the appropriate TWRP backup and stock recovery for your phone model.
The thread is new, so if the version you need isn't linked yet, ask for it, and @ckpv5 will provide the link for you..
Can anyone point in the right direction in rooting the m8 without breaking the warranty or unlocking the bootloader?
Rooting without breaking warranty is what we all dream off. Want to bring the phone back to 100% stock you need s-off so you can return bootloader tampered flags back to normal.
S-off is only available via sunshine !
ok have learnt basiclaly sunshine need to be done to unlock the bootloader. However sunshine needs root access to work and it says come here to find out how to get root access. I personally dont care for soff all i want it root.
What are the methods to gain root. Dont care about soff.
is htcdev method the only way? As htc would have a permanent record you have requested your code. Doesnt seem like a great idea imo. Are there any other methods to gain root?

Bugger all about rooting. So it would seem it shouldn't break the warranty, or am i missing something?

For previous OS version, there was an option in sunshine to temp root, s-off, and unlock the bootloader. And from there you can install custom recovery and perm root.
But no such temp root exploit has been found for newer (Lollipop) OS versions, unfortunately.
I really hate the idea of having to unlock the bootloader just to root and also giving your imei to htc. The HTC looks like a complete ball ache to get back to stock also.
I was wondering if the phone can be rooted with htcdev then s-off ed with sunshine. Tamper flag removed, recovery restored and s back on and bl relocked, whilst leaving the phone rooted. Essentially making the phone look stock but rooted and then a factory reset should wipe out and unroot the exploit?

Nah doesn't work after son modified banner in bootloader comes back. is there anyway to keep root with son and the banners all stock so locked with genuine banner? Perhaps running su as a daemon?

I know lots of threads about different ways to do it, but if I could, I would like to get a step by step on what I need to do.
I was rooted and unlocked, never s-off. I unrooted, relocked, and flashed stock RUU, but it still says relocked and software modified in bootloader. How can I get rid of all of this? What I want, if possible, is S-On, unrooted, locked phone, that doesn't save any of if anywhere and you cannot see anywhere on the device? It looks like I might need to re-root and unlock to do this, which I'm fine doing, if I can achieve my end result.
Is there anyway to do this? Thanks in advance!
Yeah, use SunShine to gain S-OFF after actually you got SuperSU installed back..
