[Q] Wiped /sdcard and /system - only have CWM now. How can I proceed? - Samsung Galaxy Nexus

I had an older custom ROM and CWM 6.0.10 and I wanted to update to a newer 4.4 ROM (Omni) so I booted into recovery and proceeded to do a factory reset along with a format of /sdcard and /system. Now when I reboot, I enter recovery automatically.
I managed to push the ROM .zip to the phone using "adb push ROM.zip /sdcard/ROM.zip". The problem is that when I try to install from zip (withing CWM) the install fails with this message:
SymlinkingApplyParsedPerms: removexattr of /system/addon.d/50-hosts.sh to 0 failed: Operation not supported on transport endpoint
script aborted: set_metadata_recursive: some changes failed
set_metadata_recursive: some changes failed
E:Error in /sdcard/omni-4.4.4-20140825-maguro-FML.zip
(Status 7)
Installation aborted
I'm stuck here and I'm guessing that I shouldn't have formatted /sdcard and /system as the directory "/0" that the error message refers to isn't on /sdcard anymore (it used to be). Not sure what to do at this point.
I've tried flashing TWRP, but fastboot waits forever with a "<waiting for device>". I thought TWRP might let me sideload the ROM (I was thinking that this manner of installing might create some default partitions on the SD card, as needed.
I welcome any suggestions. BTW, this is all on a Galaxy Nexus GSM

[Q] need help flashing ROM

I have a new evo updated. Rooted with unrevoked and running sprint lovers (PC3IMG) I installed clockworkmod and tried to flash a rom and had problems. I thought that clockworkmod was my problem so I flashed back to my RU recovery. I then went into recovery and wiped data, dalvick, and cache. I then chose install zip from sdcard and then I selected the new Fresh ROM zip. I tried to install it and it goes all the way to Writing boot.img and I get an error that says
assert failed: write_raw_image("/tmp/boot.img","boot")
E:error in /sdcard/fresh-evo-3.5.0.1.zip
(status 7)
Installation aborted.
So then I tried to install Myn’s Warm TwoPointTwo and I get to copying files and I get this error:
E:Can't symlink /system/xbin/[
E:Failure at line 15:
symlink busybox SYSTEM:xbin/[
Installation aborted
I am stumped and dont know what to try next. Anyone know whats going on with these errors?
Just tried to install pc36img from sdcard and I get this after it says installing update:
E:Can't find update script
Installation aborted.
I think that this will only work if I flash it at Hboot and select yes for the update? Either way, I would rather have Fresh as my ROM because I used it on my Hero and I loved it. Any help?
After reading other posts, many say it has to do with the HBOOT size not having enough space??? Its not seeing the boot.img correctly. I have no idea. Anyone have any ideas?

[Q] Stuck in Recovery, can't flash ROMs

Decided to resurrect my old Verizon GNex, and found that it will only boot to TWRP recovery. Was able to successfully sideload a ROM zip, but attempt to flash produces the following result (copied from phone screen, site won't let me link screenshot):
Formatting /system
Extracting /system
Symlinking
set_metadata_recursive: some changes failed
E:Error executing updater binary in zip '/sdcard
Error flashing zip '/sdcard/sideload.zip'
Updating partition details...
Failed
Any ideas on how to proceed?
First copy ROM on sdcard, then wipe all except sdcard, and then try.

Re
Maybe first of all you two write the steps performed for flashing Lollipop Roms? I've tried all the Lollipop Roms compiled for SXS and nor, ally I used to do this way:
1. TWRP recovery (but CWM also worked)
Install TWRP recovery in FOTA partition using Terminal apk provided your phone is running on 4.1.2. kernel supporting TWRP recovery. Flash kernel from ROM zip file prior entering TWRP (this step helps avoiding an error like "update binary...").
2. In TWRP recovery - Advanced - wipe everything but sdcard. In Mount section unmount everything but sdcard.
3. Flash the ROM and wipe Cashe&Dalvik
4. Flash appropriate GAPPS, wipe Cashe&Dalvik and reboot.
that's all

error executing updater binary

trying to flash a cyanogenmod rom onto my rooted phone with TWRP.
Everything okay up to the point on installing the new rom (a zip file).
here's what i get as an error message:
***********
E:Error executing updater binary in zip '/sdcard/<rom_name_here>.zip'
error flashing zip
updating parition details...
...done
****************
I've tried with all the recent cyanogenmod builds for the htcone m9 and I get the same error everytime. Even when i try to sideload from my computer in adb.
I've also seen this error in other forums and they have suggested doing certain wiping procedures and reboots, but I've tried them all and I've gotten nowhere.
any ideas?

Failed to mount /cache; no such file ora directory

I just had a custom rom, and I don't like it, I decided to replace another rom, and when I wanted to enter twrp with volume up + power, it didn't work, then I factory reset, and restarted, I entered twrp, but twrp lost, and want to install twrp again, but it can't, the device is not detected, and I tried to format the system partition, but it can't with the words "E: [libfs_mgr] skipping mounting '/ dev / blok / bootdevices / by-name / cachce'
and I tried to enable adb it also failed with the words "E: infotify_add_watch failed: no such file or directory
