After more than 1.5 yrs of using my Note 2, recently I noticed that sometimes the camera focus gets stuck. It seems to be at macro focus distance. So everything close to the lens will be clear, and things beyond that blurred. There is no way to refocus. No matter what I do (using manual touch focus, setting to autofocus, setting to macro focus, setting to video mode, killing the camera app or restarting the phone) it is stuck for some time. And then it just starts to work again by itself after few minutes. Completely randomly it seems. I'd say it gets stuck approx every 5th time when I turn the camera on.
If I look at the lens, while the focus is stuck, the lens, naturally, also don't move as they move when the focus is not stuck.
What could this be? My bet would be an issue with camera, but I'd like to exclude any possible known issue, before I RMA it. I still have few months of warranty. Normally I would be doing the factory reset, but I was kinda waiting for the 4.3 or 4.4.2 update, and fresh flash.
I'm running stock 4.1.2 (still the latest official for me), and it is rooted, and triangled away. Everything else is stock. I use stock camera app. Also I don't seem to have any other problems. When it does focus, the camera produces sharp photos. SO no problems there either.
PS The lens are clean and undamaged, as is the whole phone.

I'm on CM11 nightly, which did at one point have problems with the camera. However it had been working fine for a while. I don't remember doing anything to my phone (no upgrades, drops, etc) but the camera just stopped focusing.
I've tried ALL the setting in the stock and several other camera apps.
I tried wiping.
I then installed jelly bomb which (I believe) is based on 4.1 stock.
Still won't focus.
It seems to be stuck in a macro focus, I.e. when the camera is 2 inches away from an object, the object appears in focus.
Anyone seen this or have an idea?

Also, like I mentioned, I had wiped and gone back to (4.1) JellyBomb just to try the camera and it didn't focus there (eliminating 4.3/4.4 AOSP camera issues).
It had been working fine with CM11 nightlies (after the initial issues you referred to were solved).
I'll keep trying stuff, but I'm starting to fear it's a hardware issue.

The issue with snapchat, is that the front facing camera distorts whatever it's looking at. For instance, when taking selfies with snapchat my face is distorted in the form of a stretch. I'm not sure if this is a snapchat issue regarding the galaxy s3, or a cyanogenmod issue regarding my phone.
PHOTO QUALITY
As for photo quality, well this is a much bigger issue. Let's start with the problem: The camera (using any rom, stock or not) is creating vertical colored lines in photos containing, but not limited to, low light environments.
I had my phone replaced and when I got it back and flashed cyanogenmod I started noticing the issue. I reverted my phone back to stock uprooted to see if the problem persisted, and it did. I took my phone to the sprint store, and they replaced the camera in my phone while keeping the rest of the phones body. I took a few photos using stock 4.3 touchwhiz, call it what you want, and I didn't see any issues. Note, it was only a few photos that I took. Eagerly to opt. out of Touchwhiz, I rooted and went back to the same version of CyanogenMod I was on. Photos are still ending up the same, with vertical lines of all colors disrupting the picture.
In addition, the flash makes a noise with the stock cyanogenmod camera app, almost like the flash could be malfunctioning. Snapchat pictures with the flash work fine. When taking a photo with the flash on and my thumb covering the flash, the photo worsens with the issue. (more defined, and more quantity lines)
Any ideas, comments, questions and concerns are all appreciated.

i don't use snapchat at all but does it use the stock cm11 camera for its picture taking? cause I got a few fixes for the cm11 camera cause their cameras ALWAYS gave me problems and crashes...i'll link you over, then you can tinker away and see what's what:

i have a big problem with the duo Camera every time it says the lenses are blocked but my hands are far away from the lenses. i tested a lot close up shots same error message, outdoor shot with a lot of light same message.

I'm having the same problem..
I just got my M8 today, played with it for a couple of hours before unlocking the bootloader. Restored titanium backup after unlocking, everything was fine until I opened the camera. I keep getting the "duo camera blocked" warning, I tried taking pictures for an hour straight but the warning persists and I can't use U-focus or any other duo feature on every picture I take.
I'm on stock ROM and recovery and unrooted. Tried to restore factory settings, no success. Just updated to the newest firmware (1.54.401.10) via OTA but still no working duo camera.
Did you find a fix for it or did you send it back for repairs? I'm a little frustrated because I unlocked the phone before noticing the camera problem...
I have the same problem
I got a warranty replacement and I'm finding the camera cannot focus at distance. Up to 1 foot it focuses great, but anything farther is a mess. Even with manual focus at infinity it doesn't work.

What about with video ?
Give the lens a clean. Take a good look at it and see whether some dust got trapped on the inside. Give it a tap and see if it moves.
With the camera app closed and not in memory try clearing the camera app cache and restarting. If that does nothing try clearing the camera data too.

Same thing with video. I had read about clearing cache/data, so I tried that as well. Even tried factory reset. It seems this camera just cannot set focus at infinity. Used several other camera apps with the same result.
Same here .. Got a warranty replacement for my bricked G4 (boot loop failure) and the new phone has serious camera focus issues. Will not focus on infinity and even at short distances has grainy unfocused pics intermittently. My old G4 camera was amazing even at infinity on max 4.0 zoom super clear pics. This one not even close. Video is the same. Very disappointed. Maybe I need a new Camera module? Any advice would be helpful.
I feel like I need to see what sprint ir LG can find out but sure they will say it works fine but I know better. Arrg!

Did you get a solution for this problem? I have noticed the same situation, focus for close objects, macro, works amazing, but for far objects it's out of focus, I notices for stock and 3rd party apps. I've tried Google camera, Stock camera, LG G5 camera, LG V10 camera, Pro Shot, Camera FV-5, Open camera, this issue also happens for video. I start to suspect that issue it's hardware.
Any suggestions?
I have the mi max for a few month, suddenly, one day, I had this problem when I tried to focus on close object, the focus got stuck on the close object, and when I try to focus on some object in different distance It couldn't change and got hot.
My guess is that this is not a software fail cause I tried full wipe. I even installed a fresh stock. I also replaced the camera part but even this didn't fix the problem.
If anyone have faced this problem or have any idea it would be just great.
