Alright folks, here's what I've got:
An S3, which is rooted, using TWRP for recovery and on a CM11 nightly. This had been running happily with no problems since February.
About a week ago, it began randomly shutting down / restarting for no apparent reason. It would do this maybe once a day.
Two days ago, it did this again while I was surfing the internet, but then proceeded into a boot loop and now will not start up at all. It shows the Samsung screen, then the Galaxy S3 screen, then stops and loops on the CM animated screen. As far as I know I didn't do anything to trigger this - no new app downloads, no new ROM flashing, etc - it just did it suddenly in use.
I can successfully get into both download mode and recovery and the phone will stay there (so I don't think this is the power button issue), but I can't do anything useful from either mode. In TWRP I get error messages regarding failure to mount cache regardless of what I do. I can't do a factory reset or wipe it, and can't flash anything.
It will connect to Odin in Download mode, but anything I have tried there comes up "Fail", or "Complete - Write Operation Failed". Can't flash back to stock, can't flash CM again. I've tried a couple different cables and different USB ports, and reinstalling the drivers. The computer in question, which has worked with Odin before, is running Win 8.
I'm at the end of what I know to do to this thing, and am beginning to suspect it has a hardware problem. Any thoughts or things to try?

This does sound like a similar issue. However, since I had been running the same ROM for almost 2 months with no problems, I thought I was safe from ROM-related issues.....
I've now tried a couple different versions of Odin and they all fail, though 1.85 gives me more info (much like you saw).
Have you tried removing sim card and sd card from the phone before going into download mode and plugging to computer? I did this when I almost bricked my phone and I was able to flash factory firmware with Odin 3.07. From now on, I'm staying away from any Nightly build...

Yep, I tried pulling the SIM and SD card. No change in behavior either with or without.
If you are not running a stock 4.3 bootloader, could try using Odin to flash a stock ROM from sammobile.com. If you have the 4.3 bootloader, this could be causing the failed flash. What is the screen info when your phone is on download mode?
Have you tried using Philz Touch to flash?

I've tried to get Philz on the phone and that fails, as did trying to flash stock from sammobile. I'm getting a write failure in Odin.
Also just tried to install Philz from within TWRP.
This goes through and says "Successful", but then ends with:

Selecting Wipe Cache/Dalvik results in:
Failed
Formatting Cache using make_ext4fs function.

Try flashing Philz for the d2att instead and see what happens. I have used the d2att and d2lte version on my Canadian Bell i747m.

Hello, my note II died one night. Either displays battery icon when charging or stuck on samsung logo. I can get in to odin recovery but cannot get in to bootloader mode. Device is not rooted and I did not turn on usb debugging mode earlier on. And now is the question. What do I do to recover pictures from it?. Is it possible at all?

Do you have a custom recovery? If so: make a data backup.
Otherwise: try rooting and recovering using download mode. (If it still works)
There are many tutorials on rooting (with download mode).
Question: How long is it stuck on the samsung logo? Do you have a bootloop? You are not rooted, but are you on a custom rom?

It is not rooted. I have stock ROM and dont have any backups. It shows samsung galaxy note II for ever or sometimes battery logo but it frozen and dont shows its charging. Cannot go into bootloader mode just into odin mode

And with odin mode you mean download mode? If so: Flash SU via odin, flash recovery, make a backup, flash a ROM. Most data will be in the backup, If you flash the ROM without wiping you still have all of your pictures and stuff.
Edit: I see you did not turn on USB debugging, however, as far as my knowledge goes, this is your only solution...
There is a way to push it to your device though (ADB), but I think it doesn't work without USB debugging...

Try to reflash stock rom using Odin. Flashing stock roms probaly keeps ur apps and data safe.. Unless you to a factory reset.. U can flash stock rom without triggering USB debugging

Have you tried using kies? Isn't there an emergency firmware package or something like that. If your not rooted kies is your best bet

Try emergency firmware with kies if that doesn't work get the pit file from mskips toolkit. I think you have to go to the website for it now. skipsoft.net

finally I got PIT file for note II, cwm6 with root and stock ROM but...
odin3 stays forever on: get PIT for mapping.....
tried just PIT but same effect. Without PIT file it says: no PIT partition, flashing failed.
toolkit does not talk to the device as waiting for USB debugging mode on the device. even if I choose download via odin still the same.
Any ideas?

Is it possible that even if I can put device in the DL mode nothing can be done?
I tried kies on different PC. PC can see only USB camera and USB composite device and Kies stays forever on: connecting to device.
I tried my second device and kies recognized it straight away. Anyone have any ideas what is going on with my note II?
As I mentioned before I can put it into DL mode but it looks like its frozen just after doing that.
Any help will be appraciated.

still trying different things as no replies on my post.
when I leave my device connected to PC (switched on as normal but showing only samsung logo) after 5 mins PC says found USB device, device not recognized. then after 5 seconds it disconnects. and it goes in circles. same if I try in bootloader mode, but it stays on samsung logo as well. In DL mode, PC recognizes only composite device and modem.... Any ideas please? I dont want recover data anymore as I think its lost but I would like to unbrick my note2

Using ODIN, select the PIT file, and make sure to TICK the Re-partition option.
I hope you already have a stock ROM downloaded. In the PDA tab of ODIN,select that rom and then press start. Wait and pray.

Are you sure the repartiton option was ticked? I seem to have forgotten whether it should be ticked or not.
Do one thing. After loading the necessary files, tick re partition.

If I choose PIT file, re-partition box is ticked automaticaly. W/o re-partition box ticked, I have message: no binary file selected.
As I mensioned before, In DL mode, my device is not showing model number. Does it matter?
Before I tried all possibilities and suggestions on Win 8 PC, now trying on win 7 and I have more promising results, however I will try on newly installed system just to be sure that drivers and other stuff are ok. I tried different USB cables and different batteries. Still nothing, so hoping new system will help. except, anyone has different idea?
Ok, so I have done some research and even if I can put n7100 into DL mode but there is no product name displayed, device needs either new EMMC, or new PCB or JTAG. I will go with JTAG as my first option and will report as it might be usefull for someone in the future
I Have tried everything....
I am at a loss here.
First of all, I got into a Bootloop/Soft Brick type thing.
Tried to reset using Kies and that just failed, now when I turn on my phone all I see is "Firmware Upgrade Encountered an Issue. Please select recovery mode in Kies & try again.

*Sits at 0% for ever*
So then I try Odin,
I download the .tar of the stock firmware from Samsung-Updates (.) com when I flash I get a message in Odin that is "FAIL"

If you have the i747, do you have a 4.3 bootloader on it? If you do, you cannot just flash a stock ROM via Odin. Repeated attempts to do so may brick your phone.
You can either download the necessary 4.3 Odin files from XDA or flash a custom recovery in order to flash a stock 4.3 ROM using your custom recovery.

I have an I747 and I've already tried to flash a custom recovery (TWRP) But it just shows the TWRP title screen and then goes back into a bootloop.
Im kind of a noob at the Odin thing, I've only used it to root so i'm not sure what bootloader I have but I assume it's a 4.3.
Even if I do have a 4.3 Bootloader, where can I find such a ROM?
Did you try flashing Philz Touch via Odin? This is what I use on my i747m. You can flash the latest tar version from here via the PDA box in Odin: http://goo.im/devs/philz_touch/CWM_Advanced_Edition/d2lte
In Odin, do not check auto reboot, only F.Reset Time. Once you see the word Reset in the status window, remove the USB cable, pull the battery, insert the battery and boot into recovery, wipe cache, data, and Dalvik. You can copy a custom ROM (I use Validus 13) to an sd card and use Philz to flash it.
So do I download the tar.md5, if so I just tried and in Odin it says MD5 hash is invalid.

Thank you so much, I made it into recovery! But just one problem, when I click Wipe data/Factory Reset the screen freezes and then it goes back into the bootloop. Could this be because I have no SIM or SD card inserted?
The freezing could be something else.
Did you try formatting the system?
I recommend doing a quick format before copying the ROM onto the SD card.

Yes, try installing the rom without a reset. Theoretically, installing a new ROM should overwrite the existing ROM. If there are any files in the data or cache, this may prevent the ROM from fully booting.
Does Philz recognize the phone's SD card slot?

When you boot into download mode, what does it say on the screen? If you let me know what it says, I'll be able to tell if you have the stock 4.3 bootloader.

Your phone has the 4.3 bootloader and I recommend against using Odin to flash any ROMs.
If you want to restore a 4.3 stock ROM, use a method from the ROM development section.

Hello all, all of a sudden my phone start booting in to CWM with the error message:
E: Can't mount /cache/recovery/command
E: Can't mount /cache/recovery/log
E: Can't open /cache/recovery/log
E: Can't mount /cache/recovery/last_log
E: Can't open /cache/recovery/last_log
With any attempts to restore/wipe data, I get a similar message saying it is unable to mount. Now, I am unable to boot my phone even to the CWM menu since it keeps rebooting on its own and sometimes it won't even go into the CWM menu. I have looking up this error many times and I have been unable to find a solution that works for me.
I have tried wiping the cache from CWM, wiping all data, and nandroid data, all of which ended in a error message except wiping the cache which did not solve the problem.
I have also tried this : http://forum.xda-developers.com/showthread.php?t=1284196 but could not get my computer to recognize the device. If anyone could help or provide any suggestions, it would be greatly appreciated.
Try flashing firmware via Odin. Uninstall/ reinstall drivers if connection issues.

I have tried this and when I hit Start in Odin, I get the message:

and a FAIL! in the top left.
If kies is installed uninstall it. Does odin go through the md5 check? What is your current firmware build and what build are you trying to flash? Also uninstall drivers through device manager. Then reinstall.

My phone doesn't boot up anymore. Goes straight to CWM and reboots itself. I don't remember what the current firmware build is and I'm not trying to flash anything. My phone just decided to turn off one day and won't turn back on.
Do you at least know if you updated to 4.3? If so, you can only flash the same or newer firmware from now on. So when you tried to flash with odin, if you used older firmware it would fail.
Can you boot into download mode? Hold volume down, home and power.

Booting in downloading mode was working at some point by it isn't anymore. I don't think I have updated to 4.3, it is most likely 4.1 or 4.2 and I have tried flashing 4.3.
I suggest trying to use the debrick.img from NC2 then. Its the latest available so it should work regardless of what you previously had. If you do get download mode this way you will have to odin flash the NC2 firmware.

I'll try to keep this short. I have an ATT GS3 i747, was running Slimkat 4.4.4, my Bootloader & baseband were "I747UCDLK3." I also had PHILZ recovery
I wanted to go back to a reliable stock image where everything didn't crash. It's late now but this is what I believe I did:
-Went to this thread and downloaded the ROM and the bootloader "Touchwiz KITKAT 4.4" (KT747-TW-KK-4.4-ATT-05-13-2014)

-I'm not sure which I did now since it's late, but I believe I flashed the above ROM and bootloader. After this my phone could still get to Download mode but not boot. So I flashed the below STOCK Restore rom.

-At this point it wouldn't turn on. I used Debrick image "SGH-I747U-CUEMJB3" and was able to boot into the stock recovery or download mode.

-Downloaded all the files from that thread, started by flashing TWRP "openrecovery-twrp-2.6.3.0-d2att.tar"
-Now my phone boots into stock 4.3 and works fine but only boots with debrick SD card. I can still get to download mode but can't get into a recovery mode. It will say "booting to recovery" but just boots normal.
-Looks like in the process of all these mess ups I flipped the warranty counter to 1. Wasn't really planning on selling it right now anyways.
So any advice on what I should do next to get it to boot normally without the SD card in?

-Build: JSS15J.I747UCUEMJB
Maybe your version of TWRP is too outdated. Try flashing TWRP or CWM for the d2lte.

Yeah I had PHILZ before and really liked it but I think it kept failing to flash.. I'll redownload and try it again.

okay I'm able to flash these without any problem but I cannot boot into recovery. It says booting to recovery... Then it goes black for a few seconds and will vibrate and boot normally. I believe it's because of the Debrick image its booting from? I still can't get it to boot without the debrick image.
Do I need to try flashing the MJB Kernel/bootloader?
Can you get phone into download mode without the SD card in the phone?
I've never had to restore a bricked phone so I'm just guessing here.

No :/ It doesn't even turn on without the SD card in.
With the SD card out, i can plug the phone into the computer and the red notification light will come on. If I then put the battery in and hold the power button down, the red light will eventually go off (like it's turned on) but the phone stays blank.
I can get to Download mode with the SD card in and remove the SD card though, Once the phone is on I can take the SD card out and it still works fine.

Yeah.. The Kernel is "kernel 3.031-2024954" Is that correct? Not sure what to do lol

Maybe the partitions are messed up? Since you can get into Odin, try flashing a stock recovery and see what happens.

Okay so I tried flashing that w/re-partition enabled. It flashed but nothing changed... (I realized though that I still had the SD card in when I flashed)
I reflashed without the SD card (and re-partition on) but it failed .

Before I turned it off, I just flashed it one more time (only auto-reboot checked) and it completed, but won't boot on its own still
Where did you get the rom? I see that there was no aboot.mbn included.

I clicked your link and saw these instructions on the same page.
In the thread you sent me, I didn't a stock recovery file I could flash with ODIN. Seemed like everything needed to be flashed with in the custom recovery that I cant get to.
Tried a different de-brick image, didn't work. Tried a 2nd one, worked but then my phone said unknown baseband and unknown IMEI (maybe wrong version?)... Trying to go back to the one I had been using and now that's not working! This phone is driving me crazy
not buttin in audit13 , but havent some people had to try different SDcards and maybe multiple times ?? (p.s. what the heck is "aboot.mbn" ?). excuse newbness please.

Yeah I have 2 SD cards with me. The one that has always worked is a Sandisk 16gb class 4. I have a Samsung 16gb class 6 but it has never worked when I write a de-brick image to it.
I should probably just stop trying things because I seem to be making it worse. I will try loading the de-brick image that HAD been working great for me again but i've already tried multiple times. I had to go back to the de-brick image that messed up my IMEI again as it's the only one that works now.
I got into download mode (which looks slightly different now (doesnt show the flash counter anymore). I tried starting over and flashing the 4.3 rom through ODIN again. It failed, trying a 2nd time now. Might be failing because I took the SD card out? Was going to try flashing 4.3 and then going back to the 4.3 De-brick image before it loads. This may accomplish absolutely nothing lol, just trying to get back to where I was so my phone will have connectivity

If any of this is making since, the 2nd reflash of stock 4.3 ROM worked fine. But I can't get it to start up with the MJB de-brick image yet.
Edit** Yeah what I did, didn't do anything.
I've borrowed another phone (An identical ATT Galaxy S3) so I have a phone for now but would still like to try fixing mine. I guess I'll do some more research unless someone has a suggestion. Unless I didn't see it, I couldn't find an ODIN flashable stock recovery the other day.

I've had mroe time to mess with the phone. I've got it booting without the SD card now and can get to recovery I successfully flashed TWRP with Odin now and can get into it fine. :good:
Now I just need to figure out which rom/stock rom to load and hope I dont have to start over again
Hi, I've digged through much of xda and not found a satisfying answer. My phone boots into download, is detected by odin (7,8,9,12 all ok), and whatever I do, odin says it was successfull. I tried flashing the stock ROM, the official ROM with 4.4.2, pit file, all in vain. Before crashing my s4 was running cm13 with twrp3.0.2-1. When turning phone on, it just displays "Samsung GalaxyS4 mini / GT-I9195 label on black screen, with "set warranty bit: kernel" in upper left corner. Download mode boots ok, but recovery - twrp 3.0.2-1 logo is displayed for about 8 seconds, and then phone reboots to "normal" mode - displaying the logo.
It would be great if some of you had an idea of what to do as I've been trying on my own for 3 weeks now and nothing changed for better.
As there are people looking at this thread, it would be great to hear from you! I still did not succeed in flashing anything, twrp screen shows for 8sec and then phone reboots to normal, which is samsung splash screen. I'm using samsung drivers, have tried different usb ports, used different computers, still nothing.
Did you try 4 files firmware?

Unfortunately I can't help you here, but I have a similar problem and it might be related to yours. I have also have a Samsung S4 mini GT-i9195 LTE. Originally with Android 4.4.2 but lately with no OS at all. And I can't figure out how to get one back. What happened, chronologically and (very) detailed:
I rooted my phone with KingoRoot, which worked fine as far as I can tell. When I didn't like the KingoSuperUser-App I installed SuperSU. But it failed to work, apparently because of Knox or something like that. I later wanted to flash my phone with a LineageOS ROM (lineage-14.1-20170505-nightly-serranoltexx-signed.zip) and therefore I wanted to install TWRP as custom recovery. As the usual way with Odin seemed a bit complicated to me at the time, I decided to install the "TWRP Manager" App, which went well. But as soon as I tried to install my ROM with it, it asked if my bootloader was unlocked. So, after searching the internet about that and it said that Samsung phones usually have an unlocked bootloader and only have to be rooted, I went on with installing the LineageOS ROM using the TWRP Manager. Mind you, I am not talking about the custom recovery that you have to boot in to by pressing Vol-Up+Home+On/Off-Button, I am talking about the app that you can download and just needs root permissions to do the same stuff. I had downloaded the ROM-zip onto my SDcard before and wanted to flash it from there.. Now, when I let the app go on, the screen just went blank. And this might be the moment I overreacted. I panicked and tried to start my phone again. But it only went as far as the Samsung logo and didn't go further and I had to pull the battery to turn it off. I then tried to boot into recovery mode by pressing Vol-up+home+On with no result, I then tried with Vol-down+home+On and found Download mode with:

After some research I downloaded Odin3 (v3.12.5) and managed to flash TWRP (2.6.3.0) onto my phone. And I found the ROM-zip file, but it would not let me unpack and flash it and it didn't help to uncheck the "Zip file signature verification." When I tried to flash I always got the same error massage:

And it gives a "failed" response. Now, I tried moving the zip to internal_sd and flashing with no better result. I also tried to flash a stock ROM which I downloaded from Sammobile.com. This didn't work either. The adb push method and the sideload didn't work out either.
Interestingly, when I go to "Reboot" -> "Power Off" to turn the phone off I get a dialog:
No OS installed! Are you sure you wish to power off?
-> "Swipe to power off"
-> then comes the next dialog:
Install SuperSU?
Your device does not appear to be rooted.
Install SuperSU now?
This will root your device.
-> "Swipe to install."
Which had no effect at all but to inform me that my phone apparently isn't rooted after all. I therefore downloaded the S4Mini_RootKit_v2.zip and tried flashing it, which was successful. But I still got the same dialog when powering off and I couldn't install the zip files either.
That's where I am now and I really don't know what to try or do next. As I could flash TWRP onto my phone, shouldn't it also be possible to flash an OS?
I hope this was a precise enough description. If someone has any further question, please ask.

Download a multiple Firmware from Sammy with PIT.file
Boot into Download mode.
Download Odin 3.07 or 3.09 not newer
Flash all Files with Pit.file
Reboot.

I posted this thread precisely because my recovery DOES NOT work. No option for factory reset. It just reboots after ~8 secs of twrp 3.0.2-1 screen. Flashed with ODIN 3.09, official intern. ROM from sammobile into AP and drive. google. com /file/d /0Bx2R0nz6jh17bllKenctWnVsWkE/edit <-- this PIT file. Also tried this pit with different versions of twrp - 2.6, 3.0, 3.1 - still no change at all, no recovery booting

But you are not tried a multiple ROM with PIT.file.
You flashed only a Single File ROM under AP.
On the multiple ROM you can extract the CSC File and than you have the PIT.
Pit you can only flash with Odin and a ROM.

Perfect! Worked like a charm for me! While factory reset itself did nothing to improve the phone, the other stuff worked. I flashed my phone with the .tar.md5 file I extracted from the .zip file (from sammobile.com) using Odin 3.09 and my phone works again, thank you soo much!
I then rooted it again and flashed LineageOS (lineage-14.1-20170505-nightly-serranoltexx-signed.zip) onto it. It worked but during the first startup I got the error massage "Setup wizard keeps stopping". I then tried the newest (20170519) version and got the same error again. Any Idea how I might repair this?
About the pit. file: what exactly is the difference between this multiple pit file, a "usual" pit file and the .tar.md5 file I used?
On a multiple ROM file is all separately, Bootloader, Phone , Modem and CSC file.
The PIT file is the partion File of the RAM. You can extract this seperat from the CSC file.
To the issue the the gapps. Try an older Version or flash first the LOS and setup WiFi.
Than flash Opengapps with wipe cache and dalvik than normally works the wizard.

i got the same problem, that the recovery is not working, flashing a multiple rom with pit doesn't help (neither recovery nor rom is working).
but i guess the main problem is, that i got a REV02 phone and I only can find REV01 firmware (e.g. this one here: http://firmware.smartphonewp.com/download-samsung-galaxy-s4-mini-gt-i9195-full-firmware-4-files/)
are there any REV02 firmwares out there, which maybe can help? or does anyone (@bierma32, @albaCross or @desolate_sorrow) got any other ideas?

Thanks for replying. I tried flashing the LOS ROM and booting without gapps. According to TWRP it was successful. But when I booted for the first time I got similar error messages as before "[any app name] stopped running". Just with several names and all of them one after the other. Most of them appear to be named something like .android.com with different pre- and suffixes.
I then tried a more actual version of LOS with a similar result (same error message, similar names). After several times I decided to go back to my previously created TWRP backup (of the stock ROM with root access and several of the more annoying apps removed, also I had checked all partitions when backing up): I did a custom recovery and recovered my backup file and it seemed to work, TWRP said "success". On booting I again had the now so familiar error messages on the screen. Same happened when I flashed the stock ROM and recovered my backup after that, or at least tried. While TWRP still said it was a success it would not get past the screen where you choose your language.
After this I gave up and went back to the stock ROM and "rebuilt" my old phone. Maybe my phone just doesn't like anything but the stock ROM, so I should probably just stick to it.
But, just out of curiosity, what exactly is the problem with the phone if neither custom ROMs nor system backups work on it properly?

this was me just 2 days ago!
i was on lineage os 7.1.1 with TWRP and was stuck wanting to go back to stock.
i flashed many many roms and they all failed in ODIN.
Eventually i found a Netherlands ROM that did not fail on the REV02 check - might be worth a go!

I own s4 mini gt i 9192
i have rooted it with kingroot when it was in a stock firmware, then i have decided to install lolipop rom with twrp with superuser in it, everything gone smooth i have used my phone over 3 months, one morning i woke up, the battery level was 43 %, i have set it in the charger returned back after 30 minutes and have seen that it only got 3% energy, pulled out the battery and inserted back, charged the phone for a while in a turned off mode, and powered it one, strangely it started but showed some kind of upgrading panel on the main screen than immediately it turned off and started to restart again and again, i have booted it in recovery mode, wiped the cache restarted the phone, main screen turned on to enter my password but in a seconds it went black again, opened it up in twrp recovery mode, went into advanced settings and wiped something in there(i can not exactly remember the name of the thing i have wiped but there was an option to wipe it in twrp advanced settings, it warned me "that my phone would not start correctly but anyway i have done it") after it's wipe phone started with s4 mini logo, and restarted like this over and over again, in recovery mode it only shows twrp logo no options panel to access to internal or external sd card, neither wipe or factory just nothing but twrp logo. I have flashed the 4 files recovery pack, (pit, ap,csc and etc,) but same it is never helped, i have used multiple recoveries, and the original stock rom from sammobile, but it has nor cure, i am a newbie on tech and xda please if you could help me, i have tried to post it to xda forms but i did not find a button to post a thread, thanks.
