Hello to anyone that can help,
I tried flashing [ROM][10/23/13][4.3] ParanoidAndroid 3.99-RC2 but then had no service at all. The baseband version was listed as unknown and the IMEI was also listed as unknown. I was able to restore back using my nandroid backup (after manually creating a new blank nandroid.md5 to fix the MD5 mismatch error). I'd like to try again, and flash the modem if it fails, although I read the modem firmware is not part of a nandroid backup and this is my daily driver. I've tried searching everywhere, not just xda, and I see this has been asked but no answers to fix it. Has anyone fixed this issue? How did you do it?
Model: SGH-I747
Recovery: CWM 6.0.4.3
Baseband: I747UCDMG2
Went from Stock 4.1.2 to ParanoidAndroid 3.99-RC2

I've lost my baseband and my IMEI when I wiped my phone and flashed JellyKat 6.7. Before I was on 4.1.2 stock rom.
Any way to restore my Baseband so that I can make calls? I don't care about IMEI as its barely useful.:crying:

OMFG, after a month of search, I finally found the guide on this website that fixed my phone. Now, I will back up my esf folder everytime I install a new ROM. Thanks again to this very awesome website: http://www.ibtimes.co.uk/install-n7105xxuemk9-android-4-3-galaxy-note-2-lte-1431979
Trying to install CM11, but so far I keep getting either a "Status 7" or aborted error message. I have tried many of the fixes, but so far no luck.. I think I created this issue when I upgraded to 4.4.2. I would be happy to go back to a stock version and then CM11, but I have not been able to do that either. Here is my current phone details:

please quit trying to go back to stock if you are referring to bootloader.! you will most likely brick your device!!! STOP.
which recovery are you currently using ?
have you only tried flashing CM11?

I'm currently using philz_touch_6.15.4-d2att and have also tried current versions of CWM and Twrp with the same results. CM11 and CM10.2 were both tried as well as editing the updater-script in the Cm11 zip file and removing the "assert checks".

6.15.4 is too old to flash kk roms. Status 7 errors can be caused by an outdated recovery.

I did try the latest CWM and Twrp with the same results. I flashed AOKP just fine. Something strange about my phone. Maybe it has something to do with me replacing my motherboard recently? Relaced my Sgh-T999 with the Sgh-i747???
Anyways, it's back up and running

after flashing new rom and modem I have lost imei number, I dont have any backup of efs drive....
Is there a way to restore it?
I tried flashing back to older modem and stock rom, but no luck. i have still no imei and baseband unknown.

We did try that a few times, it did not work. I was genuinely surprised as we used a Swiss stock rom that restored the imei number and network connectivity.
Hey Guys,
So i recently flashed from stock to CM13 via TWRP Recovery which was installed via Odin.
After doing this my device is now almost operating perfectly (Exceptionally better than it performed on Stock),
except i now get Baseband Unknown, IMEI Unknown and IMEI SV Unknown.
- Samsung Galaxy S4 Mini GT-I9190
- I do not have any backups of /efs! (sorry!)
- I have my IMEI number written down
Any Solutions to this ?

Can you please elaborate, i'm not sure what kies does or how to use it,
I have tried flashing couple stock roms and all had the same problem.
Install Samsung kies(jellybean) or kies 3(for kitkat). Then click on tool tab and click on firmware upgrade and initialisation. Enter the phone model and S/No as shown on the back of your phone. and it will download your stock rom which might help you restore your imei.

My phone is currently with unknown baseband and IMEI on stock 4.4.2 flashed via kies recovery. If I try to flash any 4.4.4 port ROM the phone locks up seconds after boot, it will only work with stock rom. I already tried a few modems i found on another threads but no success.
Can anyone help?

restore efs backup
there is a thread about imei and patched DN4.
Update everything, modem, bootloader, get these from newest firmware downloaded from sammobile and post results

I already got latest firmware from kies temp directory but I have no idea where to get modem and bootloader.. Can you point me in right direction?
Firmware file from kies is N7100XXUFNI5_N7100ORGFNJ1_N7100XXUFNG1_HOME.tar

from sammobile, download newest firmware for your region and unpack it from stock rom and then pack it with 7zip to tar file and flash it with Odin.
you need sboot and modem

Isn't that a bit pointless? Maybe I'm missing the point, but if I already have flashed latest firmwares from both sammobile and kies, it kind of makes the point that modem and bootloader files from sammobile don't solve the problem, right?

Well truly doesnt seem pointless
I got my baseband resolved (Previously showed 'Unknown') by flashing pure stock rom (got from SamMobile) THROUGH ODIN so might be there is a point.
Try stock rom flashing from odin as according to me stock rom has everything in it radio(Baseband),boot(efs), system, etc etc
The unknown baseband and imei can be due to a modem and bootloader mismatch.
To confirm the bootloader and modem on the phone, install the Samsung Phone Info app from the play store to confirm the version.
