So I tried flashing this ROM and i got this "set_perm some changes failed" at the end. It started to bootloop my phone and recovery wouldn't work anymore. I was able to get to fastbootUSB and tried reflashing recovery, but same issue. After wiping the cache via fastbootUSB and refalshing the recovery once again, i was able to get to recovery.
With that outta the way, whats this install error? I've really been waiting for ViperOne on VZW and i can't install it...

the guy who ported the rom over is fixing it. his thread said to check in the morning.
on another note, how did your recovery not work anymore?

went it bootlooped, i held the volume down key and while on HTC screen, top said "Entering Recovery" instead of booting to bootloader. once it tried to start recovery i see it for a sec then it reboots again.
BTW, is there any way i can post in his rom thread, he said he'll fix it in like an hour, and i'm up to the late hours of the night. i'd like to coment in that thread for any probs or help of any sort, cuase i get a forum error saying i new here, i may not post much but i've been on xda for a couple years now.

So my note 2 had been working very well, but just the other night it turned off and then was stuck at the Samsung logo screen. I attempted to boot into recovery, but it would freeze at the TWRP logo screen. I decided to Odin a new recovery and put the latest version of TWRP on there. I can now boot into recovery, but it asks for a password. I assumed I had just used a bad version, so I tried several others which resulted in the same outcome. Pressing cancel lets me get into TWRP, and it seems as though I can browse files, but trying to perform any other action gives an error message saying that TWRP cannot find the partition for data, system, and a few others.
Is the phone dead? Or is there something I can do to bring it back to life?

Try to flash a stock rom with Odin.
If this does not work you are facing a compareable problem to mine http://forum.xda-developers.com/showthread.php?t=2440687 .
Maybe you find a way to get it work in this link.

I really hope it isn't SDS. I can odin a recovery onto the phone, but so far other odin-able images just fail. I'm downloading a pure factory image currently and will check to see how that works,

I have the same problem but for a different reason. Self inflicted unfortunately but in the interest of not hijacking your thread I'll simply say I'm anxiously awaiting the answer as well.
A quick explanation of how I got here... Did the "back to stock" steps and worked great. Did triangle away with no issues. Then I did the OTA update and I suspect that my issue is the result of not unrooting first? Cause it DL'ed update fine, appeared to install it fine then upon reboot it got stuck on Samsung Screen like OP is now stuck on. I have tried to repeat "back to stock" without success, tried to install rooted recovery without success. Everything I do shows "Passed" in Odin and appears to install but still stuck on Samsung screen. Let it sit for an hour and still doesn't load.
I'm assuming (hoping) at this point it's a similar solution?

Not sure if this will help you or not but it just worked for me I used Odin to flash 4.3param_tz.tar and it worked for me. I have NO IDEA no idea if this will solve your problem or if it will further compound it so I would confirm before trying it.

mine is solved. the back to stock may have had nothing to do with it as the problem occurred AFTER I successfully went back to stock and updated with the OTA update before uprooting and setting stock recovery back. I'm assuming my problem was caused by the Knox security stuff.
Anyways, I'm back on OMNI ROM again. I just occasionally like to start all over and I overlooked a big part of it. (Tired Mistake on my part)

Can you at least boot back into recovery? If so, I would try clearing the cache and Dalvik cache to see if that might fix the problem.
If not, I would first try flashing Philz again to see if that fixes the problem. I would use Odin for that since Phil's instructions for installation indicate that for Samsung devices Odin should be used.

And in HBOOT my OS is blank
First things first. I was running liquid smooth 10/01 build. Its been running awesomely for a day now. I downloaded the newest build of the dolphin emulator, tried running baldurs gate dark alliance....and my phone rebooted, and got stuck at the HTC logo screen. So I held the power button and vol up to reboot then quickly hold pwr and vol down to get to hboot and rebooted to recovery.
In twrp (ive fasboot flashed captain throwbacks 2.8.0.6) it reboots literally about 10 seconds in.
I look at twrp's log and Im getting a weird error.
I have 3 lines of error
bear with me here remeber i have 10 seconds to see the error then it reboots.
***** is refering to the file name
"unable to stat ' /data/system/procstats/*********.bin"
"unable to stat ' /data/system/usagestats/usage-******"
"unable to stat ' /data/system/sync/stats.bin"
so....my question is how in the heck do I fix this.
I literally cant do anything, Hboot and fastboot work fine.
But I cant wipe any partition, I cant flash anything long enough for it to actually flash correctly.
And flashing the partition fixer doesnt help...

since recovery is freaking out, can I just flash the stock rom through fastboot or hboot with S-ON? Could someone direct me to the correct stock files for "sprint M8 Harmon Kardon edition"?

Can you please post what you did? I'm having a similar issue. TWRP keeps rebooting after like 30 sec. fastboot flashed older version 2.8.0.1, still having same issue, so I flashed back 2.8.0.3, and keeps rebooting after a few seconds in recovery

Ok, so I did something really stupid this morning. I had a notification that a software update was available. I decided to ignore my better judgement and install it. After it downloaded, the phone rebooted and went into recovery. When I say recovery, I mean TWRP, since that's what I have installed. Now every time I reboot the phone, it just goes straight to TWRP! I tried wiping the phone (factory reset). I even tried installing a different ROM! It just keeps on booting straight to recovery.
Any suggestions? Please help. I'm dead in the water.

I purchased a 5x for my wife so she can get on Project Fi. They said it was stuck boot looping, thinking I could solve the problem.
Here are the basics.
-Yes, I can get into boot loader (the menu of start, recovery, etc), but I can't get into TWRP. Tried flashing TWRP a few times and nothing. SkipSoft recognizes that its installed though.
-After bootlooping for awhile, it will load normally and work fine. However it wants to update to the newest version of android. I run the ota and back to bootlooping. It will show the android dude and say "error". Will eventually start bootlooping and then boot normally.
-I have flashed different stock versions. I tried the newest, the second to newest and the last M available. All will flash but continue the bootlooping.
Bootlooping can be anywhere from 1 minute to 10+.
The device will also first display something like, "Your device software can't be checked for corruptions. Please lock the bootloader"
But it'll show up whether the bootloader is locked or not.
The phone will freeze at some point. Whether right away or after 5 minutes. It freezes and then restarts and goes through the bootloop again.
So what else can/should I do? I've never dealt with Kernels but wondering if the last owner did something to them. Is there a way to flash stock kernels just incase or is this done when I flash a Google Factory Stock Image?
Would flashing a non-stock rom be a good idea?

So I tried sideloading. I was able to get into the stock recovery. I tried wiping cache and data, then tried installing the newest stock rom.
Everything seemed to work. Then I had to restart. A message popped up after the same "Your device software can't be checked for corruptions. Please lock the bootloader". It had the title in blue stating "Encryption unsuccessful".
Then said, "Encryption was interrupted and can't complete. As a result, the data on your phone is no longer accessible. To resume using your phone you need to perform a factory reset. When you set up your phone after the reset, you'll have an opportunity to restore any data that was backed up to your Google Account."
Then there is a single button that says, "Reset Phone".
Problem still persists.

When my Nexus 5x got into bootloop, I sent it to LG and they had to change the whole circuit. So yeah.
When you flash Google Factory Image, it flash the stock kernel.
If you have warranty, then send it to Google/LG. If you haven't got, then you're in problem. New motherboard needed, but it's expensive.
