That's not the issue....
1: updating with root is no problem
2: updating with an unlocked bootloader is also no problem

Is it the stock rom or a custom one ? Give all info. What rom, what update exactly (version number)
I am using stock android 4.1.1 and update is android 4.2.2 and my phone is not rooted only boot loader is unlocked

Then updating should not be an issue. Seen that problem before on T-mobile branded phones.
Maybe a factory reset can help and after update again. Sure you didn't delete any (system) apps that could cause this ?
Nope I didn't delete any system app but I can't do system reset cuz I have lots of apps plz tell me any other way and I am on( Optus)

No other way .... or backup with titanium backup or something like that.
I emailed htc and they send me this message so I factory reset it but the process of reset didn't completed and set freeze on quitely brilliant

So the update still no go, then you have 2 options :
1: flash a custom recovery and rom
2: relock the bootloader and send it in for warranty
I don't know how to flash and relock bootloader

had the same problem
try this this should work as it worked for me
go to general section and download Hasoon2000s toolkit
connect your phone to pc and copy the update zip file to pc. (only copy, dont move)
in your pc, extract the update zip and repack it WITHOUT the "firmware.zip" present in it.
in the toolkit, choose flash factory ruu zip.
choose firmware.zip
after the flashing is complete, phone should reboot to recovery and your update should be installed.
if it does not, and the phone is still on the update screen (blue-gray screen with htc logo), or reboots to fastboot, reboot it to recovery manually from fastboot.
if still it does not update automatically, unlock your bootloader, flash philz CWM, reboot recovery, mount the storage to pc, copy your REPACKED zip (which does not contain the firmware) and flash it like normal custom ROM WITHOUT wiping (wipe only if you get a bootloop but dont wipe system in any case)
your phone should boot up normally

I have Android 2.3.5 rooted S-Off phone. I've received the new FOTA system update and cannot install it. Whenever I tap and agree on the update the phone reboots to boot screen with green progress bar which ends up in a screen with a red exclamataion sign inside a red triangle all on the top of a phone image. Does anybody have any solution to overcome this problem?

To install the ota update first install the original recovery, then relock the boot loader and(optional unroot your phone).
You won't have any problem updating then.
To relock boot loader you can use the guide HTC has made to unlock it or else(easier step) use some tool such as rezo609's all in one tool.

If you already relocked your bootloader and cannot upgrade, then try with installng last RUU, corresponded to you CID number and then try to upgrade.
Maybe during the time you had unlocked device you uninstalled some of the preinstall apps, and now this doesn't allow you to upgrade.

Find the correct file for your phone,
Use command fastboot flash recovery recovery.img (recovery.img is replaced with the name of file you are using.)
Stock recovery back for ota.
Check out a few of the guides, will walk you through anything pretty much

The problem is I can not ("unlock" Bootloder) for Root
This problem got me after installed (stock ROM) to my device via (RUU)
I followed the same steps taken by the first time , but that does not work Because (Bootloader) in the case of (relocked)
And I want to become a case (Bootloader) is (unlocked) again
But how????????????

yes locking it after the first unlock,it says relocked.for unlocking it again-
place the Unlock_code.bin image file u received from htc the first time in the fastboot/platform-tools folder.
boot ur phone into bootloader.
select fastboot.
connect phone to pc.it should say fastboot usb in red.
now flash the Unlock_code.bin to ur phone using the following command-
fastboot flash unlocktoken Unlock_code.bin.
select yes.ur bootloader is unlocked again!
for rooting it again-
flash a recovery first.boot into bootloader then go into fastboot as mentioned above.connect phone to pc.
then place the recoveryname.img in the fastboot/platform-tools folder.flash it using-
fastboot flash recovery recoveryname.img.
the recovery is flashed.now,boot into recovery.and then flash this zip in recovery-

How I Rooted my device before with that same system and the same files without any addition or deletion
I told you that the old way did not work I want another way to "unlock bootloder" to root
Sorry there is no other way to unlock bootloader;the only way is giving u an error,and that error is caused due to improper driver installation and environment setup.

I have unlocked my phone which was pretty straight forward. I have the stock rru installed I also have twrp installed. My question is this I have done a software update via the htc software update option on the phone but when it comes to restarting the phone after the update it takes me back into the twrp main menu. What do I need to do to bypass this and get it to do a normal update

Download this file and copy it to your fastboot folder on the pc

Rename the file to recovery.img and put the phone in the bootloader/fastboot usb mode
Then open the commandprompt inside the fastboot folder and flash it
Fastboot erase cache
Fastboot flash recovery recovery.img
Fastboot erase cache
Reboot the phone and update again

I am posting this for my frnd he was trying to unlock bootloader which he successful did but when he tried to reboot after that it stuck in bootloop(htc logo) and later he realized he accidentally clicked on flash kernel from some rootkit and I suppose that's why it's suck in boot then he tried to flash recovery and flash a new Rom but his phone is not detected by the computer
Any suggestion

Yes he can enter bootloader from the power+volume button combo nd drivers are update since he unlocked bootloader from same computer

So my suggestion is: flash a recovery like philz 5.15.9 via fastboot, enter recovery, mount the phone as external mass storage, copy there the zip of a rom, wipe data to install new rom, reboot to bootloader, flash boot.img from the rom via fastboot (erase cache before and after flashing boot.img) then enter recovery flash the rom zip and choose wipe in aroma... Then should be ok... Ask here for further infos
