Are u trying to flash the rom immediately after unlocking the bootloader?? When the bootloader unlock is performed, the data is wiped completely.. Now u have to restart the device to mount the sd card again in recovery..

1) you will never be able to mount the sdcard in recovery, so stop trying
2) what is the fastboot command you typed to generate the output?

do you have the files in the same directory with your fastboot.exe?
you could also boot into your custom recovery and use adb to push a rom to your phone.

y is this happening
*sigh* ...
Op, uninstall all drivers, reboot your system;
Connect device in fastboot to PC, manually update drivers, disconnect device, reboot PC
Connect device again, flash recovery, after it finishes, disconnect USB, use volume keys to "reboot recovery", reconnect USB, manually update drivers again
'adb devices' should return your SN
'adb push ROM.zip /sdcard/' or if that doesn't do it, 'adb push ROM.zip /data/media/'

Hello everyone, I made a mistake (I am a big noob) and I don't know how to fix it.
I wanted to have fresh phone (real factory reset) to back-up and start over, so in recovery mode (interim one from paul) I wiped data, cache partition and I also formated cache, data, sdcard and system.
Now, I am stuck on htc quietly brilliant screen, I have access to hboot, recovery etc. I have been looking for a solution for 2 days without succes, I am sad.
Can someone help please ? thanks guys.
You wiped the system so there is no android on there!
Eirher download a new Rom and flash or relock bootloader and use ruu

The part you did wrong: It's true that you wipe (dalvik) cache and data before flashing a custom rom. That will perform a factory reset. Wiping system means deleting android itself, as stated before. That's what you did wrong.
Now, we can't know why you wanted to start all over again, but assuming you already flashed a custom recovery, you wanted to install a custom rom. Just go ahaed and do so, your problem will be fixed.
thanks, but I can't because this custom recovery doesn't allow me to mount usb and i have no rom on my sd card

That's not actually a problem. I assume you aleady have the android SDK set up, if you don't, please use the search function or google. try the following command while in the platform-tools folder:
adb push path/to/custom/rom /sdcard
or for linux/mac:
sudo ./adb push path/to/custom/rom /sdcard
This will copy the file to the root of your sdcard, so you can flash it. Good luck!
P.S. there's a thanks button, too

That wont work as ADB does not work on any of the custom recoveries at the moment. Im stuck in the same situtation at the mo and as phone is branded cant get the RUU to reflash so just playing the waiting game until either ADB/USB mount available in recovery or S-Off available.

I do exactly the same thing , but half way through the RUU process it gives me an error saying i need the correct RUU. anyone can help ?
All you have to do is grab an RUU, run it. now on your pc search for %temp%in there locate ROM.zip. Extract the boot.img and system.img. and push them thru fastboot
Code:
fastboot flash boot boot_signed.img
Code:
fastboot flash system system.img
Code:
adb reboot
i did that , but i get this error:

Hi Everyone,
sorry in advance for what is probably a dumb question. I have a HOX on Orange UK. It is successfully rooted but still on stock firmware - Android 4.0.4, HTC Sense 4.1, Software 2.17.61.3.
When checking in about I am told that there is an update available. I have downloaded it and can see that the file is called OTA_ENDEAVOR_U_JB_45_S_ORANGE_UK_3.16.61.6-2.17.61.3_release_3015.zip. However when I click in stall the phone reboots into teamwin recovery project v2.3.3.0. I then select Install adn browse to theis file which is in /sdcard/Download but the install keeps failing MD5 check. Unckecking the MD5 check still results in a failure since the error message is that the signature verification fails. Unchecking that still results in a failure- this time because of an "assert failure" whatever that is.
So a few questions.
1) Is this update worth having?
2) I am new to Android (10 years on a blackberry). Am I on ICS now and is this update to JB?
3) I am interested in a custom room. I really like the look and feel of the HTC sense and I have seen here that there are a lot of Sense custom roms. I presume I should stick to one of those. My main question is which one to install and what are the advantages. The honest truth is that apart from the Orange boot up and shut down logo there isn't much bloatware on this phone.
Thanks in advance for helping out a noob...
Yes its worth it, you have to flash back the stock recovery ! After that you can update.
Download this file and rename it to recovery.zip and copy it to your fastboot folder on the pc
http://db.tt/U5CK4noo
After that you put the phone in the bootloader/fastboot usb mode
Open a command prompt inside the fastboot folder and flash the stock recovery with these commands
Fastboot erase cache
Fastboot flash recovery recovery.img
Fastboot erase cache
Reboot the phone and try to update again

Thank you. I am afraid I need you to spell it out in a little more detail since I rooted my phone using the Hasoon toolkit so that did all the command prompt side of things. I do know how to launch command prompt but that is about it!!! Sorry
Well you can check if the toolkit you used is capable of flashing a self providided recovery.img
Otherwise you have to install the HTC drivers and download the fastboot files from me
http://db.tt/ldx3qOOU
Place the files in a folder just like this
C:\fastboot\fastboot files i gave you
Then you open a command dosprompt inside the fastboot folder
Then you keep the phone on the homescreen and type
adb reboot bootloader
This will reboot the phone into the bootloader and you should see there FASTBOOT USB (connect the phone to the pc)
Then you do the commands i posted earlier
(Make sure usb debugging is enabled in the settings menu of the phone)
after flashing the stock recovery and updating root is lost, but superSU still there. How can we obtain root back? thanx
You have to reroot the rom again just like you did on ICS

you can do this with the stock recovery because you flash directly from the bootloader and not from the recovery

Thank you! so my problem is I'm still in stock recovery? cause superSu is still in my apps from ICS.
Lets get it straight...you updated the phone to JB and after that you restored apps (titanium backup ?) You need to reroot the stock JB rom properly again like flashing the root script from a custom recovery
i updated from OTA , so i didnt restore any apps. and when i write 'su' at terminal i dont get any errors. doesnt that mean su is installed?
If SU is installed you should see the SU app in the apps list ... My recommendation would be to reflash the custom recovery and re-apply the root

Hello,
I also have this problem but, moreover, I think I'm also not able to do a data/factory reset (from the recovery I can start the procedure but than the phone reboots without finishing...)
All the rest is the same:

- the new rom I've pushed was disappeared from the folder
So now I don't know what else I can do...

On the terminal after the command

it tells me "error: closed"
I also tried to reinstall the Clockwork via bootloader but didn't change the situation (still not able to do a complete factory reset)
you first did click on "install zip from sideload" in clockworkmod to activate sideload on the phone, didn't you?

You are right, I didn't click "install zip from sideload" before. Now I tried in this way and, effectively, it worked: I managed to send and install the zip apparently without any problem but than after rebooting I'm still not able to turn on the phone completely. Again the same problem. I re-tried to do wipe data/factory reset both from the recovery and using adb but didn't managed to finish the process...
It seems I tried all the options since now.... but my nexus is still unusable...

when the phone is in fastboot mode you should find your bootloader version at the bottom left of the screen, in your case it should be:
BOOTLOADER VERSION - PRIMELC03
when you flash a new bootloader through: fastboot flash bootloader {your-bootloader.img}
you need to reboot for the changes to take effect, you can do it with: fastboot reboot-bootloader
after that you should see a different version on your screen, is that the case?

Yes I have BOOTLOADER VERSION - PRIMELC03

But than, after the reboot, on the screen it's still written BOOTLOADER VERSION - PRIMELC03
I think this is the main problem now... if it's not updated the bootloader I can not reinstall the rom probably...
you can bypass the version checking by extracting the update zip and flash each image manually
in your zip you should find 4 img files: {system, boot, recovery, userdata}.img
you can flash them on by one like the following:
fastboot flash boot boot.img
fastboot flash recovery recovery.img
fastboot flash system system.img
fastboot flash userdata userdata.img

Tried as you suggested to install the PRIMELC04 bootloader flashing each image.
Apparently is not possible also in this way. After rebooting on the screen is still written BOOTLOADER VERSION - PRIMELC03.
So I tried to install a stable cyanogenmod 10.2.1 (which should work with the bootloader 03).
I used adb sideload and after the "installing updates" message, everything stopped with the message "fixing fs_size in crypto footer". After more than 30 min I rebooted and the situation was the same as before.
My DHD will not boot up. I can put it into recovery mode, but am not able to navigate the menu with my volume buttons or select options with the power button.
This occurred after an (apparently failed) attempt to flash the 4ext recovery.img onto my phone.
My phone has an unlocked bootloader, S-ON, and not rooted, if that helps.
I appreciate any help with my issue in either successfully flashing the recovery.img or getting my phone to work in any way. Thanks!

probably the misc partition is fked, or the chip's gone bad.I'd try reflashing HBOOT by putting it manually via the sdcard, naming it PD98IMG.zip and putting the sdcard back.I'd wait out on another opinion though.

In case I don't get another option, could you instruct me on how to transfer the file to the sdcard? Sorry, not very knowledgable on this type of thing.

Remove it from the phone, insert it into a cardreader of some sort, transfer it to the sdcard renaming it to PD98IMG.zip,putting the file in the root of the sdcard, no folders, putting the card back into the phone, booting to Hboot, pressing the power button just once to start the procedure. If you can't press any of the buttons in HBOOT too, then your phone is effed.
again, don't try this before someone else gives an opinion on this matter.

Have you tried Glevitan's tool to flash CWM recovery? You can find it here:
http://forum.xda-developers.com/showthread.php?t=2133336
Or actually, these are the steps his tool do for flashing the recovery (use adb.exe and fastboot.exe, and change the location of CWM image):
echo Rebooting into fastboot usb
echo.
adb reboot-bootloader
adb kill-server
sleep 10
set recovery=tools\recovery\recovery-clockwork-touch-5.8.1.5-ace.img
fastboot flash recovery %recovery%
fastboot reboot

Hallo,
this is my problem: i dont have working system. When i turn on my device i only see one Plus logo. I have access to fastbot and recovery mode.
The things i have already tried:
https://forums.oneplus.net/threads/guide-mega-unbrick-guide-for-a-hard-bricked-oneplus-3.452634/
I have aleready executed method 1 and 2 ( without How to Prevent Data Wipe in Method 2) and everything is working properly until I perform Step 14, when I try to flash via adb sideload latest OXYGEN OS it always stops after 47%. (Installation failed).
I have also tried fastboot method:
PS C:\adb> fastboot devices
755549d1 fastboot
PS C:\adb> fastboot oem unlock
...
FAILED (remote: oem unlock is not allowed)
finished. total time: 0.000s
PS C:\adb> fastboot flash recovery recovery.img
target reported max download size of 536870912 bytes
sending 'recovery' (22605 KB)...
OKAY [ 0.625s]
writing 'recovery'...
FAILED (remote: Partition flashing is not allowed)
finished. total time: 0.641s
now i really dont know what to do . just hope that someone can really help me.

Have you tried to flash any full Rom from stock recovery? Doesn't need unlocked bootloader as long as its the official OnePlus software.

You are trying to flash TWRP with a locked bootloader and it obviously fails. Download the full official ROM from OnePlus site and the corresponding recovery also. First flash the recovery and reboot to bootloader and then flash the OS and reboot. If you are successful, you can enable Developer Options and from there OEM unlock and USB debugging. Thereafter you can try TWRP, Magisk and other ROMs.
Hmm, I thought he said he can enter bootloader and recovery. If so, reboot the phone in stock recovery and flash full Rom from local update.

NO.
So that is where you are committing a mistake. You have not been reading the posts carefully.
All of us are asking you to download the STOCK recovery compatible with the version of the stock ROM chosen by you and to FLASH THE STOCK RECOVERY FIRST, NOT TWRP.
Because TWRP can't be flashed with a locked bootloader whereas the stock recovery can be flashed from bootloader. Then go for the ROM, sideload it using adb and boot.
