I have a t989, running Jedi Jelly 5 using a 32gb Sandisk sd card.
Every 2-3 days my sd card says it has an unexpected error and is blank and to reinsert another one. I check, it is inserted and it is indeed now blank. Then it will just keep mounting/unmounting itself. This will not stop happening and I have no idea what to do. I can format it in recovery which will stop it from mounting and unmounting itself, for the time being. Then, in a day or so it will erase itself and unmount itselfs again and it just repeats the cycle over and over until I format.
Please, anyone have any idea on what is wrong or what I can do to fix this? Before Jedi Jelly 5 I was running Jedi Jelly 4 and never had this problem.
Thank you in advance for any help whatsoever.
Either format it or try another rom. That's a weird situation.

Normally formatting will work for the time being but now I can't even format it. My computer won't recognize the sd card and formatting isn't working in the phone. It says it failed and says the sd card is blank or has unsupported content and to reformat, but I try and keep getting the same "failed" error. So now my sd card has been wiped clean and won't work at all. I have no idea what to do...
I just put in a different sd card, a 2gb SanDisk that I had in another phone, and the "blank sd card..." notification went away and all seems ok.

Hello,
Problem with the SD card.
If you start from sleep mode appears card was removed and then can be used again, see on status.
This has been adjusted after a few weeks.
What could be the cause.
:crying::crying::crying::crying:
I am also seeing this issue, it looks like card is unmounted and mounted again automatically....
What do you mean by adjusted after few weeks? you don't get it now?
I am still trying to find out why its happening, might try to format the sd card and will try...

I meant earlier was the SD card mounted, and I had always unmount any mount.
I have already formatted the SD card, itself effectively.
Is it still happening after formatting the card?
Also have you formatted the card from recovery or from system settings

Hi,
If I format the card, the same error occurs.
I'm going to swap the card and test.
Let me know how it works out

OK, I think I am able to resolve my issue. I haven't seen that message for last one and a half day where I was seeing it 2-3 times a day.
Here is what I have done. I have removed the card and formatted it from the laptop, reinserted it properly again and format it again in the phone.
I think the key is how you insert the card, there can be some tension in the strings which can push the card back, so make sure while inserting it is going in properly and there is no friction while inserting.
I will post if it doesn't happen again for another 2-3 days...
Hello, I am on an HTC One V running PACman ROM 15.3, with the kernel JmzPrimoC+.
I started having this problem a couple weeks ago. It would randomly unmount my SD card saying 'SD card unexpectedly removed'.
This started to happen more and more often until it would say that as soon as I put the SD card in.
I did a factory reset, which helped for about 5 minutes before it started happening again. Now, more recently, it started saying 'Damaged SD card. SD card is damaged. Try reformatting it.'
I put it in my computer (which doesn't show anything wrong with it), and did a full format. It didn't help at all.
This problem is happening with both my regularly used SD card (SanDisk Ultra® microSDHC™ UHS-I 32 GB (the red and grey one)), and my ordinary SD card (8 GB class 4 Kingston).
I don't think this can be a hardware problem because when I boot to recovery, TWRP recognizes it perfectly.
But I don't either see how its a software problem, because a factory reset should have helped...
Please help me, any help will be greatly appreciated.
Maybe its a bug in the rom?

Hhmmm...
Good idea. What other ROMs work with JmzPrimoC+ kernel? AOKP?
Sent from my One V using xda app-developers app
OK, update, now TWRP won't read the card! Either card!
Now I'm getting really nervous...
Does this mean it must be hardware?
Is there ANYTHING I can do now?
Please help!
Wow, I screwed up really bad.
I was gonna use TWRP to flash a new rom, so I did a factory reset, and then started flashing the rom.
But in middle of flashing the rom it was interrupted. So I rebooted, rereset, and then I realized... it's not reading my card.
I can boot to bootloader and recovery, but that's it. No OS. I'm screwed.
Is there ANYTHING I can do?
Is it possible to flash a rom over USB? Or SOMEHOW get my card working again?
Just enter recovery and use adb sideload to flash a ROM.

But I need an SD card in order to use ADB sideload!
It fails every time because
Code:
E:Unable to mount /sdcard
Is there ANY way to do this without an SD card?
OK, I was able to do it following this guide.
I flashed the latest version of PACman. Still rejecting my SD card though.
I guess it's a hardware issue. Too bad my warranty is void from bootloader unlock :\
Try to partition SD Card in recovery
I am facing same issue and I came across this thread while looking for solution. Here is what I tried. That resolved issue for five minutes and then damn! same issue again but someone can find possible fix using the info here.
HTC One V rooted, currently running MIUI 3.1.2 with JB 4.1.2
Sandisk 8 GB SD Card.
Firmware CWM Recovery
I was facing same issue even before I rooted my phone while Original HTC ICS 4.0 Firmware was installed. MY observation is the problem starts few months after you start using SD Card. (Insert a new SD card in your phone, it will work fine for days / months and then will start giving same error.
While I tried thousand time to remove SD card, clean slot, clean card and reinsert, it did not work. May be worked just once for few seconds. When I tried to connect phone to PC and browse SD card contents, the card was not detected.
Booted to recovery, tried to format card, not detected. Tried to mount SD card, not detected. Then tried to partition SD card in Advanced options in recovery. The available options are 128MB, 256MB, 512MB, 1024MB, 2048MB and 4096MB. Since my SD card is 8 GB, chose the largest option. Now got an option to choose SWAP partition size. Chose 256 MB. Next chose option fix permissions, you have to wait until you see message permissions fixed message on screen.
Now rebooted phone, and voila! It worked!! SD card with 3.94GB memory is detected. But what happened to my rest of 4 GB?
Tried many apps to manage SD card from Play store. Using an app called APart, was able to delete partitions on SD card and create new ones. It worked for few minutes and then again I got SD card unexpectedly removed message.
Does this info makes any sense? I am not a developer and do not know much into phones but someone who know might be able to help. Buying a new SD card is not an option here because its going to work temporarily.
Have you tried formatting the SD card with your computer?

Yes, tried formatting SD Card using computer. Works fine when connected using adapter. Not detected when phone is connected to computer.
Could anyone please help me with the "SD card ejected unexpectedly" error? I have gone through countless online forums and posts but there is no definitive answer! Nothing has worked. I have SGH-T999 phone and 32 GB class 10 Transcend card. Here are my observations. Please help.
1. I tried CM, AOKP, BAM, Liquid Smooth ROM. I run into the same problem repeatedly. It's unpredictable. At times, it happens less number of times, at times, it is more.
2. I formatted the card in exFAT to see if it helps. but no luck! Perhaps the frequency has reduced. But it is still happening.
3. The original OS never had this problem. Once I flashed stock mod rom. It also didn't have any problem. The moment I get to AOKP or AOSP roms, it keeps happening!
Is there any solution or workaround to this problem? Please help.
Please format the card to FAT32 and then see.
Can you restore a Nandroid of the StockMod Rom and then see if it happens for few days ?

I tried FAT32, same results. I however found less number of occurrences with exFAT though. with FAT 32, it was happening way too many times.
I do not have nandroid, I flashed StockMod Rom again and again, it was peace. no failure.
My s3 ate up b 3 quality sd cards in a year and a half. Alwaya got that error and norhing would work to fix. Except a new card

I didn't have contacts on external SD card. I have photos, music on external card. Those are in tact. I can access those until the moment the phone throws the unexpected card error.
Hello, I have a completely factory phone that I have never rooted or flashed. My phone all of a sudden cannot read my sd card and says I have to format it, I've pulled the battery out and restarted it, I've taken the sd card out and put it in my computer as well and my computer says there is an error and the card needs to be formatted. I would just do that except for the fact that I have probably 200 pics on my sd card that I would like to get off first. Would anyone no how I could recover the pics on my sd card or repair it so I don't have to format it.
I would appreciate any help solving this problem.
Thanks

Been working fine for over a month now all of a sudden the SD card appears to be randomly mounting/unmounting itself and has become unreadable on my phone.
I've put the SD card in my PC and it works fine so I suspect my phone has developed a fault. I hope I don't have to get an exchange as it's otherwise faultless and I'm dreading getting a unit that can't be S-Off'd
I haven't modified or installed anything prior to the fault developing,
Any tips would be most welcome.
PS It's a genuine Transcend Premium 300x 64GB bought from MyMemory.

well, a simple solution is to enter recovery and see if the sdcard works there if this way it works, change the rom/perform factory reset, if this doesn't, well, you might try to flash everything again, but i's possible that the sd slot is broken...
i wish you all the luck, let me know
I would copy EVERYTHING off the card to a PC and do a format and try a different file system. I would has perform the format on the phone, to make sure its gets formatted to the correct file systems.
Long story short, It sounds like you memory card is starting to get Corrupted. I have to SanDisk cards that started doing that in the biginning and ended up not being read at all on PC nor the Phone.
Also while its doing that, youll also notice the battery start to drain more, at leasst thats what happended on my Note 3 when I had the san disk cards going corrupt.
Ended up buying new Card and now everything is fine.
I doubt is the card reader built in phone, more then likely its your memory card going corrupt.

I agree most likely the card. I'd download an sd card format tool and do a full format, don't use Windows format then do full check for bad sectors. I had a similar problem and it was the card at fault, but strangely worked ok on the pc.

Thanks for all the advice.
I could read the card in recovery but it failed to format there.
I tried to format in windows but my only options were NTFS or exFAT, no FAT32. But this failed anyway also.
Downloaded a SD format application but this said the card was write protected and to move the lock switch, obviously MicroSD has no switch like standard SD.
So next I deleted all the files on the card and ran h2testw test. This failed and reported card is probably faulty.
Then I checked the contents of the card again and all the original files were back again, so it's definitely 'Donald Ducked'. I was able to get all my files backed up and fortunately I had my photos already thanks to Dropbox & G+.
So I will now look at getting MyMemory to replace it but my fear is they will want me to return the faulty card but it has sensitive information on it that I can't delete. Any ideas on how I can wipe it?

How did you attach the card to the pc? Try a usb card reader and then try again. If still problems download mini partition tool (Google) and delete the partition. You can then wipe it properly.
I reformatted the SD card from the settings within the phone and since then it been working fine :\
make sure you're using 2.0USB also..
when i connect my phone to charger
Its been few days i am noticing when i conmect my phone to charger it says 'sd card removed unexpectedly or either it suddenly stops showing content from my phone, then i have to go to settings unmount/mount the sd card frkm storage option and then it shows the content even when charging, or sometjmes it doesn'fshow option for that so on that case i have to restart my phone. What could be the possible reason and solution to that. It was fine on 4.4.4 now i am on 5.0.2 all stock htc one m8 h/k edition with sim unlock and s-off via sunshine.
