Did you backup your efs data before tou flashed the rom ????
Sounds like you wiped your imie code
Sent from my GT-N7105 using xda premium

I did not backup efs.

1 try flash your origin phone modem . Still not work
2 flash origin stock rom. Stock rom from here http://forum.xda-developers.com/showthread.php?t=1957218
This is different version phone forum of yor phone that why you flash wrong rom in wrong phone and mess up your baseband & Imei.
So you need to flash your original stock rom & modem to get its back.

Flashing stock rom will do it? Do i also need to flash modem too or the rom will fix it? and do I flash from odin or recovery?
Which is the correct modem file, ive been searching. Dr Ketan's modem file, is that the one? Flash thru odin?

Yes, flash both by CWM. A lot of user didn't know keep do something like you did.you not only one.
Make sure full wipe and double check your phone model before flash .

And could u post the correct modem file for my sprint phone. De-odex or odex which should be the one to choose for stock rom?

try a factory wipe/reset maybe? you'll still be rooted after that...couldn't hurt till someone else may chime in
this is also why you always, always, always make nandroid backups via recovery before doing anything to change your system

damn...hopefully someone can help you out before I would say use my 2nd link in my signature and completely reinstall the entire 4.3 operating system and then root all over again...did you attempt while on the stock rom, going to settings>more>system update and then update your prl and profile?
edit...actually, you can try this, 4.3 mk3 stock roms already rooted...see what happens, it has some other stuff too in it that might help you out with your errors:

PRL and Profile reset-from the dialer on a TW ROM, dial ##72786# it will reset itself after two automatic reboots (don't touch anything while process is occurring).
Do you mean I should flash tw rom first ?
Then call ##72786# and the phone will work fine as before
Can I flash tw rom because my stock rom is 4.3 ?
And I hear that if we have 4.3 rom we cannot downgrade to anyrom

Yes, you need to be running any TW based ROM (stock, Wicked Sensations, MOAR, etc.) to run a PRL and Profile reset from your dialer, this function does not work on AOSP based ROM's. Hopefully you have a backup of your stock ROM, otherwise you will need to download one, and be patient because the file size is huge.
As to the second part of your post, stock is TW, and the downgrade rumor is only applicable if you 1) Have the Knox bootloader and 2) try to use Odin to change to a different firmware version that contains a non-Knox bootloader. You can flash any Android version of any ROM that you want when using custom recovery like Philz Touch.
Are you on 4.3? If so, no modem lower than MK3 will give you data if you do that
thank you guys and especially mr struck i already make the phone work again after dial ##72786#
and also i use someone trick that i should take out the battery for an hour after dial the code , so i take it out for a whole day and when i try the phone everything is working normally again.
thank you

IMEI Null/Null, Baseband Unknown

I've researched all day on this, flashed stock rom and recovery again, and nothing. Phone is out of warranty obviously.
My IMEI is showing null/null and the baseband is unknown. I've flashed stock, rooted, tried restoring the efs folder, and nothing has worked. Phone worked just fine yesterday/this morning. Any suggestions?
In my efs folder, there is no NV_DATA.BIN file either.
Any help? TIA.
1) download just the modem from this thread http://forum.xda-developers.com/show....php?t=2252932
2) connect phone to pc
3) copy zip over (do not un zip)
4) reboot to recovery
5) install zip
try this it worked for me
What bootloader is on your phone? Flashing the a modem onto your phone that is not compatible with your bootloader can brick your phone. For example, flashing a 4.4.2 modem onto a 4.3 bootloader will hard brick your phone.

I'm on the 4.3 bootloader and 4.3 Stock ROM.
Do you have a mjb modem on your phone?
Yes, I do.
If the baseband is unknown, how do you know you have the mjb modem on your phone?
Maybe you need to re-flash the 4.3 mjb modem. If that doesn't resolve the issue, you could try injecting the imei using nvwriter.

Well, that's what I flashed. Maybe I flashed incorrectly, I'll flash again.

Bold.
Good luck. Report back with your results.
Thanks.
Reflashing didn't work.
Trying the instructions, I'm getting stuck at step 2. When I dial that code, I get this screen:
and when trying to select the other option, the button highlights blue, and then goes back to how it was before.
What are you using to flash the modem? Which file are you flashing? Are you sure you are on a completely stock ROM with a stock dialler?

You flashed the 4.3 stock ROM via Odin? Where did you get the file? It must have been a custom compilation because AT&T never released 4.3 as a full ROM, only as an OTA.

I believe I downloaded the .tar from here, let me see if I can find the link.
I got the .tar here
http://forum.xda-developers.com/showpost.php?p=53563728&postcount=322
I assume that tar.md5 file has the necessary modem but I don't know for sure since I used to have the i747m, not the i747.
Maybe this will help with flashing just the modem: http://forum.xda-developers.com/showthread.php?t=2540998

What, exactly, did you flash via Odin? What is the model number for your phone (as can be found on a sticker under the battery).
Odin is extremely powerful and unforgiving.
4.3 MK7 firmware for T889 ....that's my model. That's what I'd been using since until I used Dn4 and now I can't go back.

You can try to flash a patched modem, find following my signature to post #2.
If it doesn't work flash firmware through Odin but forget about that old modem which seems to be for JellyBean or older.

Maybe the EFS folder got corrupted? If you have a backup, try restoring the EFS folder with a stock ROM.
After flashing stock, install the Samsung Phone Info app to ensure that the bootloader, baseband, etc. all show OL1.
You confirmed that the model # n900t is displayed in download mode?
