No files found after factory reset - Samsung Galaxy Nexus

I had a problem copying files from GN to PC. I though I could fix them formatting the sdcard.
So I've done a factory reset.
When it finished, at reboot the system (AOKP JB 3) was restored but the sd files were still there.
If I go to recovery to install some zip, it doesn find some folders and every file (I can see them mounting GN after boot).
CWR says "no flies found" when I try to open each folder.
What do you suggest?

Update: I tried updating to last CWM and .. same problem.
It looks like every folder is empty (but they aren't).

[Q] Unable to push files to sdcard after bootloop

I have been busy today with a nasty problem, i hope you anyone can help me out.
I was running JDROID's custom rom with great satisfaction, had it backupped. The trouble started when i downloaded an other rom and forgot to place the rom (.zip) on my sdcard before i flashed the boot.img via flashboot.
By that time i didnt knew how to use ADB commands, so i decided that i would forget about the new rom and i would restore my working backup. (im using CWM)
After restoring that backup a bootloop occured, so tried lots of things like wiping the cache. Also ive tried to restore the backup after lots of full wipes ect. Even installing JDROID's latest rom witch i had on my sdcard dindt help. Still had a bootloop.
I managed to use ADB when my phone was switched off, and i pushed a rom.zip to sdcard. But CWM cant seem to find it.
My question is this: in CWM there is an option to "mount USB storage". When i use it i get the message "unable to open ums lunfile". I have tried formatting the sdcard, whereby i deleted my backups still didnt work.
What whould you guys try next?

How to Re-install and Restore a CWM recovery file?

I was experimenting with the xposed framework and got stuck in a boot loop after restarting for a second time to install some more modules. The safe mode booting didn't work and neither did the included restore zip file for getting out of it. I tried to restore from the backup on the phone, but it already had the xposed framework in it and it got into a boot loop too!
I tried to adb sideload the backups that I made, and that didn't work either.
I keep backups, made from CWM Recovery, and I pull these to my computer using a WiFi file explorer. (I don't have enough storage to keep two alternative backups so I always have the second stored on my computer)
Anyway, I cant seem to get the restore file that I uploaded to my computer (a restore image) to reload on to my phone and restore. I have attempted to transfer it back to the phone into the folder it came from, but the file wont flash as a zip and it doesn't show up to be restored if I select the restore feature. (BTW-The restores have plenty of SD room to restore on, that isn't the issue)
I realize that the WiFI File Explorer that I use may be zipping the files, so shouldn't I be able to "install zip" in CWM recovery and it restores the image? (apparently not, can you explain why, and what I can do?)
Can I get access to the mnt/shell/emulated/clockworkmod/backup/ to put the file there to be restored again?
Or,.. How do I put the restore file on my phone (in the correct place) to be restored again?
Is my problem being caused by where I am pulling/pushing the file from(mnt/shell/emulated/clockworkmod/backup/)?
What am I missing/doing wrong?

Trouble installing stock ROM due to multiple '0' folders !HELP!

Hi everyone,
I am having a lot of trouble restoring my GSM Galaxy Nexus to it's former glory. I installed CM12 and CWM recovery earlier but CM12 suddenly stopped working. My phone started rebooting over and over so I decided it was to time to flash the stock ROM. I flashed the stock ROM with WugFresh's root toolkit. And it worked fine besides the fact that writing the system.img took 30 mins, which is very unusual. I rebooted my device and CM12 showed up again... ?!. I tried flashing it by hand with ADB but no luck either. CM12 was still there. I've been busy 3 days now and suddenly realised that CM12 created new root folders after a factory reset which I did multiple times. So for example, my sdcard contains one root with a folder, named "0", that contains the files om my phone before the factory reset. The problem is that this folders contains the system and CM12 ROM too. So I have like four '0' folders, and the 'deepest' one contains the system and ROM. Assuming that if you flash a stock ROM, it will install in the root of my SD-card, but doesn't boot because it didn't overwrite the CM12 ROM in the deepest '0' directory [​IMG]. If only CM12 was working, I could manualy delete all the unneccesary '0' folders... I really have no idea how to continue from now on and I really need your help [​IMG]
So I need to remove all the 0 folders and place the OS back in the sdcard's root, or I have to somehow tell android to place the stock rom in the same directory as CM12, or I need to somehow wipe the complete sdcard so I can install a new recovery and flash the stock rom... I have no idea how however.
Edit: I tried: adb shell rm -r /sdcard/0 and it is gone but if I reboot Recovery and look into my folders everything is there again...
epilogue:
- Maybe the fact that sytem.img took 30 minutes to write has something to do with all those '0' folders [​IMG]
- I tried flashing multiple recovery's to my device but they only show up if I open them via ADB. Ohterwise CWMR shows up again because that one is actually installed in the 'correct' (read:incorrect) folder.
- With Skipsoft roolkit I actually received an error while writing userdata.img. It said: remote: write fail. I thought this was due the multiple installation folders and the toolkit not knowing where to place/flash the rom.

[Q]Problem with my HOX. Recovery showing wrong folder for install zip from SD

It works great but i wan't to try Cyanogenmod 12.0.
So what i did : Downloaded the Rom.zip and Gapps -> put my phone on computer and copied both files on the Internal Storage(the only one who is showed up, its also the same where i copied this https://forum.xda-developers.com/ht...tck-rom-sense-5-debloated-4-18-401-4-t3111605)
I did a restart into the Recovery Mode wiped everything after that im was going to Install Zip from SD-card. but it doesnt show the same folders as on my Computer. its completly different. The Custom Recovery shows : sdcard0 but it isnt since i dont see the folders. I googled alredy a lot but cant find a solution.
The CWM doesn't shows the Rom.zip also not the Gapps.zip, but my Computer does.
Can some one help me?

ps : I installed alredy other Recovery Menus like twrp / PhilZ but its always the same.
Also if i do an Factory reset in the bootloader or in the recovery menu it doesnt work its saying completed and everything is wiped but if i start my phone all my apps still there??

I'm trying to install a custom rom on my HTC One M8. I unlocked bootloader, flashed TWRP, did an advanced wipe of everything, but now I'm unable to copy the rom image to the phone.
When I boot in recovery mode, TWRP shows up and the phone appears on my computer. But when I browse it, it has 4 folders:
-TWRP
-_nero_sync
- albums
- mobile
This looks weird to me, as I remember TWRP should be the only folder right there. Also, windows wont let me copy files into any of the folders for some reason.
I actually managed to fix this probably by repeatadly wiping the phone yesterday, but now I did it for at least 15 times and it's always the same.
What can that be?
Storage may be corrupted. You can try the "format data" button on the main TWRP "wipe" screen, which often fixes such corruptions issues. Somewhat obvious, you'll lose data when you format; but if you are wiping everything, this shouldn't matter to you.
