Hi Everyone,
So I have updated to Jellybean (4.1.1) via the OTA update and my bootloader become locked again due to the update.
I have Voodoo OTA Rootkeeper installed so fortunately I was able to restore root and Superuser is working fine (so I thought)
Until yesterday, I install an application that would require root access and it appear to me that Superuser was not granting this access.
However previous application that has the access granted by Superuser is still intact.
I tried updating SuperUser binary and encounter the error "Gaining root access..... Failed"
It appears to me that new request for root access was not allowed anymore but previous granted access still works fine.
I do not want to unlock my bootloader again as this means everything will be wipe out.
Therefore Flash via CWM recovery mode is not an option for me.
As I still have root access on ES File Explorer, it there any possibility that I can manually update Superuser (or switch to SuperSU) using the ES File Explorer? Like copying the new apk over to the /system/app ?
Any help is appreciated. Thanks

Maybe you need to fix permissions.
Did you have ROM Manager by any chance with root access granted? If so you could flash CWM from there, and then apply the latest SuperSU zip from CWM.

Use ES File Explorer and change the permissions on /system/bin/su (or /system/xbin/su) to 6755 and you will be all set.

I see that the Permission of the SU from ES File Explorer is rwx r-x r-x . I believe this is already the correct permission?
I'm thinking if it's possible for me the get the latest binary for Superuser and replace the SU file using the file explorer. Will this work?
Unfortunately I don't have rom manager as well.

Those are not the correct permissions. The setuid bit needs to be set as well. What you posted is 0755, not 06755.

ES File explorer doesn't seems to be able to allow me to set the setuid bit.
I guess I just have to figure out how to use adb to do that.

I just checked and you're right. (i only ever use root explorer.)
In a terminal window:
su
mount -o remount,rw -t ext4 /dev/block/mmcblk0p1 /system
chmod 06755 /system/xbin/su
I would also symlink it to bin:
ln -s /system/bin/su /system/xbin/su

I managed to do the above. However still no luck with updating SuperUser binary and still encounter the error "Gaining root access..... Failed"
I believe after doing the above I should get permissions (-rwsr-xr-x) but i'm seeing the permission as -rwsr-sr-x
seem like something is wrong?

Strange. I have no idea what the problem would be then.
What I would do is boot up CWM, and re-flash an su package.
The permissions are fine: -rwsr-xr-x = 04755 and -rwsr-sr-x = 06755. Both will do the same thing.
Also I see nothing in the OTA package that would relock your bootloader. You're sure that's exactly what happened? If it did, maybe someone here could pinpoint the component of the OTA package responsible for this?

Looks to be a problem on su.
I download the latest binary and replace it using adb + busybox and now it works fine.

I'm having ROOT Access/SU problems. I need access to some files in the root/System/ directory and am unable to get r/w.
I s-off/rooted my Incredible 2 using the instructions on this tutorial http://forum.xda-developers.com/showthread.php?t=1219246 (The How To Root DInc2) and the instructions on the revolutionary.io website. (Essentially: Get S-Off via revolutionary> flash recovery CWM> flash su.2.3.6.3-efgh)
The revolutionary s-off process was successful and I got s-off.
I flashed Superuser su.2.3.6.3-efgh-signed.zip and seem to have Root. The binaries are upto date according to the app.
I have Titanium Backup, Root Check, BusyBox Installer installed. I had to install Busybox. All these apps run properly. I can wipe, backup, and install system app data via Titanium Backup. Root Check says I have root access and the correct Busybox location.
The Problem
I installed Root Explorer today and I am successfuly able to change from R/O to R/W and back on the root directory and on the SDCard. But when I go to the System directory I cannot change from R/O to R/W.
Tryed to connect to the Inc2 from my PC (winXP) via ADB. ADB Devices command shows my device. When in ADB Shell the default profile is $. When I type in SU I get back the message "SU: permission denied" I do not get a prompt from Superuser on the phone to give access.
(To see if the problem was in the steps I was doing I tried this on my rooted Incredible 1 and in Root Explorer I was successfully able to remount the System directory from R/O to R/W and back. But the same problem with ADB I have on both Inc1 and Inc2. (Inc1 was s-off/rooted with Unrevoked3/Unrevoked Forever, with su.2.3.6.1-ef-signed.zip.)
I'm not sure what went wrong where, since some of the root access is working. Any suggestions? Help?

that happen to me before. and the only way to change the r/o to r/w is going back to stock,if you have a stock back up rom you have to run that one,and then you can install the rom you want,and the you can change with root explorer s/o to r/w.if you dont have a stock back up you have to install the PG32IMG,and then you have to go to revolutionary and install the recovery.dont forget to turn off the fast boot.if you dont know how to do that,setting,power,and then uncheck the fast boot to off.
I dont think your binaries are up to date.
Try installing SuperUser 3.0 and report back - with updated binaries. The binaries should show at least verison 3.
I had issues to with the Root access that came with revolutionary and this worked for me.
You are on the right path though.
I'd like to try and avoid flashing the original Verizon ROM back on the phone. I'll leave that for last resort. There is stuff that I've done to the phone that I would rather not have to go through to do again.
In Superuser I tap on the 'check for binary updates', the respone after checking is that the 'su is up to date'. The version that I'm currently using su.2.3.6.3-efgh-signed.zip the files are modified in 2008 and 2011. So it seems to be upto date.
Is there a place I can download the latest version of Superuser 3.0?
The version of Superuser3.0 that I was able to find, when I open the zip file, shows all the files last modifed in 2008.
You wont have to flash back.
Follow my advice and install the new SuperUser on the market. It is 3.0.
Once in the application go to update binaries and update anyway.
Reboot and let us know how it works.
Those 2.3.6.3 binaries didnt work for me either. Now im fine after following my own advice.
Oh - and also - check to make sure that you have allowed specific permission for the application you are having issues with - within SuperUser.
You will be pleased to see that the new SuperUser can automatically grant Root to all of your apps...dangerous, but speeds things up.

Ugrading to the latest Superuser 3.0 and updating the binaries did help! Still some problems, but making progress.
I can connect via adb shell and get SU privileges. # shows up! I think I now have r/w privilege on /system. Not sure, but I think I do. (I'm still learning adb shell and linux commands)
But I still cannot get Root Explorer on the phone to work properly. Same problem as before. I can get r/o and r/w on the root directory, but cannot get r/w on the System directory. The button turns green, but the nothing changes.
I can live with getting SU privileges in adb shell, but it would really be nice to get r/w on System in RootExplorer.

Be sure to click thanks if you haven't already. Lol.
Try wiping data...force closing and uninstalling root explorer (thru manage applications...not CWR).
Should take care of it. And because you are rooted you should have full access to filesystem via adb already.

You paid for Root Explorer. It doesn't work for you. I'd be bugging the developer since it's clearly not doing the job.

yes, i looking more and more like i may have to flash my rom backup that i made right after s-off. let's see, since i can get su in adb shell i can get done what i need to so reflashing is not utmost urgent.
i'm not sure it's rootexplorer since i've not seen other posts with similar issue. if the flash of the rom backup does not do it, that may then point to rootexplorer. lets see...
I'm currently running on yakju 4.1.1 and I think I've accidently messed up my root privileges. I think I somehow messed up my su by clicking the root button in Nexus Root Toolkit 1.5.
Cause:
I've always used Voodoo OTA Rootkeeper to keep my root. I update the SuperUser app via the market and found out that my su binary was out of date. I've tried various ways to update the su binary but gaining root access always resulted in a failure within the SuperUser app so finally I decided to try out the root toolkit. By clicking the root button in the app I think I've only made my situation worse....
Now I'm seeing that my su backup in the Rootkeeper app is messed up and I can't seem to delete it. The RootKeeper app seems to think that I'm still rooted and I can't successfully do the "temp-unroot" as well. Other apps such as Titanium Backup can't detect my root status now....
Can anyone offer any suggestions without re-flashing my image?

As of Nexus Root Toolkit 1.5.2, I do not see yakju 4.1.1 in the list of supported devices.

I've figured out the solution to my problem: just re-push su & Superuser.apk to /system/su and /system/apps/Superuser.apk respectively.
Basically I referenced this thread: http://forum.xda-developers.com/showthread.php?t=1531865
Since I already have su and Superuser.apk in my /system (although I think I tainted both files), all I needed to do was grab clean versions of su and Superuser.apk and adb push em through and type "adb shell su" to make the # appear. I was even able to update su binary within Superuser, awesome :good:
Note: Once you do step 6, your prompt should change from $ to #. If not, it did not work.
7) Mount the system partition as rw: mount -o remount,rw -t ext4 /dev/block/mmcblk0p1 /system
8) Copy su to /system: cat /data/local/tmp/su > /system/bin/su
9) Change permissions on su: chmod 06755 /system/bin/su
10) Copy Superuser.apk: cat /data/local/tmp/Superuser.apk > /system/app/Superuser.apk
11) Change permissions on Superuser.apk: chmod 0644 /system/app/Superuser.apk
12) Mount the system partition as r/o: mount -o remount,ro -t ext4 /dev/block/mmcblk0p1 /system
13) Rescind root: exit
14) Exit the ADB shell: exit

If you have an unlocked bootloader and the SU binary is still on the phone, this should fix the issue:

As for the superuser APK, it does NOT need to be moved to the system folder, it just needs to be downloaded from the marketplace and will function if the su binary is present with proper permissions.

I guess it makes sense, the only real issue was my su permissions.
I did this with a locked bootloader btw.
When i first bought the t989, i rooted the phone manually by flashing cwm and the su.zip file. Everything seem to have worked then. A month or so back I updated the phone to ICS through the Kies Air application. I tried browsing through my root files and found myself unable to do so. I decided to check my superuser application and noticed another su apk. It wasnt the android face but a superman android icon (sua). I updated the binary on the sua and my phone started resetting on its own. I have flashed stock roms of ics and the problem consisted. Eventually fed up of the 4.0.4 version i flashed back to GB which seems better in phone performance.
Back into stock/unrooted T989, i flashed root again. However i used root checker pro and it verifies root access however I find myself unable to browse root files. Is there another possible way to check through? maybe via terminal emulator?

I'm having the same issues with 4.0.4. It's getting me pissed and contemplating doing 4.0.3 where I had access to system files via root explorer. I've been going through different ROMs on 4.0.4 to see if that would alleviate the problem. So far, still issues saying something to the affect that I don't have superuser access even though I can run any rooted application.

I've tried updating superuser from the market, even bought the elite version. Tried SuperSu and thus far, no luck. Tried using titanium just now and it didn't work on superuser.

Ive tried updating su before and the binary which seem to have no changes. As for making su a system app, when i flashed unroot to root, su appeared without me installing. Also, su doesnt seem to appear in titanium backup

I'm returning my phone today to fix the broken power button, the stiffness of the D30 case may have caused this.
Is it possible to direct me to a fully working 100% root?

I'm kind of wondering if flashing back to completely stock via Odin, then trying all over again will do it? I know as soon as I did the Kies update from 4.0.3 to 4.0.4 caused this issue on stock rooted.

Just Odin back to stock and then reroot. Its super simple. Maybe if you can get back to you recovery somehow you can just flash another rom and install it where you can have your supersu access back.

I just rooted my ATT S3 4.3. I did everything the tutorial said to do except when it got the part wherr odin reboots ur phone, i had to reboot it myself yo makr it go in cwm recovery. So once I was done rooting my phone and turned it on for the 1st time I instantly was greeted with superuser has stopped and security violations. So I tried random fixes from google and it keepa teling me there is no binary code. I can't figure out how to fix this, can anyone help me?

I can't find it anymore. It was on this forum and it said it was for att S3 phones. Is there an area where all the ways to root are?

Try Chainfire's CF autoroot with SuperSU, including google play SSU update to App? It sounds like you failed to reboot into recovery first before doing a normal boot. After flashing via odin, the root is temporary and will be wiped during a normal boot, unless you first reboot into recovery. So, you have to uncheck the reboot after flash option in odin. After successful flash, pull usb cable and boot into recovery using three button method.
Did you use CF autoroot? Try it again.

I did reboot into recovery. I remeber the files I downloaded were named clockword mod and another was called superboot with cwm. Don't k ow if that helps.
I'm having a big issue though, I had tried fixing it by flashing dif ways, now whe I try tk flash it. When it loads up in download mode, my phone turns off or of ot doesn't do that, after I flash my phone and it reboots, it shows a red screen.

I just tried that root method you suggested and I still have the same issues. I don't know what to do!

Download and install the latest APK for SuperSU like MrW0lf suggested.
I was having the same exact problem (unfortunately, SuperSU has stopped), so I downloaded the latest UPDATE-SuperSU file and extract it to have supersu APK, put it on my SD card in my phone and installed it from there.
It was strange because the version that was already on my phone was the same 1.89 version that I downloaded, but regardless, when I clicked on the APK, it saw it as an update, it asked to disabled knox, and worked like a charm.

I have an SGH-T999. I did an OTA of 4.3 UENC2 version, then rooted the phone with Root66 and SuperSU. I installed TWRP recovery. Both ES File Explorer and File Manager by Rhythm do not see the phone is rooted. All other apps I have tried work with root. It seems to be a permission issue from debug logs. ES File Explorer is a 4.1.3 debug version (since getting this I can't get a reply from them) Here are the logs:

Have you tried the released, non debug version? Did you factory reset after root66 and run clean (no restoring of backups)? Open supersu and itll check if the binary needs updating. Have you installed busybox?

Please Open SuperSu and see if it prompts to update the SU Bianry. Do so if it does. Second, try Root Checker app from Play Store. See what it has to say.
I tried the release version first which is what led me to email their support and get the debug version. I have the latest 1.94 version of SuperSU. I did NOT do a factory reset, just an in place upgrade and root. I did uninstall and re-install both apps and delete them out of SuperSU so it would ask for approval again.
Root checker is one of the apps that does work, along with FX Explorer. Root checker sees I am rooted and sees Super SU 1.93. This is interesting since it doesn't match the 1.94 that SuperSU thinks it is. Root checker is looking at /system/xbin. How do I see where my shortcut for SuperSU is pointing? It of course doesn't show up in my applications.
Unix Utils shows toolbox installed. It doesn't see busybox. I thought that busybox was installed with FX. But I guess that is internal? I remember one of my apps like FX or Titanium Backup or TWRP recovery saying it had to download busybox to finish the install. Is it ok to install it myself? I was afraid of a version conflict.
I just got a reply back from Rhythm who thinks the problem with them is an issue in Android 4.3.
Did you check within the SuperSU to see if it prompts to update SU binaries ?
Reflash the supersu pkg. Download in my sig.
