Strange root problem - T-Mobile Samsung Galaxy S II SGH-T989

So, I was fully rooted (4.0.4 Stock) I just went in to try to remove another system app, using "System App remover" and it tells me that it cannot mount system R/W, so I go into root explorer, and go to system, and click the R/W button, and nothing.
System app remover was working fine before, as I have removed apps.
Any ideas?
**UPDATE**
I just restored back to when I KNOW I had root, and it is doing the same thing.

Maybe this time you denied SuperUser permissions?

Restoring nandroid backups has been nuking permissions for months now. You'll have to wipe and fresh install.

What about Fix permissions? would that fix it?
**Edit**
Interesting..............fix perms will fix it, but you can only go in once (system app remover), if you go out, and back in, it gives the same error

When you say wipe and fresh install, how would I do that with Stock 4.0.4?

You don't need to fresh install. Grab the superuser flashable zip from androidsu's website (get the ARM one)
http://downloads.noshufou.netdna-cdn.com/superuser/Superuser-3.1.3-arm-signed.zip
Boot to recovery and flash it. Fixed it for me.

If your recovery isn't stock, you should be fine. If it is, you'll probably have to Odin a custom one on your phone and go from there.

But I was wondering what was meant by fresh install? the whole ROM?
I am using CWM

Wiping everything and then installing. Darkside super and cache wipes first, then the ROM. Factory reset before you install can't hurt either.

SU Binary Update Failing

See pic guys. Anyone know why this is failing and a possible fix? I am running Mr.X's T-Mo ICS leak with DarkSide Venom 03 kernel. Thanks!
Try it after a reboot. That's what has worked for me when a su update flunks.

Rebooted and now it's failing at "copying su to /system"
Rebooted a second time and now it's failing at the same spot it originally was in the OP.
Have you tried a Darkside cache wipe? Or another super user app?

Yes, tried darkside cache wipe. Still failing
I had that same issue, I just did a complete Re-flash of my ROM and everything worked fine after that.

Don't wipe, just try flashing your current ROM again, or download (search for it, sorry no time to link) su.zip to flash from recovery. Either way should work in restoring SU and a matching binary, after verifying it works try updating again (app first, than binary).
Uninstall superuser, then re-install from market, then try updating the binary through superuser. Should work.
I deleted the SU from system/app and then installed SuperSU funny thing though after a reboot i still see the old super user in app drawer and it opens...weird

Thanks. I'm not rooted so I guess I will have to factory reset. This is really odd. Never had this happen on any of my android devices so it got me stumped.
I rooted and flashed cwm. Clearing dalvik and cache along with change permission didnt fix it. Had to factory reset and was able to side load. Gonna restore my files back via TB and hopefully it wont happen again.

So even after i factory reset my root isn't going away.. how is that possible.. do i have to unroot this thing to start over? I need this to be like it came out-of-box which i thought a factory reset would do.. i did the reset from CWM Recovery and wiped all data as well as both types of cache and i am still rooted.. any tips?

this has happened to me before....you are probably going to have to boot into download mode. Hopefully you have the samsung drivers loaded onto your pc. When you get odin booted up and it recognizes your device, click the .pdf button...that is the only button on odin you need to press!!!! When you are prompted for the file load the .tar file then click start....It may take 3-5 mins to finish it will reboot. You will now have to root again. Hope this helped.

Same problem here cannot wipe cache but I can install roms they just wont work properly I think its something with permissions....
Ok so here is how I fixed it. First I followed this post http://forum.xda-developers.com/showthread.php?t=1513359 steps 1-8. Then I followed this post http://forum.xda-developers.com/showthread.php?t=1481141 steps 1-7. Then I downloaded superuser3.1 from here http://forum.xda-developers.com/showthread.php?t=1698897 Installed that with recovery. Finally I re installed the rom that I was trying to install.
I know its a bit much but it should get you back up and running.

Doing this worked good. I did flash 3 different recovery versions before rerooting my phone I think the permissions were corrupt.

[Q] Lost root

Ok so the story goes like this:
I have stockorama V2
Stock kernel
I was trying to install busybox with busybox installer from the play store, but it said I didn't have root access
So I went to superSU and did the uninstall from the app itself
Rebooted, installed superSU from play store, it told me to update su binary. Then it failed.
when I try to open SuperSU it says that "I don't have su binary... ...that's a problem" and then closes the app
I think I lost root, but I was wondering is there anyway to get it back without a PC.
I still got twrp recovery so I can still flash stuff

You can flash SuperSU via recovery using the link in my signature. Stockorama includes busybox. Don't install it.

You did flash in recovery correct? Cwm or twrp? What version?
Not sure if its any different than Aerowinders, but you can try SuperSU linked in my sig. Or use Superuser, you should find both there.

Flash it in twrp 2.5 I think, did it 2 times... same thing
Its weird because I still get a toast (when I bootup) that lightflow gets superuser permisions; but when I open Titaniumbkp it gets stuck on a message "asking for root rights"
Btw superSU is as a system app so I can't uninstall before flashing the supersu.zip you guys gave me.

You don't uninstall, just flash over the old one. And wipe the data for it also.

Flashed zip in twrp, wiped cache and dalvik, fix permissions, reboot.
Open supersu, and get the same message
I'm juat thinking reflash the rom the thing is I wanted to backup app+data with Titanium but no root. So any suggestion to a backup app that backups data without root...

Well, make a nandroid backup just in case. I dont suggest using it as it may just bring back the problem. But you will at least have everything backed up if you need it later.
Aside from that, you can use the recoveries built in file manager. Also you could boot into recovery and then try to ADB pull what ever you need.
But I would reflash your rom. You only need to wipe dalvik if it is the same rom you are using now. But if that doesnt fix it you will need to do a factory reset at that point.
