dear XDA members ,,, as i got a lot of solutions in this form and i see the OTA updates are making problems for many users ,,, here is the solution for people who unlocked the bootloader and installed CWM recovery ......
ON STOCK ROM ...
1- you dont need to relock the bootloader ..
2- download voodoo rootkeeper .. (from market) and run the program and click protect root.
3- you will need your original recovery that you changed to CWM ..
example: if you had 1.28.415.9 and you installed CWM then you need recovery.img for 1.28.415.9 .
install the stock recovery from http://forum.xda-developers.com/showthread.php?p=24738635#post24738635
4-install recovery.img from fastboot ..
5-boot into phone and OTA update the one x to 1.29.xxx .
6-after update is done just open rootkeeper and click restore root ,,
7-DONE

Actually I never really had to relock the bootloader to apply an OTA... just flashing the stock recovery and keeping the unlocked bootloader + rooted custom ROM did the trick for me... Tho its not a really good idea to install an OTA patch on top of a custom ROM.
i added ( FOR STOCK ) what i know if you have custom rom you wont receive OTA ... will you ???!!!! ... my solution is about if you are still on Stock ...
if you search google youll see people talking about locking the bootloader and removing root and all kind of crap ...
...since the radio is part of the rom now and the only "advantage" of flashing a RUU is an hboot update.
So what is the "advantage" of an newer hboot? If someone finds an exploit on the 0.43 hboot which is fixed on newer hboots you gonna cry (-:

I also didn't have to relock my bootloader.
Just restored my default back up with cwm before root. Fastboot restore to original recovery (stock), update and done. Went back to fastboot and restored cwm, rooted the phone and done.

Has this been confirmed, that the radio is flashed when flashing a rom?

What i did was flashed stock recovery, downloaded and installed the update done, that's all i did and root was intact, that was like last month
I'm still haven't issues installing the OTA update.
I followed your instructions and I still get the red triangle error

just did some fishing, and it's because my stock rom is so heavily modified that the OTA won't update anything

the OTA will not include CWM, as CWM is not stock.

Whatever Rom version is shown in "about" e.g. 1.26, 1.28, 1.29 , then this is the recovery version you need

My phone version is 1.29.720.11, but in the OP link, I can find only 1.29.401.7 and 1.29.401.11. What can I do?

I unlocked my boot loader and flashed clockworkmod Recovery V5.8.2.7.
I just received the update from HTC:
1.28.720.10 to 1.29.720.10 for Asia India. When clicking on the file to update, the phone reset itself and the clockworkmod recovery screen appeared with an error.
So I was unable to update the phone.
I have uploaded a screenshot with all the info.
The phone is an HTC One X unlocked version bought from amazon (Made in India)
I am not sure what other information is required so if you need anything just let me know and I will update.
You need to flash a stock recovery prior OTA update. The update will fail with a CWM recovery installed.

I think you also have to re-lock your phone after restoring your stock recovery.

Flash a custom rom instead. Most devs have all the latest OTA updates in there builds anyway. As soon as a new OTA update is available then the Devs will update there roms.

If you would like to stay with stock kernel and ROM, there is no other way.
Even if you choose to flash a custom ROM, you will need to do these steps anytime you would like to go back to stock ROM delivered in the form of an RUU.

You can find what's available in this sticky thread.
As for recommending, I would suggest you check out the OP pages for different ROMs and read up on the latest posts in each thread. This will give you an idea on what the ROM looks like, the features it supports and the level of support you will be getting for each ROM.

NO. Your phone should have the original recovery on it and, if it's unlocked, it has to be relocked to get the RUU to install.

So you are among the lucky few. Hats off to you.
I don't know what it's like to have s-off but normally install stock recovery before rellocking. Otherwise, you will be stuck.

I flashed a stock recovery and relocked the phone. I then downloaded the update and started to install it. The phone turned itself off and a logo appeared of a download sign but then after a few seconds it changed to an exclamation mark and stayed like that.

I tried to go back to stock...basically I installed a NANDROID compatible with my system...everything went fine till the last update JB...
I downloaded it...OK ,
After INSTALL NOW...It reboots the phone to install...After that, shows a message and go back to BOOTLOADER...I cannot see well the message... What can I do?? It's driving me crazy

I'm using a Stock from a recovery Nandroid, not mine but the same CID 044... I guess I shold go back to RUU...but I've tried without any success
I can't even use my RUU file...donno why... I have this file: (RUU_ENDEAVOR_U_ICS_40_hTC_Asia_WWE_1.29.707.11_Radio_1.1204.105.14_release_260743_signed) and It works, not now but It used it once...do you have a suggestion so I can use it again??

I'm not...I'm on 2.17 yet...JB update does not install...that's the main problem

You need to relock your bootloader (if you haven't already) and run THIS RUU.
The problem is that the version of software you're running (2.17) is newer than the RUU you're trying to install (1.29). you can't revert to an older RUU. you can only flash the same version or newer.
Once you've flashed the RUU, you'll be able to download and install the JB OTA.

Well I am currently running Team Venom's Viper but I have HBOOT 1.31. My device's CID is HTC_J15.
I have checked the list of RUUs but unfortunately all of them are older than the version I need, which has to be 3+.
Does anybody know if there's an RUU available for my device? It is an ubranded WWE version, 401 if I remember correctly.

In my experience the 2.17.707.xx RUU Asia WWE does not work properly. It does its thing installing everything, relocks the bootloader but somehow it does not reflash the recovery. After 4 hours trying everything under the sun, I gave up and looked for a nandroid of 2.17 for CID_044 and restored from that, reflashed stock recovery and went to town with the OTA.
That's very strange, no such problem when I did the same thing. You'd think it would throw an error or something if a component of the image failed to flash.

Did you run it when your bootloader was unlocked? Maybe that's the problem? I ran it with the bootloader relocked and didn't ahve any problems. I was under the impression you couldn't run an RUU with an unlocked bootloader, but haven't tried it myself.
Refer custom to stock thread posted earlier. Go through step by step procedure. It will help u to be on stock Rom. After that flash any Rom suitable to your CID only

What I did... NOTHING else...
Only Reboot the phone...wait to initialize completely, then reboot again, when came a little update about 1.25mb...installed and reboot...after that...JB update installed correctly.
If someone else needs...Someone like me who relocked the phone waiting for JB and Has problems
1. Unlock Phone
2. fastboot flash recovery recovery.img (from nandroid 2.17)
3. fastboot flash boot boot.img (from nandroid 2.17)
4. fastboot erase cache
5. restart bootloader (I like to do this way)
6. Install RUU_ENDEAVOR_U_ICS_40_S_hTC_Asia_WWE_2.17.707.3_Radio_2.1204.135.20_release_278245_signed on windows
7. Wait to boot completely, then restart phone.
8. Update Software ...a little update came before JB. (about 1.25mb)
9 Reboot phone.
10. Update Software... finally to JB.

The RUU ran with no problem, and it relocked my bootloader, but it still did not receive any OTAs. So I had no choice but to do a nandroid restore instead (recommended anyway )
I have an unlocked HTC One X, running Stock Rom and CMW recovery. CID 621. (never installed any custom roms, only CMW recovery and unlocking the phone)
I haven't updated my phone in a while since OTA failed due to the recovery. Last night I reverted back to the stock recovery and updated, successfully, OTA version 2.18.751.7.
After updating my One X informs me there's a JB OTA update, 3.14.751.20 but when I try to run it, it reboots, shows the green progress bar for about 30 seconds then shows the red rectangular of failure (rebooting starts up the device normally).
I tried reflashing the stock recovery, tried the recovery from the latest RUU for my CID (RUU_ENDEAVOR_U_ICS_40_S_Chunghwa_2.17.751.2_Radio_2.1204.136.21_release_275588_signed) but the OTA update keeps failing.
I also tried disabling SuperSU and used Voodoo Root Keeper to disable root, still the same red rectangular. Tried using the nandroid backup and running through the updates again, but same issue (2.18.751.7 installs correctly, but 3.14.751.20 fails).
Any help would be appreciated!

Have you tried relocking your bootloader? 'fastboot oem lock'
Also, maybe try flashing the stock kernel as well?
Did you remove anything?
You might try running an RUU if you can one one new enough, or restoring a backup (there are threads here for both backups and RUU, but look on htcruu.com, the device is endeavoru)

I'm hesitant about flashing a kernel, as I did not change the original stock one, and it feels too risky.

I thought about that, but the most recent RUU for my CID is older than what getvar is telling me (INFOversion-main: 2.18.751.7. latest RUU I found is: RUU_ENDEAVOR_U_ICS_40_S_Chunghwa_2.17.751.2_Radio_2.1204.136.21_release_275588_signed).
As to relock, I went through quite a few similar threads and in all relocking did not make any difference. Also, if that was the case, I don't think the first OTA update would have gone through.

I think the update to hboot requires it to be locked, your previous update was most likely ok with the hboot it was on.

Sorry to be a noob but what do you mean? I thought I couldn't use a backup older than the one listed under "main version". Or am I confusing things?

Update: locking the phone did not help
I locked the phone, but the update still fails at the same location. The green bar expands for about 20 seconds, covering about a quarter of the bar, and the the red triangle appears.
You could always attempt to update the firmware/hboot manually download it here for your cid http://forum.xda-developers.com/showthread.php?t=1957376

the tool looks easy enough, but they ask to include the custom JB rom of my choice. I want the Stock update, so what do I do?
The manual update list points to a broken link. I did find it somewhere else. How do I manually update? are these the commands?

do I need to unlock my phone again? can I use the same Unlock_code.bin I got from HTC last time (saved it) or do I have to go through the process again?
I'm asking all these questions because I don't want to end up with a bricked phone.

There are a couple of really good stock looking ROMs on there with enhanced performance. There's one mentioned in my sig, it may also be the easiest route out of your issue using the tool posted above

Yes I am running to issues... turns out that manually flashing the firmware.zip just got my phone into a bootloop since it turns out you must install a custom rom after it. I did not want to install a custom rom just to get the OTA update to install. now my phone keeps rebooting and I'm waiting for ADHD to download (takes FOREVER! for some reason).
This really sucks. I wanted to stay with stock, and it looks like I can't even go back anymore, can't even restore with a RUU.

There should be a RUU out for your region soonish but Until then, a stock style custom rom is a brilliant option beleive me

downloading maXimus as per your recommendation (and because ADHD will take another 9 hours
You can restore an older backup, it's just like installing a custom ROM.
You might also be able to install a 2.17 RUU on 2.18, I'm not sure the hboot was updated. It's also possible to dissect an RUU and install the bits manually.
You have a few options.

Sorry did not find a solution. Ended up installing a custom rom, even though I wanted to keep using the stock one

I hope you did this the other way around......first flash the recovery and the relock the bootloader.....
But if you are on stock rom, kernel and recovery you do not have to relock the bootloader, it updates fine with a unlocked one.
Guys, I got the new update, but I can't install it. It gives me red icon during installation. I have installed the stock recovery, but still can't install the update... Can someone help me? My device is rooted and the last time I got an update, I installed stock recovery and installed the update successfully, but now I can't install lollipop update, although I have done the same, can someone help me?

this problem happen to no-root, no unlocked bootloader user too
i don't know why?

Don't even how to unroot. Whenever I hit "full unroot", it deletes super SU from my device and asks for a reboot, but when I reboot it, it gets back again..
And I don't even have a nanodroid backup..

Yeah i went back to stock unrooted and flashed the stock recovery from your ruu post and then relocked the bootloader but whenever i try to install the update, it doesnt boot into the recovery. It goes straight to the bootloader i think im going to run the ruu zip you provided and then try again.

Yeah, same thing is happening to me. My bootloader is unlocked, and the phone is unrooted. I'll download the update, but whenever I hit install, it'll go to the install page, and the green bar starts loading, then it goes to the pre-recovery screen thing with the red triangle with the exclamation point. So I've tried two things. Number one is pressing and holding the volume up and tapping the power button, which gets me to the stock recovery screen. It says that the update was aborted. So I go and restart the device. This seems to have deleted the update, so I have to download it all over again. I do that, then install. Again, I get to the triangle ! screen, so instead, I press and hold both the power button and volume up (which is basically the equivalent of a battery-pull. I get the same result. The update has to download again. What do I do? I was contemplating locking the bootloader and even factory reseting the device. I've kind of been waiting for this update since 5.0 was released period, so I would like some guidance as to what to do next. Oh, and just for helps-sake, I'm using a mac, not a pc. I don't own a pc, only an iMac, and this MacBook Pro.
Anyone know what's going one? Please do help

After flashing stock recovery and relocking bootloader you need to flash RUU with your CID number.To find out your CID number install CID Getter from Play Store.After you find out your CID number you need to download RUU.And that is hard part of problem... I didn't find my RUU anywhere on web,but you maybe will have better luck.
When you have your RUU just rename file in OP9CIMG.zip and put it in to your sd card.After booting into bootloader phone will search for OP9CIMG.zip file and if CID number is same on phone that file will be flashed.

k, my CID is HTC J_15.. How could I find my RUU now?
My VMUSA 816 is still upgrading, but it looks like the solution for some people will be running the last version of ruu???.exe from HTC for your particular version of the 816 before downloading and installing the OTA update for Lollipop. Before running the ruu???.exe file the OTA would just reboot into the bootloader. Note that you should run ruu even if you already have that rom version installed. I already was running 1.50.652.3 but the OTA just wouldn't work. So the ruu just reflashed a clean version of the same rom.
As others have done I unrooted, installed stock recovery and then relocked bootloader before running ruu???.exe and installing OTA update for Lollipop.
The reason flashing a stock recovery would work before is because when doing a smaller update that stayed within kitkat it didn't do an entire system check, when upgrading to lollipop it checks EVERYTHING so the only way it'll install is having a nice fresh stock ROM from an RUU with a locked bootloader and stock recovery.
If s-offed is it necessary to relock bootloader?

Did you check the HTC site yet? Look for ROM updates there first. I assume they have country specific sites that will list ROMs for your region. For me in they show all their ROMs for US carriers.
