Dialer won't pop up on incoming calls - Samsung Galaxy Nexus

Using Modaco rom, franco's kernel - dialer won't pop up on incoming calls. Phone rings/vibrates, calls show up in the log but nothing pops up to answer the call. Anyone else seen this? This is happening with stock and other dialers.

[Q] Missed call notification when declining incoming call

By default in AOSP, if you decline the incoming call, there won't be a missed call notification. In Sense, you get one on every missed call, even if you decline it. Is there any way to change the behavior in Sense to match AOSP? (Besides installing CM6 of course ) I'm getting tired of clearing my missed call notifications all the time.
So I guess there's no way to get around this?

Audible message notification during a call?

I would like to hear a notification when I get a text message during a call. Anyway to do this? I'm not sure i this is a Sprint issue or an android specific one. (I'm running CM6)

[Q] Trouble Rejecting Calls, Unread messages after reboot!

Hi All!
I´m using ILWT´s CM7 rom on my Desire Z. Great rom, but I wonder if anyone is experiencing the same issues I have.
Firstly: I cannot reject an incoming call. It just keeps ringing. Until it is out-timed Iguess. It says that the call is rejected but keeps ringing, and the phone locks until it´s quiet. If I hit the powerbutton on incoming call the same thing happens, but without the sound ringing. Now this may not be a rom issue, since I had this problem before I rooted and installed this rom. I kind of hoped that this would make it go away... Anyone have a solution?
Second: Everytime I reboot the phone alerts me in the notification bar telling me I have a number of unread messages. A number witch is increasing with every reboot. This issue is not that frustrating, but I wonder if there is a fix for it.

GVoice Caller Announce / Dialer Touchpad

I use GVoice for incoming/outgoing calls. When I receive a call, I swipe to answer, GVoice then announces the caller, and tells me to enter "1" to connect. When I activate the dialer touchpad, there is no response when I hit "1" and the screen blanks. I'm sure there is no touch tone sent.
What's wrong here. I've had to turn off GVoice's caller announce because I can't connect to incoming calls.
This is a VZW Nexus FWIW.
Are you running a custom ROM? perhaps something has been disabled. I run codenamedroid, and when I press 1 I here no tone yet Google voice still connects me to the call.

Pure "out-of-the-box" ROM. No root. Plain vanilla.
EDIT... this is interesting... I ran into a related problem. I answered a call I was expecting on the first ring, and GVoice told me to press "1" to ensure I wasn't an answering machine. Of course I couldn't do that since the keypad was disabled on received calls. Out of curiosity I disabled the pattern lock I was using, and now I can answer immediately. Just wondering if the pattern unlock is related to my keypad disable, since my incoming calls come to a locked phone. I haven't tried other lock mechanisms because frankly I'm tired of this problem... waiting on CM9

Incoming calls not received

Have searched for any similar posts and found nothing helpful.
I have Samsung Galaxy Note II, OS is Android ver. 4.1.2, remainder of system is stock.
Phone functioning normally then discovered that incoming calls were not being received at all. I mean by this that: phone did not ring at all - no ring tone, no vibration, no notification of any sort. Also no voice mail message received, no record of any sort of an incoming call.
Phone still functioning normally for outgoing calls and other functions.
Does anyone know what the problem might be??
