I've tried a few different file managers and I have the same issue with all of them. If I delete something from a file manager, when I connect to my PC, the file I deleted still shows up on the PC. Also, some files and folders that show up in the file manager don't show up on the PC when the tablet is mounted. Anyone have any idea what's going on?

are you trying to delete ANY file or IMAGE, AUDIO, VIDEO file?...
from external sd card or internal?...
It doesn't matter what type of file. I have no external sd, so it's all from internal. The file manager deletes the file and it doesn't show up in the file manager, but the file still shows when I mount to my PC.

Ok. I just tried to open a zip that I had deleted on the tablet with a file manager. It shows in Windows Explorer, but when I double click it, nothing happens. Somehow Windows isn't "refreshing" even when I click refresh. I'm still confused. Sometimes a file shows in the file manager, but not in Windows.
I'm on the stock OTA ICS.
Try clearing Media Storage and Gallery data/cache. Then reboot, the database should then reload.

Can you offer any additional info? Have to used Root Explorer or a similar app to confirm without a doubt the files are gone and not just in a different place? And I am slightly confused when you say "move to sd card" because internal storage and SDcard on this phone are the same thing since there is no EXT..
I copyed files as usualy from computer to the nexus
After the copy (I copied the new google apps zip) this zip is ont the storage, but all other 12Go of data in the card are disapeared
I try to find it with windows, root explorer, es file explorer, I tryed android undelet tools and windows undelet tools... noone can see the MTP memory of the nexus .......
And I disabled the upload to picasa..

Do you have fast usb charge enabled? If so, try disabling that.
all files in /sdcard disappeared
The same thing just happened to me. I copied 386MB of music to /sdcard/Music and all appeared to be fine. I used Windows7 x64 Explorer over USB mounted as MTP.
I noticed that I could not copy some files back to Windows from Nexus. After messing around with that for a while I used ES File Explorer to check out /sdcard. Only a few folders were there, as if /sdcard were newly created. I could still see all of the lost folders in Windows Explorer.
Am I screwed?

This is from a corrupt MediaStore DB. I ran into something similar when moving files to my phone over WiFi using File Explorer. Afterwards only a handful of folders showed up when connected via USB. Only way to fix it is to reset the DB. Settings > Apps > All > Media Storage > Clear data/cache. Reboot. It should completely rebuild the DB after the boot, and things should show up correctly again.

That did not get all my data back, I must have done something else while trying to fix it. The good news is that this fixed one of my next questions: How do I reset the Google Music album art? The suggestion above fixed that wonderfully.

I have some files on the internal storage, from the downlad folder, that i want to copy to my sdcard, but theres no file explorer that will let me do it. I've tried both Root Browser and Root File Manager. They just give me an error saying 'Operation Failed!".
If i plug it into my PC through MTP and tries to copy the files, either to the SDcard or my own computer, windows 8 gives me an "Unspecified error" or saying "The device is off, or file not found" (or similar).
What is wrong? Why cant i move the files from the internal storage? Hope you can help me.

Sounds like an sdcard issue
Try copying files within the sdcard and within the internal storage. If it fails only when copying within the sdcard, remove it and try on a card reader on your PC.

I can without problems copy files onto or from the SDcard(from PC), but not the _internal storage_, thats the problem. The SDcard (partition?) works fine.

To clarify, i meant the download folder on the internal storage chrome etc., saves files in that folder..
Wow that seems like a nice file manager, and it does the trick! No errors what so ever. But how come it works, but other root file managers doesnt?
And how come it doesnt work in windows? :<

I must admit that I didn't think X-plore would work either but am pleased it did; it does do pretty much everything as far as files and folders go.
I've not had a similar problem and cannot see a reason why the other methods you have tried don't work.
So I recently found out that I cannot delete the contents within my SD Card, worked before.
I have tried it with the stock file explorer from samsung and ES file explorer. What happens is that I try to delete it, it disappears, then I load up the file manager app again and the files are there!
I can load up these files no problem (video files and everything plays well)
Does anyone know any way to fix it?

Download, " Folder mount" from google play, it will detect a faulty SD card & will fix it, reboot, you will have again a functional Ext. SD Card.

sorry didnt see your comment about it being fixed.I deleted what i wrote.
Ok so I just rooted my S3, 4.4.2 Boost Mobile phone. Worked fine. No problem rooting it. Made a backup using CWM and then I installed Philz recovery and also made another back up. In recovery it shows BOTH backups, and says they are located in
/sdcard/clockworkmod/backup
then in individually named folders of the backups.
HOWEVER, when I look using ES File Explorer, I only have the clockworkmod folder, and it does NOT contain any other folders, no "backup" folder at all.
Ok, so let me try the Aroma File manager. Same thing if I go to /sdcard/0/clockworkmod/ there are two very small files:
".last_install_path", and ".recovery_version".
HOWEVER, I am able to navagate to /sdcard/clockworkmod/ and THERE I find the backup folder with my two backups.
Why can I not find this folder from android? Even when I use STORAGE ANALYZER and tell it to look at the system parttions and even when I use ES FILE MANAGER and tell it to do an SD CARD ANALYSIS, it doesn't show these backups. So I have copied a backup to the download folder of the sdcard and am hoping to copy it over to my PC before flashing a new ROM.
NOTE: I do NOT have an external SD Card installed yet.
I just thought this is super strange. ?? Anyone have any idea? Thanks.
Now after booting into Android and looking into the folder I copied to /Download, the folder is empty.. !?!? I'm SO LOST right now....
It SHOWS that space is missing on my device so these files DO EXIST SOMEWHERE but SOMEHOW it's as if they are being hidden... is it knox???
Ok if I use "MY FILES" I can see the folder that I copied to /Download and the files in it, but NOT if I use ES File Manager, and NOT if I use Root Explorer. However even MY FILES still shows clockworkmod dir as empty!?
Ok so it turns out it has something to do with a permissions issue. Why is that? I run Storage Analyzer and when I try to click on the recoveries I copied to /Download it tells me that I don't have permission for that folder. Do I need to run some kind of FIX PERMISSIONS on this? Is there such a thing for the S3?

I don't know how else to describe this issue I'm having:
Essentially sometimes I cannot create new folders, either in an explorer app (ES File Manager, Explorer (free, not Root Explorer), or other file manager) or when connected to my computers via USB. Sometimes I can do both. Similarly, sometimes I cannot move files from one directory to another, both using file manager or when connected to computer over USB. It seems random, I think?
An example of this is I downloaded a PDF file from SharePoint using Chrome. The file is downloaded to the /Downloads directory by default. I wanted to move this to a different directory (/Documents), but the cut+paste operation failed. ES File Manager says the operation can't be completed, Explorer just sits there and doesn't say anything. When trying to do the same thing over USB connection, Windows just sits there saying the device is busy, and it'll sit there indefinitely. But an OGG file I downloaded from Dropbox was free to be moved to and from several directories when I was testing file operations.
Basically it's like my device doesn't want to play nice with files and folders. I tried doing a factory reset, but that didn't seem to help. Do you think completely reformatting the phone (as in fastboot format userdata, and a fresh ROM + kernel install) would help? It seems drastic, but at the same time being unable to move files and folders is very strange and likely hard to pinpoint the culprit.
I'm rooted (using traditional SuperSU root) running Pure Nexus 12/01/2015 ROM and ElementalX 0.07 kernel (Enforcing disabled). I doubt any of those 3 factors are the cause of it, but every detail might help, right?

OK an update to this, I discovered that some folders that I thought I had deleted using ES File Manager are still showing up in USB storage when I'm hooked up to a computer. That is to say that what I see using a file manager versus what I see while connected to a computer are different.
Further to this, I am now getting "Permission denied" by the file managers when I try to copy and paste anything from anywhere to any other place. I have booted into TWRP to run Fix Permissions already. It looks like everything now has rwxrwx---, which means to me 770? It should be 775, no?
Any ideas what might be causing this permissions issue?

For this, I read somewhere that the latest ES File app has a recycling bin for when you delete apps. So, that needs to be emptied too.

I did indeed clean out the Recycle Bin (and subsequently disabled it), but the problem persists. But thanks for pointing that out!
OK, an interesting discovery:
If I am on my device and using ES File Explorer and if I am in the path /storage/emulated/0/* then I am unable to copy/move files and directories, as I've described above.
If I am on my device and using ES File Explorer and if I am in the path /sdcard/* then I am unable to copy/move files and directories, as I've described above.
If I am on my device and using ES File Explorer and if I am in the path /data/media/0/* then I am able to do anything I damn well please.
Now this makes a bit of sense to me, given the permissions for each directory. For example, when viewing the permissions of a file or folder when in /sdcard or /storage/emulated/0, I can only see the permissions as "Readable: Yes, Writeable: Yes", but I cannot change said permissions. However, when I view the permissions for something when under /data/media/0 then not only can I see the explicit permissions "rw-rw-r--" I can also change it if I wanted to.
I suspect this might be more than just the ROM. Perhaps I did not flash the system.img correctly when I first went through the process? Is this possible?
