So i got this error after doing the update to Virtuous 1.0.2 or w.e the most modern version offered by Rom manager...
phone worked last night, did a restart this am, and thats what came up...
now it says i have to wipe the data part partition or my device will be unstable.
So is this just telling me to do a full wipe? So go into clockwork, re load the virtuous, and do a full wipe while doing it?
is that the easiest way to fix this.... also any idea why this happened?
Don't know why that happened but try fixing permissions. Instructions here

maybe try fixing permissions before flashing the rom?
Thinking perhaps it isn't being allowed access to that area to wipe it.
Just guessing though.

make_extf4fs failed on /dev/block/mmcblkOp2
http://forum.xda-developers.com/showthread.php?t=1168243
looks like it is meaningless
Humm then why cant i get the ROM flash it stick? that's the only error i am getting.
The only other thing i tried was changing out the MicroSD card.. D/L the Kernal and ROM and tryed that. but same deal.
said it installed but after reboot was still the EB13 as in nothing happened.

Ok i fixed the error by CWM 3.1.0.1>Advanced> Partition SD Card>4m>512.
IDK why that worked.. was trying to fix another problem and now i do not get the errors on wipe.
but still ignoring ROM flash..
Glad you got it working, so it was something with CWM trying to do something with the SD card even though you had never set up anything on it.
Strange; you had to give in and setup a section on your sd card in order for it to work.

Ya i am not really sure why that worked?? I was just doing the old faithful try any option that looks remotely relevant.
But as soon as i did that partition and a full reset all was back on track.
IDK if that new partition is important to CWM3.1.0.1, But i am guessing so since that was the thing that fixed my problem.
But i did appreciate your input. Thanks!
What's going on here?
I rebooted the phone last night and I wake up to no service and the phone app crashing every 1.5 seconds.
I tried to gain some insight to the problem and found this:
http://forum.xda-developers.com/showthread.php?t=1260715
..Tried it but no luck, still the same problems.
Also tried formatting the cache and dalvik in recovery.
Any ideas before I go do a full wipe? (which I read might not even work...)
Thanks guys
I'm not sure there is anything you could do without a full wipe & flash besides maybe a reflash of the rom.
Also, you posted a dead link.
Thanks, link fixed.
Let me start by saying I'm new to posting on these forums. I've been flashing roms now for over 7 months and have enjoyed it immensely.
The other day I ran into a problem that I've never had before. I was running Nil's 3.5 rom which is a great rom, and installed the V6 Supercharger Script (also great) to see how it would work with the rom. I installed everything, had V6 working and was prompted by V6 to change I believe it was /data and /cache files. I thought ok there shouldn't be a problem. So I ran V6 and everything seemed to run with no problems. Then things got a bit laggy so I decided lets try and uninstall V6 and see if that helps. V6 restored my settings that were backed up, and I uninstalled V6. I decided at this point to do a complete wipe and reflash the rom. This is when I encountered my problem.
I reboot the phone into recovery mode: (Using the lastest clockworkmod recovery)
wipe data factory reset
wipe cache partition
I go under mounts and storage, format
/boot
/system
/datadata
/data
/cache
I get an error stating "error formatting /data" and "error formatting /cache".
I am unable to restore any backups that I had already made because I receive this error message. When I install a new rom, I receive bootloops. The only rom that works, is Nils 3.5 rom that is still stuck on the phone, and when that loads, nothing works properly. I cannot download apps, the apps already installed force close on me.
I have no clue on what to do. Now I am asking myself, did I brick my phone? Any help, suggestions would greatly be appreciated.
Thanks
Update:
I decided to try downgrading to the official 2.2 stock version with S-On with hopes of trying to start from scratch and now I am stuck in bootloops. The phone goes to the red eye, pauses for about 20 seconds and reboots. Any suggestions on what to do now?
Just had this issue on my wife's phone. System random rebooted after an app update and now /cache and /data won't format. Nothing posts past the white Incredible screen.
Came up with some solutions that I have yet to try, but here's what I found:
http://forum.cyanogenmod.com/topic/32272-error-formatting-data-when-restoring/
http://forum.cyanogenmod.com/topic/6433-solved-messed-up-partitions-on-internal-storage/page__st__20
http://forum.cyanogenmod.com/topic/18288-error-formating-cache-wont-boot-past-htc-screen/
Looks like basically we need to force fix the file system and push something initial onto the phone though adb.
I'll keep this tab open and post my results as I try some fixes tonight on my wife's phone.
Bad news for you guys. I had this happen a month ago and could not fix it. Someone over at the CM forums helped me try and fix the partitions in the internal memory, but we both think the hardware was the culprit. I was under warranty still and got a replacement phone.
Link
That is the thread I'm following. So far I've had moderate success with the commands posted here:
http://forum.cyanogenmod.com/topic/...hen-restoring/page__view__findpost__p__250529
that are designed to totally re-create the partitions. Attempting to format at the moment from the phone recovery with no connection to the computer. I'm hoping this isn't all hardware related.

Grr... still getting errors. Might be dead in the water.
After about a week and a half messing around with things, I have been unable to fix this issue. Luckily I was able to upgrade and went with the Rezound.
I was able to get the format data command to work from within CWM once or twice, but not consistently. I really do think my phone hardware failed. It is scary that if it happened three months later, I would have been out money.
If I'm not mistaken nils roms format the phone to a different type ext. You may want to find a way to reformat the phone to the stock ext. I may be completely wrong on this but since you currently have a brick it might be worth looking into.

I have an Htc incredible rooted 2.3.4
i have recently run an RUU utility to fix something but that was some time ago
So yesterday i installed an app "Linux Installer" and it told me to intalls something on data/local which turns on to be phone storage and i get a message saying "Low Space" long story short i tried deleting it, only way i could was by doing a format phone storage.. but then it boots up and i get the message saying "system UIDs Inconsistent" UIDS on the system are inconsistent, you need to wipe your data partition or your device will unstable and a button saying "im feeling lucky".
So i boot in the hboot menu wipe cache, dalvik and do a factory reset and problem hasnt gone away and is worse now, i cant use the buttons and every app keeps force closing. Please Help

You could try fixing permissions thru recovery but i doubt if that will help. Your best bet would be to wipe everything in mounts and storage in the recovery menu, and then restore a backup from before you tried the linux app. Also i believe amon ra recovery has an option to repair uid mismatches, but i think its much like fix permissions in cw and probably wont do you much good.
yes i have tried it "fix permission" didnt help but theres an issuse and that is that i thought a had a backup but when i tried using it says "checking MD5 sums..."
"MD5 mismatch!" Another thing im not really into roms but is there a way of installing one without actually using my phone, ive tried cyanogen 7 before and didnt like it and recommendations and a point to the right direction will appreciated.
Also when you say wipe everything from mounts and storage do you mean everything because there are a lot of thing there saying "format"

Jermaines stock plus rom (http://forum.xda-developers.com/showthread.php?t=1260994) is probably for you, it has the original look with some added features, and is very stable. You will have to use the phone to flash any rom.
Sorry should have been more clear,by wipe everything i mean everything except sdcard and emmc, which are your storage areas. This will essentialy wipe your phone clean leaving it fresh for a backup or new rom install.
wiil give it a try
Installed stock plus, no problems. Still wish to know how to fix that without having to restart all over

Me too! Had to do full wipe :'(
I also had this issue after installing a stock 2.3.4 Rom. It alerted me on the first boot up that I had mismatched UID's, then never told me again. However, many functions didn't work on the phone, and installing a different Rom fixed the problem. Sucks to have to start over, but that's part of the fun!
Probably the cleanest solution HERE

So I was having issues with MikG and my settings.... Reinstalled aosp system apps by accident over sense system apps and it kept popping up with error messages....
So I wiped everything and reflashed MikG 3.xx and it boots up... But all I see is the wallpaper and status bar. I see no clock/apps, and am never prompted with entering my google info and whatnot.
I also deleted the ROM and put it on again thinking it might be corrupt but I still get this problem. Wtf just happened? Did I just brick my phone?
Gonna try and flash my old Deck 1.3d rom and post back with results

did you factory reset wipe?
I factory reset from within amon ra and wiped each thing individually as well. Flashed the full wipe version of MikG 3.11 and it proceeded to wipe everything again. Everything installs fine, but when it boots, it just shows the MikG walpaper, and the notification bar. No apps, no clock, no new user "welcome to android" prompt either. All I can do is turn the screen on and off.... I have to pull the battery to turn the phone off.
Putting my Deck 1.3d nandroid on my sd card as I type and am going to flash in just a minute to test and see if I can even boot anything...

You sure you got the FULL WIPE version? If you flashed the UPDATE version you will get these exact same problem.
I got both.... originally thinking it was an update TO the ROM itself. (idk why lol) Decided to keep it for if I ever have to reflash the main rom due to a user error....
But no, I made sure I was flashing the full wipe version. You think I should redownload again and try it out? I had the file saved on my friends laptop and hadn't touch it since I put it on my SD card so.... figured I could just delete the one on my SD card and retransfer
Just restored my Deck 1.3d and its booting as I type. Will post back with details.

I'd do a complete wipe on everything and start from scratch. I just got a new SDcard and formatted it and did a fresh install of MikG 3.11 and it runs like no other.
Back up your SDcard to a PC and format and start fresh. I noticed a HUGE difference in function and performance although it sucked loading almost 10gigs of music back on.....lol
you think it might be an error on the sd card like a bad sector?
Just booted back into Decks.... Although the backup of my sd:ext didn't take apparently and it's stock Decks.... Kinda odd; No apps or anything. Or is that supposed to happen? Same wallpaper though....
Gonna try and reflash MikG and if it still doesnt work I'll follow your method. Would rather not as I did that a few months ago when I partitioned my SD card for a2sd lol...
Will post back with results
Redownloaded and flashed MikG full wipe. Its good now. Thanks to you anyway.
