Okay, I know you guys always need as much information as possible. I keep getting stuck on the splash screen. Sometimes it reboots/turns on/off fine. Most of the time I get stuck on the white splash screen. I was using Tmartin's version, however, I didn't want Titanium backup to be installed upon initial install, so I switched to this.
1.) rooted HW003 with Epson screen (launch day Evo)
2.) flashed AMON_RA Recovery 2.3 through hboot
3.) wiped cache and dalvik cache (twice as matter of fact, after it kept happening). Then ran FORMAT_ALL.zip from Caulkin
4.) flashed the deodexed version of the this thread:

5.) rebooted entirely to the ROM.
6.) didn't set up google, facebook, etc) skipped all that. Turned off fast boot and booted into recovery.
7.) nandroid backup
8.) wipe cache, dalvik cache
9.) installed the new radio through recovery
10.) rebooted
11.) starts fine and works fine
12.) Shut down the phone (again, with fast boot off under applications menu)
13.) waited a few minutes, and booted phone
14.) Phone boots fine to ensure boot (Get the point, how many times i ran into the splash screen issue). I am double-checking everything.
15.) This time, i set up google and facebook ONLY. Didn't even go to Market or anything yet.
16.)SHut down the phone
17.) Get f-ing stuck on splash screen!! WTF!!!
The only way I found to fix was through clearing cache/dalvik cache. But, then the phone seems sluggish even after it gets the system all loaded up.
NOTE: This doesn't happen EVERYTIME I turn off the phone. But, it's been pretty constant. It also has happened ONCE when I used the restart option from the power menu.
Any idea's or help would be greatly appreciated.

I'm running a rooted HTC Evo with Fresh Evo 3.4.0.1 on it. I understood that the reason AppBrain's fast web installer does not work is because the "Android ID" on the device is the same, and it needs to be unique...
I changed the Android ID, and now the phone only get's up to what I assume is loading the OS. It's the part where you hear "fresh" and see the penguin.
It repeats fresh over and over again, but never loads the OS.
Any ideas on what might be going on and how I can fix it?
In case anyone is interested, I figured this out. It was a bit of a process, but was able to get my system to boot again doing the following:
1. Booted into recovery mode
2. Used 'nandroid' to back up bad system (but it was the data partition that I was interested in.
3. Performed a clean wipe/factory reset (the system booted up fine)
4. From the clean reset, grabbed out of the data partition the file "/data/data/com.android.providers.settings/databases/settings.db". Put this file on my sdcard.
5. Rebooted the device and went back into recovery mode.
6. Used 'nandroid' to restore the bad system back
7. Copied the clean 'settings.db' over the bad one.
8. Rebooted and everything was fine. I had lost a bunch of settings, but other important information was all there.

OK, so I have my wife's old Incredible that was Rooted with Unrevoked. It was kept stock Rooted.
She got an Incredible 2 and I now want to sell her old Incredible. Tried to Wipe data and SD Card using the Data and Storage Settings menu. The phone acts like it is doing both things, but doesn't actually format the SD Card, and when doing a Factory Reset, the phone reboots and acts like it is wiping the data. When it boots back up, her background is still there and there are a bunch of widgets that say "Problem Loading Widget" on them. It does seem to remove some items as email accounts are reset, GMail, etc. are all wiped.
So, I reboot into Recovery to try and reflash the stock Radio, put S-On, and install the stock ROM, but no matter what I choose in Clockwork Recovery, I get a "blank" screen after choosing a menu item. For example, when I go to "Install Zip from Sdcard," It will take the input and where it would normally ask what Zip I want to install, it only shows the background of CWR. If I try to Wipe Data, where normally it would have all the menu options with "NO" and the one that says "Yes - Wipe Data," that menu never shows up.
So, I can't fully wipe the thing, and I can't seem to flash stock Boot, Recovery, etc. to get this back to stock so I can sell it.
Any ideas?
I'll try helping you out.
1) download a new Incredible recovery from dougpiston.com/ and follow his directions above the downloads.
2) reboot into hboot (you can also turn off phone then hold vol down + power). It should prompt for an update, which you should accept. Let your phone do it's thing, it may take several minutes or longer.
3) Once you have a new recovery, delete that PB31IMG recovery file.
Your recovery should work as expected now. Btw, where did you download the s-on file from?

I downloaded both the Stock recovery, flashed and got a picture of a phone with a Red Exclamation point on it. I then downloaded CWR 3.0.0.8 and flashed that, this time it goes into recovery but does the same thing the other one does: Shows the menu options but then the screen goes blank if I try to choose an option.
EDIT: Holy poop, I think I know what the issue was all along! Basically, I was thinking that recovery worked like my Thunderbolt in that the Vol +/- was how to scroll in CWR menus and the PWR button was the select. Turns out that you use the trackball to both scroll AND select menus in CWR on the DInc!
I just wiped the device in CWR and it is booting up. I already sold the phone to a friend and she told me to keep it Rooted with a stock ROM on it, so I don't have to go any further provided that it wipes properly!

So I was trying to to change ROM's and now I believe I have "soft bricked" my phone. I am able to reboot into recovery (CWM) but I did a full format (internal SD along with all system, cache, data, etc). So, needless to say I have no backup. When I enter CWM I did a full wipe and format, restarted recovery, loaded my ROM and went to reboot.
This ROM had a disclaimer that said "may return to stock", or something to that effect, so I clicked go back. It instantly rebooted anyway. I thought is was something with permissions so I went back in and fixed them but still nothing. So I dumped a different ROM onto my external card and loaded it (using the same above method...full wipe). Same.
The phone goes to post, gets to the SG3 screen then goes black and nothing. If I pull the battery and put it back I can get back inot recovery but nothing else.
What do I need to do?
Ok, so I booted into download mode and and got the recovery file from http://forum.xda-developers.com/showthread.php?t=1727443 and followed his instructions and it it booted. Don't know what I did wrong, or if I should do something now, but I'm good for the moment.
A Galaxy 3 is not a Galaxy S3. Just ticked as I'm searching for the 3.
I woke up today to a bootloop. After pulling out the battery, I cant turn it on, it was stuck at the google logo. I thought it was some rom issue or something. When I had the time, in the afternoon, I just boot to recovery, and wiped data+sd card. Turns out sd card was not wiped.
Just now, I found a way to boot it. (AFTER WIPING DATA) I used the gnex toolkit and boot with insecure image, and guess wat? My old rom boots. Everything was the way it was, and then it crashed and stuck on the google logo again.(I made sure i wiped things two to three times, data and sd) I did a few experiments:
1) Flash using ODIN
2) Flash CWM using toolkit--it changed back to twrp after reboot
3) Changing the icon on my homescreen after boot--- everything was the same again after booting
4) Flash stock image using toolkit
5) Wipe data using manual adb
6) Try to lock my bootloader
I was trying to load Lineage onto an old One M8, I had just cleared the system, cache and data partitions using TWRP. I then clicked reboot, which I now realise was a mistake. I am now stuck on the HTC logo screen, none of the physical buttons are responding, so i cannot get into bootloader etc. I can't send ADB commands to it, it won't show up on windows file explorer, I can't even turn it off. As is said this is an old phone so I am not worried about losing data, which is probably just as well at this stage!
Any advice would be appreciated!

The physical buttons will respond, but you need to hold them for a pretty long time in the current "stuck" state. Do this:
1) Hold vol up button and power button, until the phone restarts (may take about a minute in the current stuck state).
2) Once the screen goes dark to reboot (but before the HTC logo screen appears again - I think you have 5 seconds or so), let go of the buttons, and only hold the vol down button. Do not let go of vol down, until you see the bootloader screen.
3) If you fail to get to bootloader, you simply got the timing wrong. Either started pressing vol down too late, or you let go of it too early. Simply repeat steps 1 and 2, until you get to bootloader.
4) Once in bootloader, simply select recovery, which will boot TWRP. And flash Lineage as you originally intended.
A note on wiping system when flashing a ROM: I know some folks will swear by this, and some guides will tell you to do it. But it is not necessary, and as you've seen can cause some complications. Nothing that isn't easily solved, but may give you headache nonetheless (like this case here).
The ROM install script will wipe system before it installs the custom ROM. So doing it yourself is redundant. And in the case of the ROM fails to install (it happens) you are stuck without a working OS. My recommendation is only to wipe cache, Dalvik and data when flashing a custom ROM (the default wipe in TWRP).
Any perceived "benefit" from wiping system is usually a placebo. In the rare event that the custom ROM fails to boot (or some other issue), you can always go back and flash again, wiping system (as a troubleshooting measure, etc.). Of the years of flashing ROMs (dozens and dozens of times) on various Android devices, I can count maybe 2 times where I felt I needed to wipe system when flashing a custom ROM.

This can be expected when "stuck" on the logo screen. adb will only work when booted to a working OS, or alternately when booted to TWRP. Same thing with MTP access (File Explorer).
