So I've rooted and installed Cool Sense HD along with Data2ext fine. Everything installed from SD Card without a problem. When I rebooted after install it just hangs on the HTC logo.
I turned the phone off and removed the SD and it boots up fine, have gone through all the setup and can make calls etc. As soon as I put the SD card back in and start it up again it goes back to hanging on the HTC logo.
Any ideas?
Sounds like it could be due to a bad partition. Just format your sd card, partition it again, place your rom on the sd card and perform a clean flash.
This should solve your problem.
I couldn't get gparted to detect my card before so did it with a windows app. After reading your post I fiddled about with it and got it working in gparted. Repartitioned and everything working fine now!

I didn't. When I partitioned it I did a full format, wiped the phone clean and reflashed.

Recently I flashed the latest LeeDroid rom to my HTC Desire... All was working fine until the intro screen began to loop, following the thread I applied the patch to correct the problem. However now due to the many complications with Leedroid I wanted to restore my backup! But i cant as now my phone will not detect my SD card, however my PC will?
Any suggestions? Im desperate here!

It probably doesn't detect your SD card, as Leedroid, if I understand, requires specific partitions on the SD card, which I might guess you don't have. Or, you jimmied your sd card partitions, which is why you can't use it.
I would copy the sd card to the pc, partition your sd card as leedroid recommends, then copy your info back.
Now I'm not moaning about CyanogenMod here, I'm just trying to understand how I can fix the issue.
Basically, I had CyanogenMod 7 RC2 on my Desire Z. That was it.
Eventually I saw through ROM Manager that the full release of 7 was out so I downloaded, took a backup, wiped everything and did a fresh install of the new ROM.
It installed fine, but none of my apps that needed root access were getting it.
So I booted into recovery, restored my backup, root was back.
I then installed 7.0 over my RC2. That worked fine, but again root was gone.
At this point, my phone started damaging SD cards.
It suddenly started saying that my 8GB card that came with the phone was damaged. I tried to fix it on the phone, but it just said 'Checking card' and then giving up.
Putting it in an old MP3 player, it is not detected at all.
Putting it in a card reader, the card is unreadable and when I try to format it Windows says the card is 'read only' (this is not due to the reader).
I put a functional 2gb card I had lying around into my Desire Z and it broke it in the exact same way.
My question to you guys is - what could be causing this and how do I fix it because I have a 16gb card coming and I'm obviously cautious of putting it in the phone.
Also, how would I go about flashing the new Cyanogenmod 7.02 ROM if my phone keeps breaking SD cards I put into it?
This error can be because when ever you install a CyanogenMod ROM over another, It leaves files your new ROM think is useful such as data that the new ROM doesn't support. It is damaging your SD Card because it keeps attempting to fix the error but is making it worst by trying to remove system files. Your best bet is to try another ROM and see if it does the same thing and report it to CyanogenMod Developer Team if it only does this to CyanogenMod ROMs otherwise it would be just a virus on your SD Card feeding off of personal data. If it is only CyanogenMod ROMs, we will adress this issue ASAP. Hope I helped

I've since heard some people have managed to fix their SD cards by formatting them in ClockworkMod but I can't even mount it first in order to do this! Is my card truly dead?
Also, do you think the reason that CM7.0 had no root (even when it was a clean install) was due to me not 'fixing permissions'?

Just a note to say that I've managed to fix it!
Despite being unable to 'mount sdcard' in clockworkmod, I found I was able to activate USB Storage and through this I was able to format the card (Windows 7 was previously saying that the same card was 'read only' in a multitude of card readers).
I hope this helps any poor chap with the same problem.
I had this exact same problem, munched 2 of my 8GB cards, and had a 16 on the way. I somehow fixed it before the 16 came and I've never had the issue since, glad to see an explanation of it.

I had the same problem on my Samsung Fascinate with CM7. It ate my 16GB SD card to the point where the phone wouldn't recognize it, CWM wouldn't mount it, and it wouldn't show up on my computer in multiple SD readers.
I wasn't quite clear what you were suggesting when you said you were able to "activate USB Storage." I thought maybe you meant within CWM. That didn't work for me, but I was able to boot the phone up with the problem SD card in it and turn on USB storage as normal. Suddenly my computer could not only see the SD card, it could see all the files that were on it before it died! I backed everything up, formatted the card in Windows, and everything's good now. Thanks for the tip!

im running cm7 ... it wont read from computer. i tried several micro sd card readers. it just tells me it cant format. its just like what was mentioned before.
Weird, I've never had any problems with Cyanogen 7 roms. I've flashed one over another and had no issues at all, Mayb because I use SuperWipe before hand? Or maybe its a certain sd card issue???
Running Desire Z and Andromadus Beta 11 I had similar problem after I had applied remote wipe from Exchange (wanted to see if it works). Remote wipe worked: it destroyed contents of SD card and erased phone back to factory settings. After wipe mounting SD card failed and Andromadus seemed to be unable to format the card.
Solution: Booted phone to Clockwork recovery and applied advanced -> Partition SD Card.

Go into Recovery (Clockwork), go to mounts and storage, then scroll down and it should say mount USB storage.
I noticed that I had a ton of garbage on my SD card so I used the menu on the phone to erase it. It wasn't working after the erase so I plugged it into my PC. It said I needed to format the card. I formatted it as FAT32. Now my phone will boot for about 5 seconds and then reboots.
Any ideas as to what I broke?

First, try booting phone without SD card to verify the problem only happens when the SD card is inserted.
Second, I'd use SD Formatter to format the card properly. Windows doesn't always do it correctly on its own. Default settings are fine.

Had the same problem yesterday. Had to flash back to stock using LG flash tool. Not saying it's the best method but so far it fixed my issue.
I just had the same problem.
I'm now doing the unbricking flash procedure.

Follow the guide on the sticky, I had a lot of problems doing, tried 2 different computers but my phone is back.
Same thing happened to me. If you wipe the sd card from the phone you will most likely have to unbrick it.

Hello, I am on an HTC One V running PACman ROM 15.3, with the kernel JmzPrimoC+.
I started having this problem a couple weeks ago. It would randomly unmount my SD card saying 'SD card unexpectedly removed'.
This started to happen more and more often until it would say that as soon as I put the SD card in.
I did a factory reset, which helped for about 5 minutes before it started happening again. Now, more recently, it started saying 'Damaged SD card. SD card is damaged. Try reformatting it.'
I put it in my computer (which doesn't show anything wrong with it), and did a full format. It didn't help at all.
This problem is happening with both my regularly used SD card (SanDisk Ultra® microSDHC™ UHS-I 32 GB (the red and grey one)), and my ordinary SD card (8 GB class 4 Kingston).
I don't think this can be a hardware problem because when I boot to recovery, TWRP recognizes it perfectly.
But I don't either see how its a software problem, because a factory reset should have helped...
Please help me, any help will be greatly appreciated.

Wow, I screwed up really bad.
I was gonna use TWRP to flash a new rom, so I did a factory reset, and then started flashing the rom.
But in middle of flashing the rom it was interrupted. So I rebooted, rereset, and then I realized... it's not reading my card.
I can boot to bootloader and recovery, but that's it. No OS. I'm screwed.
Is there ANYTHING I can do?
Is it possible to flash a rom over USB? Or SOMEHOW get my card working again?
Just enter recovery and use adb sideload to flash a ROM.

I am facing same issue and I came across this thread while looking for solution. Here is what I tried. That resolved issue for five minutes and then damn! same issue again but someone can find possible fix using the info here.
HTC One V rooted, currently running MIUI 3.1.2 with JB 4.1.2
Sandisk 8 GB SD Card.
Firmware CWM Recovery
I was facing same issue even before I rooted my phone while Original HTC ICS 4.0 Firmware was installed. MY observation is the problem starts few months after you start using SD Card. (Insert a new SD card in your phone, it will work fine for days / months and then will start giving same error.
While I tried thousand time to remove SD card, clean slot, clean card and reinsert, it did not work. May be worked just once for few seconds. When I tried to connect phone to PC and browse SD card contents, the card was not detected.
Booted to recovery, tried to format card, not detected. Tried to mount SD card, not detected. Then tried to partition SD card in Advanced options in recovery. The available options are 128MB, 256MB, 512MB, 1024MB, 2048MB and 4096MB. Since my SD card is 8 GB, chose the largest option. Now got an option to choose SWAP partition size. Chose 256 MB. Next chose option fix permissions, you have to wait until you see message permissions fixed message on screen.
Now rebooted phone, and voila! It worked!! SD card with 3.94GB memory is detected. But what happened to my rest of 4 GB?
Tried many apps to manage SD card from Play store. Using an app called APart, was able to delete partitions on SD card and create new ones. It worked for few minutes and then again I got SD card unexpectedly removed message.
Does this info makes any sense? I am not a developer and do not know much into phones but someone who know might be able to help. Buying a new SD card is not an option here because its going to work temporarily.

Yes, tried formatting SD Card using computer. Works fine when connected using adapter. Not detected when phone is connected to computer.

I've been using CM11 recently, but I decided to switch to stock and to swap external/internal. Everything worked fine for me until one day I woke up noticing my phone is shut down, and after restart the SD card was unable to load, awaring me as of "damaged". I put my SD to to PC and another phones and any other device is able to mount it.
I tried to format the card and reinstall the stock system to phone, but still nothing. :/
Any ideas?

are you ever check your memory card with software partition tool for your pc? just check is that your sd card still have active partition? if still have you still have a hope to recover that bro?
Try boot into custom recovery if visible there. You can partition, format sdcard because all tools available there.
