My EVO keeps on going into recovery mode with red triangle on it. I have tried to push the adb push PC36IMG.zip /sdcard but when the phone reads the zip it keeps on failing with these errors radio_vz fail bootloader fail-ic , image check fail and update fail!..
I have also tried to run RUU but it keeps on giving me usb connection errors. What do I need to do get my phone up and running

What method of rooting are you using?
At what stage are you getting to that you are at this point?
additionally it is helpful for posters to post in the section that best fits there post in this case Q&A. this si so it keeps things easy to read and find which you will quickly find can be a task all its own as you root and start playing around. it isn't a huge deal that you posted wrong though you may get a few nasty remarks for it just dont let them get to you and in the future try post questions there.
lastly if you hold the vol up button and power when at the red ! does it do anything for you?

Now I am trying to get my phone up and running , apparently something went wrong with the script posted in the forum.
This has turned out to be a quite an adventure for me . The process failed at step 7 and it never worked from that step.

Don't worry. We've all had our bad rooting experiences. You're in good hands here. I'd recommend posting in the Q&A section because there will be a lot of people that can help you there (especially when it gets buisy later tonight). Breathe, you haven't bricked or destroyed your phone. You're no where close to in trouble
Are you S-off? If so sounds like your recovery didnt flash correctly.
You can try flashing a new recovery. Then if thats good just get superuser and busybox installed once your up and running.

No , its SOFF. I already copied the file to the zip and it failed with these errors

It sounds like it never flashed the rom & recovery. Could you attach the autorootlog file to a post so I can see where it went wrong?
Since you are already showing s-off it will be an easy fix. It would be better to run the RUU, for a clean flash, then running autoroot again where it will see you are s-off and finish, but you could also use the flash recovery tool in the post below to simply install a recovery.

Could you please send me the autoroot log file that was made so I can find out what went wrong and possibly fix it?
I looked at the autoroot folder and didnot find autoroot file. Before I use your script again what precautions I should take again. If you look at the error messages it looked again the flash didn't finish completely.
Is there a way that I can back up my stock rom to sd card and restore it again if something goes wrong.

i completed part 1 of root some time ago (i can run wireless tether no problem), and im trying to unlock nand (full root). i flash mtd-eng.img without a problem and when i reboot into stock recovery with PC36IMG.zip on the root of my sdcard (i have tried two different mirrors) i have a problem. the recovery sees the file and there's a blue status bar in the upper right which goes to 100% and then it briefly says something like checking or verifying, then just returns to the stock recovery menu and never does anything like flash the image or tell me to reboot. so, this has apparently failed because when i try the next step of flashing the recovery image i get a bunch of errors.
does someone know what is going wrong? apparently im an idiot or the idiot proof guide isn't so idiot proof.
Are you using the same PC36IMG.zip from part 1?
No, I'm using the joey one from the 2nd toast post and the mirror from that same post, the first reply. Tried both, both do the same thing.
When I rooted my dad's phone, I did part 1 and part 2 together on toast's method. After I completed part 1, it said to rename the PC36img file. I'm not sure if you renamed it or not, just throwing it out there. Part 2 has the engPC36img file. Two different zip files.
Sounds like you missed a step, same thing happened to me. I recommend C&P the commands from the guides. If its still giving you problems try starting over from part 1 then continue to part 2.

Heres my situation. I want to flash psfMod-bravo-0.9.7.2 to jailbreak my PS3. I rooted my phone a while back using unrevoked.
I currently have ClockworkMod 2.5.0.1 installed. The ROM I am using is default Froyo 2.2.
So, I downloaded ROM Manager in an attempt to quickly flash to psfMod but the option to "Flash Alternate Recovery" was greyed out. After some reading, people were saying to flash to the latest ClockworkMod using ROM Manager (2.5.0.7) and then reboot and it would be available to flash whatever .img I wanted.
I ran Flash ClockworkMod Recovery, it said success. I rebooted but it was still greyed out.
Here its the part that confuses me. It says in ROM Manager that I am currently on 2.5.0.7, but if I switch off my phone and go into recovery it says im still on 2.5.0.1. I don't understand. If I select "Reebot into Recovery" in ROM manager, then it actually does load 2.5.0.7...Help!
I even tried manually flashing psfMod using the SDK and it says "FAILED (remote: signature verify fail).
Please can someone tell me if I am doing something wrong, or how to fix this!
tldr: I want to flash a new recovery (CWM 2.5.0.7) but everytime I boot up it still has the old one (2.5.0.1)

Can't you use unrevoked to flash an alternate recovery? In unrevoked you can select an alternate recovery, that's how I flashed amon ra
I tried using unrevoked already.
When it reboots it goes into the bootloader, does its thing, and then goes back into android but gets stuck on the "HTC Quietly Brilliant" splash screen and sits there. Unrevoked says Waiting for reboot. I have to pull the battery out for it to do anything. If I then take it back into bootloader again, Unrevoked says "Internal Error couldn't find misc".
I'm not sure whats going on. Someone please help!

Would you be willing to flash a RUU or do the PB99IMG method? then root again? If you are then follow this.
This might work for you
WAIT, BEFORE YOU START, CREATE A GOLDCARD. IT IS VERY NECESSARY. THINGS WILL GO WRONG IF YOU DONT.
Firstly, download this file. http://dl.dropbox.com/u/13240777/Desire Downgrade.ZIP
Extract the files.
Run the win-down.bat file.
Follow on screen instructions.
After the process is completed, Head to the root of your SD card and check if there is a file named P99***IMG (FORGOT THE EXACT NAME BUT IT SHOULD BE SOMETHING LIKE THIS) there should be one.
Secondly, disconnect your phone and turn it off.
After that boot up your phone while pressing the volume down button and the power button.
The bootloader should be checking for files after awhile and once it found the file that is supposed to be in the root of your sd card, press the volume up button (NOT EXACTLY SURE, SHOULD BE THE ONE THAT SAYS YES AND CONTINUE)
It should be writing files to your phone. and replaces everything. E.G: HBOOT, RECOVERY,RADIO and such.
It should be writing files successfully and then it would ask you to reboot. Continue with the reboot and you should be having your Desire back to stock. Perfect.
PLEASE KEEP IN MIND THAT THERE ARE RISKS INVOLVED LIKE FAILURE OF WRITING TO THE HBOOT. PLEASE REMEMBER TO FOLLOW INSTRUCTIONS. MAINLY, DO NOT BLAME ME FOR ANYTHING THAT GOES WRONG.
FINALLY, YOU SHOULD BE READING EVERYTHING UP TO HERE AND THAT YOU MUST HAVE A GOLDCARD (A GOLD SD CARD)

After applying the latest OTA 2.29.405.2 I am unable to boot my phone.
The new firmware was successfully applied, because I could start the phone for the first time.
After I shut it down again it won't got past the HTC logo.
I can get start the hboot but I if i select fastboot or recovery I get stuck again with the HTC logo.
When i select the bootloader "I get no image or wrong image."
I did not root my phone.
Please help.
For a start, unless you have a PB99IMG.zip file on your sd card you will always get that message in the bootloader. It's just telling you it can't find any of the files it looks for on your card so nothing wrong there.
First thing I'd do is try manually reflashing the OTA update. you can get the OTA ZIP file here:

Rename it update.zip and put it on the root of your SD card. Then boot into recovery (if you manage to) and flash it.
If not, go into Fastboot and clear storage (wipe). This should sort you out. If not, download the 2.29.405.2 RUU from my signature and flash that in fastboot and it should see you right if you're still having issues

What didn't work? What I put on my previous post is exactly what you need to do
I downloaded the file and renamed it ot update.zip and copied it to the root of my sdcard. when i select recovery it gets stuck in htc logo.
Then I've cleared storage/user settings and tried again, same thing.
So now do the last thing I suggested
this happened to me also. i downloaded the RUU and just did it via the pc. lost everything i did not backup, but that's my fault. all contacts were on google and and i used appbrain to pull down all my installed apps.
ok, i finally did it and it's fine now. I figured out what i was doing wrong. flashed it and it started now.

i did not go in the fastboot menu when i tried the last thing you suggested and because of this, my phone was not recognized by windows and the flash utility could not do its job.
but now it seems that my phone is restarting from time to time, somewhere between 5-10 minutes.
Might wanna do a hard reset just to make sure things flashed correctly

This is the first time I tried to root my phone and install ICS on it. The phone was on 2.3.4 and then I tried to root it using unrevoked tool. Initially it was the latest version of the unrevoked and hence it did not allow me to root it. I went for one lower version and the phone was rooted successfully.
After that I followed the instructions from the androidlookout website.
All the steps went fine. When installed zip files, it did say that file was installed successfully. Now when I am trying to reboot after that, all I get is the first screen with "htc Incredible" written and it does not go ahead. I waited for hours but it did not move. Then I pulled out the battery and tried using the hard reset using Power and Volume Down. I selected option to reboot now and again stuck at the same screen.
I believe the phone is not bricked as it is still allowing me to go to the recovery menu. Any help would be truly appreciated. I am sure I might have messed up somewhere.

You don't say anything about the ICS version you attempted to flash. I suggest you try flashing another rom such as the reliable Nil's 2.1. Then if that is up running save a nandroid copy of that and try flashing the IC-dinc AOKP or Kang roms.
Pull the battery, press volume down and power button. You should be at the bootloader screen and it should say S-off if you are rooted. On the menu, go to recovery and press the power button, it should then reboot to the CWM recovery menu. You can then go to mounts and select USB mount to get the nil's (or other rom) that you downloaded to your computer and transfer it to the root of the sd card; then go back to the main menu and do a factory reset; go back to mounts, and format system and boot; then flash your rom and reboot. It should work.

That being said, wipe the phone from recovery, and reflash a rom.
It sounds like you might have gone ahead with flashing before achieving S-OFF. Here is a good guide on how to downgrade to 2.2 and use unrevoked to get S-OFF. As always be sure to make a nandroid backup before you downgrade. I followed the steps in the guide to a T and am now rooted, S-OFF, with ICS running
You said that can get into the recovery menu, go in wipe your stuff and reflash.

Okay it's been a few months since this has happened so I'll try to remember as best as I can; I had rooted my Nexus using WugFresh's kit, everything went fine. It was rooted for about 6 months and eventually got stuck in a boot loop (no clue how this happened, I hadn't installed any kernals, roms, even apps in months). I tried like 4 or 5 different methods to fix the issue; I finally was able to get the phone working again using ODIN mode, it completely restored it to stock (as far as I'm aware).
So I tried to re-root the phone; no luck, using ANY method I've been able to find. The main problem is I can't seem to get into TWRP, every time I try to it simply gives me the dead android with the red exclamation point. I'm not an advanced user by any means, but I'm not totally clueless and I can follow directions. If anyone has any advice for me on this I'd GREATLY appreciate it, I have an excellent 4G signal where I live and the only other internet option is AT&T basic DSL so I'd really like to be able to use my wifi hotspot again. Thanks in advance for your time, I appreciate anyone who takes the time to try to help.

it sounds like you have root but cant flash a recovery, let me know if im wrong but if this is the case I would just download goo manager from the market and go into settings and flash a custom recovery and that should fix your recovery problem but sometimes you have to download the recovery twice ive noticed that in the past, let me know if this works for you or you could go through the longer process above but your fix may be very simple

I thought that too, but I tried installing super user and a couple of other root-only apps and it said that my device wasn't rooted. =P

This is driving me insane. I'm trying this method; I followed the directions step by step, but my phone is showing that it is unlocked already (when I entered the fastboot oem unlock command in CP it returned FAILED: Invalid Command, but it is showing as unlocked in bootloader.
Now for the weird part; I rebooted to bootloader and tried fastboot flash recovery recovery-clockwork-6.0.2.3-toroplus.img and it returned immediately FAILED: Invalid Argument. I decided to start the process over, and now I can't reboot from CP, it's saying error: device not found.
I really want to just smash this f*%&ing thing into several hundred thousand pieces right about now, so I'm going to take a break for the moment. Are there different types of bootloaders, or is it possible the my bootloader is just corrupted or something?
There are different bootloaders, but the fastboot commands should work just the same. You're on Windows i'm assuming? You could also look at efrant's guide, http://forum.xda-developers.com/showthread.php?t=1626895, as well as his 101 in the general section stickies. It does not make sense that the device would become not found, and I would try reflashing the bootloader. Don't smash the phone!
