Already tried reformatting Windows and installing any and all drivers I could find.
When I connect my EVO to my PC via the USB cable, the connection type window appears, the amber light turns on, and it shows up on my PC for ~2 seconds. Then it disappears. I have been unable to keep it connected.
It works on other PCs I've tried, just not mine for some odd reason. (Any other USB item works fine), and I've tried multiple USB ports.
Any ideas?
I had this exact issue when I was going through a non-powered USB hub. As soon as I plugged it into my computer's direct USB plug no issues. How old is the computer you are trying to plug this into? Is is USB 2.0? I'm on windows 7 64bit if that helps. Hope you get this fixed, it's annoying as hell.
Forgot to mention, this is on Windows 7 x64 as well.

I just want to flesh out some of the details here.
Is the phone charging when you plug it into the computer?
Is there any activity from your PC or the phone when you select "Disk Drive" from the USB menu? (Like maybe a USB error/not connected sound in Windows).
Are you able to access or see it through ADB using the SDK?
Does it work on any other computers?
Have you done anything recently? Did it just happen today?
usb connection not working
I'm having the same problem--my original usb cable was damaged, and I just bought a new (generic) one. Does the EVO require a special type of cable rather than just a standard micro usb?
Here's the details:
USB Debugging is enabled.
Phone is set to ask for connection type (default is disk drive).
Phone charges when plugged into the PC but the computer doesn't recognize it.
In the notification bar, there is the usb debugging icon, but not the other one that usually says charge only/disk drive, etc.
ADB doesn't see the phone.
So far I haven't been able to get to another computer to see if it does the same thing. I tried disabling usb debugging, no effect.

Ok, yesterday I got my phone replaced because the screen was damaged. I also got a new usb cable; this one specifically for the EVO. Same thing. I plug it into the computer, and it charges, but the PC doesn't recognize it, and the phone just seems to think it's plugged into an AC adapter. The phone is set to ask what to do whenever I connect it to a PC. Since this is a brand new phone and cable, I would think this would have to be a computer issue, but the PC doesn't seem to recognize that anything has even been connected.
Update: solved--it was the usb port. For some reason the usb ports on my pc case stopped working; plugged into one of the back panel ports and it works fine.
Any microsd cable will work, I'm using my palm pre's now, my htc cable went out a month ago.

Chevk device manager whole you are plugging your phone in. just in case there is some bad driver issue, it should show there.

connect ur evo to ur comp with usb.. then reboot and do fastboot should fix the problem.

Zune will not show my Samsung Focus as being connected to my computer. It shows up under device manager as Windows Phone USB and if I try to access pictures or music on the phone while connected, it says "connecting/syncing with computer" and will not let me access. So everything seems to think its connected except for Zune
It works fine on my netbook so I thought windows on this computer was messing up. So I reinstalled windows (after reinstalling zune multiple times), and the same problem. It opens up Zune as soon as I connect the phone, but it does not show my device as being connected.
Might be a computer hardware issue, but its a relatively recent build with a core i5 processor so im not sure. Any help/tips appreciated.

Yes I have tried the front ports, the back ports, and a powered USB hub. All have the same result.
I only have 1 microusb cable (the one that came with the phone) and it works fine connected to my netbook and charger. So I don't think the cable is faulty. But I ordered another one off monoprice just to be certain. I will test that once its here.
Is there an alternate program which I can use to transfer media on/off my phone instead of Zune?

have you tried disabling your firewall and/or antivirus just before connecting and seeing if things run then? if so, i'm guessing one/both of them are stopping it from syncing.
I had an issue with this before as well. Make sure the cable you are using is the actual Samsung Cable...other cables don't seem to work.
If it still doesn't work, try de-associating the phone from the zune software/perform a hard reset.
Try doing a battery pull and then restarting the phone and trying... That's what I have to do. I am running 7008

I've had this phone for about 4 months and it was working perfectly, flashing ROMs left and right. Yesterday I tried connecting my Samsung USB cable into my laptop and tried to hop on that USB storage. What my laptop is telling me when I connect it is strange. It says that it doesn't recognize the USB device. I tried re-installing the drivers, shutting both phone and laptop down and restarting.. nothing seems to work. Please help me, because without being able to connect to the laptop, my phone is going to be boring and unflashable. Thanks!

I've had this happen before with my first sgs2, i'm on my third now. Try it on another PC, with another cable, if that doesn't work I'd just send the unit back for a replacement because the USB port is probably malfunctioning.
I've had my One M8 for about 6 months and had no issues, until now. The phone is unrooted, and has never been altered in such a way.
About a week ago I noticed that when I connected my phone to the USB charger plug socket that it said "Slow Charging" in the status bar, however other than that everything seemed to be fine.
However, today I've connected the USB cable to my PC in order to copy some data, however the phone simply states that it's "slow charging" and does not recognise that it is connected to a PC. The PC does not show anything in device manager (not even an unknown device), and I have tried using multiple different drivers, USB cables, and even a completely different PC (a windows 7 one vs my windows 8.1 pc).
Whatever I've tried I cannot get the PC to even acknowledge that a device has been connected, even after twice factory resetting the phone.
Any ideas how I can get a PC to even recognise that there is something connected?

Install HTC Sync. Run it to get the drivers, then uninstall HTC Sync.
Also try to give an update
when/if you fix it and what you did so those that view your thread can learn
since more than 6 months the PC or Laptop doesn't recognize the P605, I tried to change the microusb port, tried different USB ports, PCs with no luck.
When I'm connecting the tablet to PC the tablet will charge but nothing appear in the device manager, no detection at all.

When I plugged in my Note to my 3.0 USB laptop, out comes 'Device not recognised' in the taskbar, and that's it. However, it all works well when I plug it to a computer with USB 2.0 running XP/7. A brief search shows something about incompatible default/microsoft's driver with old(er) devices on USB 3.0.
As you've mentioned several attempts above including a hardware change, I believe the problem lies with software somewhere.
I have the same problem and get an "error 43" message. My Note 5 works fine when plugged in. I must have damaged the port upon changing the battery. I wanted to upgrade to 6.0 but looks like I'm out of luck....unless I can update via update.zip on my P600
