I was attempting to flash my KFHD 8.9 back to stock (was running CM, but it was very slow), and I ran into some issues. I was using this tutorial: http://forum.xda-developers.com/showthread.php?t=2128175.
I ended up with a red screen. I was able to get past that, but now all I can get is the orange logo screen or into fastboot. I can't get into TWRP, and I also can't get ADB to work. I've read through many posts, but I didn't find anything that helped with this scenario. Any ideas?
In fastboot, I don't think adb will work but it is good that you can go into fastboot.
Before You Read Anymore:
When you were going back to stock, were you using a backup or a rom
You might have to stay on cm.

I downloaded stock from Amazon's website.
I was successful through step 3 in your tutorial. I couldn't get ADB working. Assuming that's a driver issue, I can get past that. The bigger problem is that the device is unable to mount any storage. When attempting to wipe, I receive the following:
Unable to mount '/data'
Unable to mount storage.

Hi guys, sorry to say I'm back with another problem.
I have a Desire Z, rooted and S-off. Rooted with the wiki method (downgrade version, adb, gfree).
Using clockworkmod recovery 3.0.2.4, and have made a backup of my first 1.32 boot.
However, I tried flashing a new rom, but recovery just won't take it. It will say Can't open /sdcard/rom.zip (Bad) installation aborted, with every rom I tried. And I've tried more than 10.
So I thought, no biggie, I have a nandroid backup. So I run the restore, which works fine. But upon booting the device, it gets stuck on the white HTC loading screen. I have tried this multiple times, deleting cache/data etc fully. Also I used the FULL-WIPE-VISION.zip for this purpose to no avail.
I have followed the semi-brick guide which said I had to go get a stock rom with root, so I did. But I can't load it through clockwork restore, because I get the same Bad rom error.
What's left to try?
Additional info I should give according to the semi-brick thread (http://forum.xda-developers.com/showthread.php?t=842495):
The last and only Rom I had was the stock 1.32.4xx to which I had to downgrade before I got perm root. This rom is my nandroid recovery.
The command adb devices recognizes my device.
adb remount is succesful
adb shell gives me root
I have /data and /system mounted when trying most roms (but I get the same error when I don't have it mounted)
After booting and waiting a minute when it's stuck in white screen, adb devices will not recognize my device anymore, adb remount and shell also say that the device is not found.

TL;DR: Clockworkmod recovery does not load roms, nand restore gives WSOD. How come?
EDIT: Problem was due to my budget 32GB microSDHC. Operations were successfull using the original 8GB card. Apologies for the spamtopic.

This isn't spam, and no need to apologize. Your post may be of benefit to others. If somebody has a similar issue, they may find your post, and from it find out how to fix it.

I have the GSM version of the Galaxy Nexus (fresh never booted). I wanted to root the device and install custom roms on it. I booted up the device, enabled debugging, then put the rom i wanted into the /sdcard location via adb. I then tried to follow the instructions found here (can't post links yet, sorry) "galaxynexusroot dot com galaxy-nexus-root how-to-root-galaxy-nexus-universal-guidegsmverizonsprintwindowslinuxmac" to root the phone. I did the following:
1) unlock the device (fastboot oem unlock)
2) flash recovery (fastboot flash recovery optimus_prime-touch-v5.5.0.4-6-GSM.img)
3) rebooted
When I tried to reboot the device gets to the Google screen, vibrates, then reboots. I have tried to go back to the recovery and was able to get to the optimus prime recovery system. I then tried to backup the device to both internal sdcard and to sdcard, each one errored saying "E:unknown volume for path [/emmc/clockworkmod/backup/2012-06-29.00.33.06]
Cant mount backup path."
At this point in time I am unable to do anything with the device. Is there anything I can do to fix this?

I was able to fix this by locking then unlocking the device again. Backing up the phone right now.
I've successfully rooted my Kindle Fire HD 8.9" and installed TWRP. I then put the zip file provided by Hashcode onto my device, wiped my cache and dalvik cache, and proceeded to install the new ROM.
After a successful install, I rebooted. It optimizes 65 installed apps, and then reboots again. An hour later the Cyanogen Mod logo is still spinning. I figured that was long enough to assume something went wrong. I powered down, went into TWRP and tried to mount storage so I could replace the Cm10.1 file with my backup files to go back to Stock. I'm receiving the following error when trying to mount storage though:

E:Unable to locate volume information for USB storage mode.
So I attempted using ADB Sideload, but at no point have I managed to get my Kindle recognized by the ADB server. I appear to be in some hell where my device still "works," but I have no way to put anything on it to fix it.
My last option appears to be fastboot. I am able to get into fastboot mode on the Kindle, though I don't know what to do with it. I'm familiar with all the other steps from my original Kindle Fire, but I never got this deep into it. Wanted to see if someone could give me some direction before I really break this thing.
Update: Alright, so between the FAQ (Item 11) and Hashcode's instructions on restoring backups I've managed to get my Kindle back to stock. I panicked and posted prematurely, so sorry for crapping up the forum. Shout outs to Hashcode and Soupmagnet for their posts.
Not sure what happened. My tablet is showing No OS in TWRP. I haven't installed or done anything to it recently. I just went to turn it on the other day and it wouldn't boot.
For the past hour I've unsuccessfully been trying to flash the factory images in flashboot. I tried using Wugfresh's nexus root toolkit, but I get an error when it tries to flash the images. **FAILED (remote: flash write failure)**
I've attempted to adb side load, but the internal storage fails to mount. the device is unlocked. I've also attempted to fastboot manually but I'm trying to do the system now and it's been "erasing" for about 10 minutes.
I've attempted to us TWRP to repair and it fails, it also fails to format the partition.
My device dead?

Ok. Attached is the dmesg. I was getting access is denied b/c Windows 10. Had to write the file to another folder.
When I try to run the second command I get an I/O Error.

Looking for assistance.
Will try and keep this short and simple, but basically my phone is unable to boot and gets stuck in a loop. This happened after having to hard reboot the phone after my daughter enetered kid mode on the latest ICE Rom.
I initially chose to run a custom rom after various attempts to update my phone to nougat via an ota, which was failing all the time despite another recent carrier update to fix the issue.
Unfortuantely I was hasty in doing so and never took note of my cid/mid information and running the commands via adb or fastboot won't returns anything... I'm a bit stuck.
As it stands:
Softare status: Modified
Unlocked
S-ON
I have access to recovery mode in the form of TWRP 3.1.0-0. Any attempts to install a rom fail at various stages (various roms) and any attempts to wipe and reformat also fail with "Failed to mount 'storage'", "Failed to mount 'cache'", data etc. etc.
If I attempt to boot to download mode I get a message stating 'Failed to boot to download mode'.
I looked to try and reformat using adb and pushing ext4fs but it failed. Equally I tried to work out which RUU I need to try and go back to a factory image but I'm unsure which file or img I need.
Could it be a nand issue? If it helps the phone was from EE in the uk and was running on the most recent software version prior to the nougat update.
Any help is much appreciated.
