Hey Guys. I don't think the phone is bricked (I have access to Bootloader, CWM Recovery and Download Mode).
I was running Android Revolution HD ROM and out of the blue, the phone rebooted. But from this moment, it does not boot into Android. It just reboots, shows Google Logo, show the Nexus Boot Animation and then hangs for a second and reboots.
I figured it's a boot loop. So I wiped data/factory reset through CWM, wiped Dalvik Cache and flashed Revolution ROM again. Same result. I flashed Modaco iR5 and same result.
What do I do from here? Any help offered would be greatly appreciated. Thank you in advance.

Is this an LTE or GSM device?
If it's GSM, I recommend flashing a different ROM. Probably something more stable. And do the whole thing - not just the rom, but the boot.img, etc. You can find that here: http://code.google.com/android/nexus/images.html

Hi. It's a GSM Version. I've tried flashing both Android Revolution HD and Modaco Ir5. Same Result. I can try VillainROM but I doubt that will help. (I'm doing that right now. I've just pushed VillainROM using this command: adb push VillainROM_whatever.zip to /data/media. Will try flashing that and report back).
I just tried restoring my nandroid backup and CWM restored the boot image, system and then it says 'Error While Restoring /data'.
Does that mean something is wrong with my internal storage?

I'll do that now. Oddly adb doesn't find the device when it's in bootloader mode. It finds it fine when it's in recovery mode and I'll able to push and pull stuff.
I think I'm missing fastboot drivers or something. I'll try flashing stock stuff.
I'm unable to push another ROM so I have to flash stock images. the internal storage is in read-only mode.

This is normal.ADB not working when you are in fastboot.Did you try the thread i post you? When you are in fastboot you must first go through cmd in the folder that conatains fastboot.exe and then type in cmd "fastboot devices" .
eg cd C:/root (root is the folder that conatains fastboot)
then type "fastboot devices"

Yeap. I read the thread you posted. Downloaded the zip file and extracted it. After which I rebooted into bootloader mode and ran the batch file from the extracted directory.
It flashed the stock images in bootloader mode and rebooted twice and I'm booted into Android now. Thank you very much, kind sir.
For my reference (and to learn), what could have gone wrong? The ROM was up and running fine and I titanium restored only user apps and didn't touch system apps at all. Everything was fine and a random reboot caused the phone not to boot into Android anymore. I've had 3 other Android devices before the Nexus (Galaxy S, SII and G1) and never had such an issue.

Haha amen to that. With Galaxy S and SII, I just used ODIN to flash a firmware. With Nexus, I just have to use that post. Thanks again!

Nope. Well, it was part of the ROM (Modaco and Android Revolution HD). I didn't flash it separately.

I've just experienced the exact same thing..
And the ONLY thing I've done today was install NFC Taginfo.
I noticed apps starting to crap out and figured I'd done something wrong and restarted.. got stuck in a boot loop..
went into CWM to restore an old back up and it failed with 'Error while restoring /data' (I'd gotten 'Error while backing up /data' when trying to back it up).
wiped everything (twice).. formatted everything (twice).. tried to restore and still no go.. interesting that it didn't seem to have any issue when formatting /data.
Am only running rooted stock (not even any MODs) so find this all rather peculiar.
Will try and push the stock ROM this evening when I get home.
EDIT: Used the method listed above to flash the original stock image which worked... although it did erase everything on my phone (kinda wish I'd known that before but what can you do). Would really like to know how I went from a seemingly normal phone to a not very useful paperweight!? Thankfully it's resolved.

Hey my phone is stuck on the loading screen after i tried to root my phone, if im honest i had no idea what i was doing i was just following a guide i found online and just thought it would all go alright but sadly it went terribly wrong. what i did was unlock the boot loader uses the htc website that the guide directed me to which went smoothly then i went into fastboot mode and ran the file install-insecure-windows from the folder i downloaded called r3-endeavor-insecurebootandroot. my phone restarted and the just froze at the loading screen. i have tried to run recovery and i just get a red triangle and i have also tried a factory reset and that just starts to load and then resets and is still stuck on the loading screen.

Since you have already unlocked your bootloader, you can try installing a custom recovery Like CWM or TWRP. You can then download any custom rom posted in the xda thread and flash it. I personally recommend ViperX. Your phone will be better than ever. It seems like you are new to this so if you need my help flashing then simply ask.

writing 'recovery'... INFOFormat partition SOS done.
what i don't understand is step 3 the actual rooting when i go to the bootloader and press power when on recovery it now just loads the HTC quietly brilliant screen again and i dont understand how to flash from recovery

The recovery will start after the HTC screen. In the recovery, press install and select whichever zip you want to flash.

Have you copied it from the computer??
If you have, then tap install and navigate to the folder to which you have copied it.

Maybe a bad download?
Try downloading again. If it still doesn't work, try a different recovery. Personally, I used TWRP while rooting.

Yes... However, you have to flash the boot.img from fastboot..
and before installing the rom.zip.. you have to wipe data/cache/dalvic from recovery.... Or use Super Wipe script : http://d-h.st/kG0

I think I have found a workaround for your problem.
If you have installed TWRP recovery then you will notice that their is a check box labeled 'toggle signature verification'. Make sure it is unchecked(whichever works).
If this works then great.
Otherwise I suggest that you download a custom ROM and flash it.(My personal suggestion)
Everything will be fine. Don't worry.
Steps are as follows-
•Download the ROM you want to flash
•Open the ROM zip with an archive manager and extract the boot.img file out of it and place it in your fastboot directory
•Copy the ROM to your sdcard
•Now boot into bootloader and choose recovery with your volume keys and power button
•In recovery wipe data and cache, dalvik cache, factory reset,etc
•Now flash your ROM by installing zip from sdcard
•If this is finished, reboot and reboot into bootloader again
•Connect your phone to your PC in fasboot mode (choose in bootloader with power button)
•Open a cmd in your fastboot folder with your boot.img and type these two commands:
fastboot flash boot boot.img
fastboot erase cache
Then simply reboot your phone.
You can download ROMs from the development section.

i got superuser from another source which worked like a dream and got it installed at last, then managed to flash the boot.img in the fastboot. then deleted all the data/cashe and installed venomX can finally turn on my phone after 3 days

I need some help, because I can't find any solution on my own.
Short story: I need to go back to stock rom and it doesn't work. I can't flash it via fastboot, and recovery mode doesn't work, because I can't use touch and CWM doesn't work either.
Long story:
This morning I wanted to install a Custom ROM, because I've got problems with RAM and touch lately. I heard that touch might be a software problem, so I thought this could solve it.
After several problems with installing Cyanogenmod, I finally made it, just to find out it doesn't find my sd card, which meant I couldn't load my backuped files onto my phone, which meant it was useless.
When I finally gave up on the sd card problem, I just wanted to go back to stock ROM. I also found out that touch doesn't seem to be a problem caused by software, because it doesn't even work in recovery mode.
So now I can't go back to stock ROM. I downloaded the original version from the Huawei Website.
FIRST WAY: Recovery mode.
I can't make a factory reset, because my touchscreen doesn't allow me to swipe, which is needed in TWRP. CWM doesn't work for me, I've tried it several times, but when I go to recovery mode, I only stay at the "huawei ascend" logo. So TWRP; but this doesn't work because of touch problems.
SECOND WAY: Fastboot.
This should work better. BUT:
for fastboot -w I get this error:

Hello,
i just unlocked bootloader and installed twrp and it all went fine..
until i wanted to fully wipe my device, i did this via twrp.
after 20Minuts, nothing happend, the bar was still white and gray (not even a little bit blue)
and it kept stuck on ''twrp formatting data using make_ext4fs function''.
i knew this wasn't a good idea, but i decided to turn off my phone,
and now, everytime when i turn it on on the normal way it just redirrects me to TeamWin in a bootloop.
even if i want to go to recovery mode i keep stuck in that bootloop, so there is nothing i can do right now, i only can stick at Fastboot Mode but nothing else,
Is there any way to fix this problem?

If you still have fastboot, why not flashing twrp 2.8.x.x?

i fixed recovery mode, so thats already one step further. Oefff @TheSSJ thanks for the help.
But now i get some other problems, when i try to wipe my phone i get the error like ''unable to mount /data'' and ''unable to wipe dalvik''
and all other kinds of these errors, i get the same problem when i was installing rom. isn't there anything or a way to fully wipe all this **** so i can install a fresh rom.

Seems you don't have a WW image installed, do you have a CN Software? Then you need another Software of course

Where did you buy your Phone? what does the packaging state? WW, CN, TW, etc? These 2 letters must be written in uppercase somewhere, then we will know which software to flash, there is a check where your phone's software is compared to the zip you are trying to flash:

Because of this i reflashed to twrp 3.0 and it works properly and now wiping shows no problem anymore.
I also got the WW ota zip but dont know how to install that.
Anyway, i tryed to install xaosp rom (even if my ota is still CN)
And it showed no problems at all!
BUT when i tryed to startup my phone it sticks with the asus logo and wont go any further, not a bootloop, it just sticks at the asus logo.

You could try flashing cm13 first and then xosp:
First wipe data/cache
Then flash cm13
Boot up and hope that it works
Reboot to recovery
Wipe data/cache
Flash xosp and maybe a kernel that fits (FlareM?)
Boot to system again

sorry for the kinda late answer, but yes i followed your instructions, and it works completely, after cm13 i also immediately installed gapps.
i skipped xaosp im fine with cm13

Hello guys, I recently formatted data on my phone to fix a bootloop I had. After fixing everything, SIM card and wifi was not working, I downloaded the Official Stock ROM of my phone and flashed bluetooth, modem, dsp and dtbo. The original plan was to run "flash_all_except_storage.bat" but that kept throwing errors. After I flashed those things I booted successfully and unlocked my phone. Problem is Wifi still doesn't work. So once again got into fastboot, tried running "flash_all_except_storage.bat", once again it threw an error, so I restarted my phone and ever since I can't boot into system. Whenever I restart it goes straight into fastboot, Power off and on again goes to fastboot, I can boot into TWRP, rebooting from there still goes into fastboot mode. Attempt 3 (HALF FIX): After some digging I realized after a failed attempt to run "flash_all_except_storage.bat", the boot was erased. I tried flashing only the boot.img and the phone booted up with the wifi working as well. Problem is after a reboot I go back into fastboot and I have to flash boot.img again to boot into system.

After fixing everything, SIM card and wifi was not working, I downloaded the Official Stock ROM of my phone and flashed bluetooth, modem, dsp and dtbo. The original plan was to run "flash_all_except_storage.bat" but that kept throwing errors. After I flashed those things I booted successfully and unlocked my phone. Problem is Wifi still doesn't work. So once again got into fastboot, tried running "flash_all_except_storage.bat", once again it threw an error, so I restarted my phone and ever since I can't boot into system. Whenever I restart it goes straight into fastboot, Power off and on again goes to fastboot, I can boot into TWRP, rebooting from there still goes into fastboot mode. Maybe is some motherboard issues?
I don't believe so. The problem I originally had was that after a restart the phone was booting into fastboot. But if I didn't restart the phone was working fine, cellular, wifi google services everything. So in order to fix that I flashed the full stock rom and that's how I ended up in the restart loop in the first place.

There is no volume buttons problem?
If no maybe you should reflash whole phone, fastboot, recovery, all, using edl ig.... I don't have this phone or any Snapdragon phone, so I don't really know....
Yeah I did, I used the flash_all.bat that comes with the ROM file and pretty much replaced everything. I tried to run it a couple of times and it failed mostly but I was able to run it twice I think (without changing anything, just running it again and again at some point it worked)

So then why your phone goes to fastboot?
I really don't know, after running flash_all the second time now my phone is stuck in a restart loop. Shows the MI logo and restarts. I can manually boot to recovery or fastboot but if I leave it on it's own it keeps turning on and off

It seems that this a software bug ig, can you try go to recovery and flash the rom?
I could try but I am not sure how to do that. My rom comes in a .tgz format and not .zip and all guides mention you need a .zip file

So where u downloaded the fastboot rom, there is a recovery rom too :

Be sure u download the recovery rom not fastboot.
Btw use your miui version u had
Tho I'm not 100% sure if you flashed the fastboot rom correctly, did u :
1. Open MiFlashTool : download the tool from here if you don't have it ---> https://xiaomiflashtool.com/
2. Select your rom ( make sure your path to the rom isn't long and the folder/folders don't have long names )
3. Connect your phone
4. Click Refresh ( to make your device appear)
5. And click flash

The official website on the recovery ROM stated that "This page shows a single update only" so I assumed it's just an update and not the full ROM. Ok so to fix the error : go to miflashtool folder and create a new folder called : log
Fixed the start up error, I get the screen that says "Please install driver", I press Install and I get this:

Pressing ok gets me back to the "Please install driver" screen
Well I just closed the Install driver window and flashed as is, I got this:
https://prnt.sc/Bk2eaj9w5Lq7 so presumably it failed but my devices rebooted and is now on system set up

flashing miui_APOLLOEEAGlobal_V12.5.4.0.RJDEUXM_451a2324af_11.0.zip from stock recovery is good idea, you can do this from MicroSD card to circumvent any usb issues you are obviously facing on fastboot.

This error is normal since you are not re-locking the bootloader.
If some functions are inoperative.
Try to flash the same rom (fastboot) that you had when you bought the phone, probably an A11.
Use clean all(flash_all.bat).
