I was messing with Vaulty which is an app that hides pictures and videos. I was messing with the storage location and now my Gallery is gone. All my pictures are still saved on my phone's internal storage, but I can't view them on my phone. I even have my pictures backed up on my laptop and tried transferring them to my phone again with no luck.
Do you use ROM manager?

Is the last update for the app itself. I believe that there's an update in google play that solves this problem. Chech if you have an update for it. If not you will have to go to the web page also theres a link on how to solve it. Let me find it and will edit the post

+1 Quite a bit of improvements on speed. Good find to OP.
Running the latest MIUI an couldn't get this to flash threw recovery. Install was aborted with system an data mounted an unmounted.

Install like a normal app. When you download it, and it completes select it in your notification dropdown, and it should install like a normal APK file. If not, find it in your downloads folder with a file explorer, and click it to install.

Every time I download ANY file (.zip .apk .rar etc...) from my phone, it comes up 'FAILED'. It will start the download fine and even download it to 99% but after that, it fails.
Anyone else having this problem? I have unknown sources box checked. Still no luck.
I'm rooted running AOKP 26 with Trinity aelp kernel.

It happens to me to. I keep clicking the link and it finally does it right. But I'm not downloading very large files. That would be frustrating to go to 99% and then drop.

Search questions and answer thread for "downloads unsuccessful" I HAD this same issue I figured out a work around and no issue since. (I wont be back hear so if u can't figure it out pm me ill walk u threw
I had the same problem. But what fixed it for me was clearing Download Manager and Downloads as well.

No... not deleting Downloads on SD card. Just deleting the "cached" downloads in the Downloads app. So that when you enter your Downloads app it says "No Downloads." Downloads and Download Manager are right next to each other in Manage Apps. Just clear data for both.
Usually dolphin browser handels downloads better.
Every time this happens to me I download Dolphin Browser and download it via that browser and then when its done, I uninstall dolphin

My SD card reader seems to have stopped working today. When I reboot the phone, it states it is reading the SD card, and 1 second later it is finished. Now, I can't access any data on the SD card via system programs, music, gallery, movies, etc...
I can use ES Explorer and see and use contents on the SD card, so I don't think the SD card is corrupt. How can I check my SD card reader program and see if that is the problem? Does anyone have a fix for this issue?
Edit: I am using SleeperROM 3.0.0 and the only problem I am having is the SD card reader app isn't working on boot or after unmounting/mounting the SD card. I can access all files via PC or ES Explorer.

Ok, does anyone know how to reinstall the system card reader "software" that seems to be malfunctioning? I can't even access Titanium Backup because the system won't recognize the SD card even though ES File Explorer and my PC (when USB connected) will!

Guys, I've noticed this going on a lot lately across several devices and different platforms of Android.
I think something might be wrong, as in some type of infected app we're all using.
I can't think of any other correlation between the many people suddenly experiencing this all at once across several different devices, many different ROMs, and completely different versions of Android.
I had a .nomedia file magically appear on the root of my internal storage on my Nexus several times today, and even after removing it my media would not be recognized at all, as well as the file would return every so often.
Someone should look into this.

I looked throughout the root directory and did not find any .media files.
Is there a way to "reinstall" the system SD Card reader app that autoruns when the phone reboots or the usb is disconnected after a file transfer?
Mine runs for only 1 second then states "media scan complete."
I am thinking my SD card reader app went bad today, if that is even a possibility. The CPU and ES Explorer recognize and play all files using non-system integrated apps (music, video, gallery being the system apps).
Also, all the apps installed to the SD Card are running fine.

Same here....... I downloaded paranoid android 1.9.9 and everything was fine until a reboot and all my pictures and music disappeared from my sd card...... Now mind you that I have flashed tons of roms .... But this has never happened to me ever...... The funny thing is that when I do a back up which I did minutes before my sd card went bad..... After my backup boots up I can see my pictures and music again.... But not for to long...... What can I do.........?

So even though CWM can read the SD card contents and install a zip file if needed it is a CWM problem? My CWM didn't update until today. I will reboot and see what happens.

I was having the same issue yesterday and installed a bunch of roms thinking it was CM9 but I looked and found that file strangely. Deleted after searching using my computer and POOF! all my stuff reappeared. Thanks!
Just an update. After the CWM update this morning and a reboot, the problem seems to be fixed now. The only other problem was that all my play lists on my music app were messed up. I had to restore data from Titanium Backup in order to get them to work properly. All good so far!

I'm sorry. It was Rom Manager that updated this morning, I believe. After that, a reboot, and everything worked well. I don't think it was CWM in itself. Crap, now I'm confused on what auto-updated this morning!

Either way, if I haven't accepted any updates in that last few days then when I do it will be all good. That's all that matters in the end.

If you cant boot into it the only way is to wipe everything and reflash, unfortunately

I'm an idiot, I didn't really understand what you meant. I fixed my problem.

My problem was that when I installed the wrong Google Play Store onto my phone it wasn't able to boot up to my docks.
So I just booted up as far as I could where those messages were repeating(my computer synced with my phone so I could access it's files), then I downloaded the correct one to my files.
I entered Recovery mode and then wiped /data /cache and then installed the newly downloaded gapps file.
Then rebooted. Everything worked as planned. Hope this helps someone else in the future.

I just installed Hyperdrive rls14 and Google search crashes every time I try to use it.
I've rebooted, reflashed the rom, clean flash, wiped everything, removed Google search with Rom toolbox and reinstalled it fresh from the Play store. Nothing seems to work.
I love the rom, and want to keep it. I use Googe now a ton though, so I'm stuck here.
Google search is a system app so if you use rom toolbox it will not delete it from the system. Using a root explorer go into system apps and delete it. Reboot, this reboot is very important to do. After rebooting the phone go back in using root explorer make sure it's still gone. Then redownload from play store and install.

I just noticed that S Voice doesn't work, either. Since Google search uses voice recognition also, could it be an issue with that?
Did you check the MD5 Checksum of the Rom before Flashing ? I would suggest downloading the Rom again. Check the MD5 Checksum to verify a good DL. The Full Wipe, and Flash.

You may be missing chaton or chaton v. I installed this app from the market, S-Voice calls on this app to work. You still needs to follow all the steps listed above to remove google now with a root browser. I used rom toolbox pro and that did the trick for me.
